Facilitating Evidence-Based End-of-Life Care: A Collaborative Model Utilizing Clinical Nurse Specialist Expertise.

PURPOSE/OBJECTIVES: To deliver evidence-based, high-quality end-of-life care through a collaborative initiative led by geriatric and palliative clinical nurse specialists in partnership with an interdisciplinary team and hospital administration. [Intensive care of elderly patients : Core nursing responsibilities and central care priorities].

BACKGROUND: Older adults represent a growing and highly vulnerable intensive care unit (ICU) population due to geriatric syndromes, functional decline, and multimorbidity. These factors increase the complexity of care and require nursing interventions that support functional stability, prevent complications and address age-related needs. OBJECTIVES: To summarize key care components relevant for older ICU patients and to identify the core nursing responsibilities within these care structures. MATERIALS AND METHODS: A systematic literature search in PubMed (U.S. National Library of Medicine) a

[Constipation in the elderly. II. Prevalence and causes of the use of laxatives and experiences with volume expanders and lactulose in nursing home patients].

A study in a nursing home in the Netherlands confirmed findings from previous studies that a high percentage of geriatric patients use laxatives. This applies especially to patients taking antidepressants or hypnotics in combination with other psychotherapeutic agents or suffering from a neurological disease or to those who are rather inactive. Our study shows that it is possible for the large majority of geriatric patients using other laxatives to switch to bulk laxatives. This changing-process requires appropriate instructions and demands close attention. Eventually it will result in the nor

Geriatric co-management for cardiology patients in the hospital: A quasi-experimental study.

BACKGROUND/OBJECTIVES: Older patients admitted to cardiac care units often suffer functional decline. We evaluated whether a nurse-led geriatric co-management program leads to better functional status at hospital discharge. DESIGN: A quasi-experimental before-and-after study was performed between September 2016 and December 2018, with the main endpoint at hospital discharge and follow-up at 6 months. SETTING: Two cardiac care units of the University Hospitals Leuven. PARTICIPANTS: One hundred and fifty-one intervention and 158 control patients aged 75 years or older admitted for acute cardi

Evaluating ferric carboxymaltose administration in elderly patients: Clinical outcomes, comparative characteristics in different care settings and cost analysis. A five year study.

BACKGROUND AND OBJECTIVE: Iron deficiency anemia is prevalent in the elderly, with ferric carboxymaltose increasingly used for intravenous iron supplementation. Evaluating its safety, efficacy, and cost implications in various care settings is essential. This study assesses the clinical outcomes, adverse reactions, and cost-effectiveness of administering ferric carboxymaltose to elderly patients across different care settings over five years. METHODS: This retrospective observational cohort study included 198 elderly patients who received ferric carboxymaltose at a Geriatric Day Hospital from

Early Telephone-Based Frailty Screening With the Vulnerable Elders Survey in Adults Aged 75 Years and Older With Lung and Gynecological Cancer.

BACKGROUND: The International Society of Geriatric Oncology recommends that all older people with cancer have a geriatric evaluation before beginning treatment. OBJECTIVE: To determine the prevalence of frailty in people 75 years and older diagnosed with lung or gynecological cancer and evaluate the adaptation of standard therapeutic strategies based on frailty, following the implementation of telephone-based frailty screening with the Vulnerable Elders Survey (VES-13). INTERVENTIONS/METHODS: We performed a retrospective observational study in 362 people screened by an advanced practice nurs

Treatment Fidelity of an Evidence-Based Nurse-Led Intervention in a Proactive Primary Care Program for Older People.

BACKGROUND: In a large randomized trial, Utrecht PROactive Frailty Intervention Trial (U-PROFIT), we evaluated the effectiveness of an integrated program on the preservation of daily functioning in older people in primary care that consisted of a frailty identification tool and a multicomponent nurse-led care program. Examination of treatment fidelity is critical to successful translation of evidence-based interventions into practice. AIMS: To assess treatment delivery, dose and content of nursing care delivered within the nurse-led care program, and to explore if the delivery may have influe

Understanding the basic assessment and treatment of lower urinary tract symptoms in older women.

Lower urinary tract symptoms (LUTS) are prevalent in older women. These symptoms are often under-reported due to the potentially embarrassing nature of the symptoms and a belief that they are an inevitable consequence of ageing. LUTS such as urinary incontinence have a significant negative effect on people's quality of life; however, with the appropriate assessment and management, improvements can be achieved. This article reviews the different types of LUTS and their causes, assessment and treatment, focusing on older women. It aims to provide nurses with an understanding of LUTS so that they

[Therapy of para-articular hip fractures in the elderly].

UNLABELLED: Fractures of the hip in old patients are life-threatening events. A steady increase of that fracture type is likely in the near future. Surgical therapies and strategies have to consider the special requirements and problems of geriatric patients to achieve better results. They have to be designed together with new concepts of geriatric rehabilitation programmes. MATERIAL AND METHODS: In 1992 148 patients with hip fractures were treated at the department of traumatology, University Ulm. Follow-up parameters were daily activities in life pre- and postoperatively as well as mortalit

Achieving age-friendly cancer care through the NICHE practice model.

More than 50% of cancers occur in adults aged 70 years and older, and this population accounts for 60% of all cancer deaths. Cancer care for older adults is complex and influenced by comorbidities, age-related physiologic changes, social needs, and cognitive or functional limitations. This NICHE column addresses the alignment of the NICHE practice model and resources to advance evidence-based care of older adults with cancer in conjunction with the Institute for Healthcare Improvement's Age-Friendly 4Ms framework and international professional oncology nursing practice standards.

[Occupational burnout in geriatrics, an Echo complex].

Caregivers in nursing homes are exposed to numerous risk factors of occupational burnout. Lack of time, work overload, caring for frail people, an imbalance between hierarchical demands and the amount of decision-making power accorded to them, as well as insufficient resources: it is difficult to provide individualised care to patients in these conditions. There are, however, ways to improve this situation.
