One hundred patients who had undergone operation for duodenal ulcer (68 vagotomy and gastroenterostomy; seven vagotomy and pyloroplasty; 22 gastrectomy and three gastroenterostomy) 10 or more years previously each underwent endoscopy. Thirty-two patients with diarrhoea, on average four years following truncal vagotomy and drainage, were studied. A comparison was made with 24 patients without postvagotomy diarrhoea. The incidence of bacterial colonization of the upper small intestine was no different in the two groups, though patients with a gastroenterostomy had a significantly higher incidence than those with a pyloroplasty. Two cases of disinsertion of the papilla following closed trauma to the abdomen were treated by suction of the papilla by a Y loop associated with temporary pyloric exclusion, gastroenterostomy and a feeding jejunostomy. The postoperative course was uneventful in the two patients. This technique is therefore indicated for associated pancreatic lesions which do not require cephalic duodenopancreatectomy.

We present a case of obstructive jaundice due to duodenal amyloidosis presenting as a bulbar pseudotumor (amyloidoma). The duodenal and biliary obstruction were treated by double bypass, hepatojejunostomy and gastroenterostomy. Our case suggests that surgical palliative treatment may be effective, and that anastomosis is probably safe, allowing the patient a good quality of life.

An operative approach to the problem of progressive systemic scleroderma, affecting both the esophagus and duodenum, is demonstrated in the case of a 38-year-old woman severely incapacitated by protracted vomiting. Nissen's fundoplication, antrectomy and Billroth II gastroenterostomy were performed. The need for a comprehensive balanced surgical therapeutic approach in patients suffering from scleroderma is emphasized.

A case of recurrent acute retrograde jejunogastric intussusception is described in a 45-year-old man who five years previously had had an antecolic gastrojejunostomy. Two previous haematemases imply that this complication was recurrent and self reducing an unusual and fortuitous outcome. Awareness of the possibility of acute intussusception following gastroenterostomy or partial gastrectomy is stressed.

This study comprises 39 patients with duodenal ulcer, 113 patients with partial gastrectomy, 9 with total gastrectomy and 6 patients with gastroenterostomy. After ingestion of a standard meal the intestinal contents were collected during 4 subsequent periods of 20 minutes each. The volume, pH, and the concentrations of alpha-amylase, trypsin, chymotrypsin, and lipase in the collections were determined. The pH was not below normal in patients with duodenal ulcer. The results of surgical treatment of giant duodenal ulcers in 104 patients are analysed. The peculiarities of the clinical picture and diagnosis of the disease were revealed. In 53 (51.0%) patients, the organ-perserving operations with vagotomy were performed, in 47 (45.2%)--different modifications of gastric resection, in 4 (3.8%)--gastroenterostomy. In treatment of the critically ill patients, the organ-preserving operations are preferable.

Duodenal Crohn's disease has an estimated incidence of 1% to 2% among patients with Crohn's disease. We report 89 patients with duodenal Cro … odenal obstruction. Reoperation was required in eight patients, seven of whom had had vagotomy with gastroenterostomy or subtotal gastrectomy. In two of these seven patients, marginal ulcers developed. We describe a laparoscopic technique of pyloric exclusion with gastroenterostomy and common bile duct T tube insertion for obvious perforation at endoscopic retrograde cholangiopancreatography with papillotomy. The patient was operated on immediately after diagnosis of the lesion. The postoperative sequellae were very comparable to those of elective laparoscopic common bile duct exploration. We believe this approach is interesting, especially in the current era of frequent litigation.

The results of surgery in 126 patients with benign peptic ulceration and 57 with sigmoid volvulus presenting at a hospital in N India are discussed. Complications of ulceration, apart from stenosis, were rare and most patients underwent vagotomy and gastroenterostomy. Gangrenous sigmoid volvulus was mainly treated by resection and immediate anastomosis, while non-gangrenous volvulus was treated by colopexy. The aetiology of both conditions is discussed.

A case of congenital duodenal diaphragm has been presented. The clinical features of protracted vomiting, epigastric pain and discomfort after meals with a gastric splash occurring at any age, the biochemical features of profound hypokalaemia and hypochloraemic alkalosis, and the radiological demonstration of a diaphragm have been shown to be of diagnostic significance. The operation of vagotomy and gastroenterostomy was unsuccessful in this case, and a literature review suggests that duodenotomy with excision of the diaphragm is the preferred method of treatment.

Gastric volvulus usually occurs secondary to another condition such as hiatal hernia. We report a case of so-called "idiopathic' gastric volvulus where the only abnormality was laxity of the gastric attachments. Upper gastrointestinal endoscopy was unhelpful in making the diagnosis, but a barium meal clearly demonstrated the abnormality. The patient underwent a Polya gastrectomy with a retrocolic gastroenterostomy, which was thought to be the best method of ensuring gastric fixation.

Results of organo-saving operations in 807 patients (72% men, 28% women) with duodenal ulcers were analyzed. Bilateral truncal vagotomy (with pyloro-, duodenoplasty, gastroenterostomy, gastroduodenostomy, economical stomach resection) were performed in 27.1% patients, selective--in 1.4%, selective proximal--in 65.5%, anterior selective and posterior truncal vagotomy--in 6.0%. Lethality was 1.6%, long-term results were excellent and good in 75.6% cases. Satisfactory results were seen in 8.6% patients, bad results--in 15.8%.

In 82% of all cases, special steps in the double-contrast examination of the Billroth II stomach make it possible to show the afferent loop. We report about the experience on 140 of these examinations. This method was specially successful before every reoperation, in the search for recurrent ulcer or a retained gastric antrum. The complete visualization of the afferent loop gives information about the anatomic relations and often the possibility to clear up troubles after Billroth II gastroenterostomy, especially when it is not possible to endoscopy the loop.
