The geriatric assessment.

The geriatric assessment is a multidimensional, multidisciplinary assessment designed to evaluate an older person's functional ability, physical health, cognition and mental health, and socioenvironmental circumstances. It is usually initiated when the physician identifies a potential problem. Specific elements of physical health that are evaluated include nutrition, vision, hearing, fecal and urinary continence, and balance. The geriatric assessment aids in the diagnosis of medical conditions; development of treatment and follow-up plans; coordination of management of care; and evaluation of

Geriatric psychosis.

This article reviews diagnostic and treatment issues which merit special consideration in the elderly psychotic patient. Psychoses are serious psychiatric disorders and can present in different forms in the elderly. The important ones include schizophrenia, psychosis in patients with dementia, psychosis in patients with depression and delusional disorder. Typical and atypical antipsychotic medications remain the treatment of choice for psychotic disorders but special considerations are warranted when prescribed to elderly patients. We have highlighted differences between younger and older adul

Geriatric trauma.

Elderly individuals not only live longer but are also more active than in the past. Accompanying this increase in activity is the number of older trauma victims. The effect of aging on response to injury is reviewed, and the initial evaluation and treatment of geriatric trauma are delineated in this article. Specific injuries of note include head and chest wall injury; pulmonary and cardiac contusion; abdominal trauma; and aortic, spinal, and musculoskeletal injury.

Geriatric mania.

Some general themes emerge from available information concerning manic states and bipolar disorders in late life: heterogeneity of both clinical features of the manic episode and of antecedent illness course; late average age at illness onset; lower rate of affective illness in families of late-onset cases; association of medical and neurologic illness and drug treatments with late-onset cases; utility of lithium salts for acute, continuation, and maintenance treatment; changes in lithium pharmacokinetics associated with aging, disease, and drugs resulting in increased plasma concentration/dos

Geriatric neurosurgery.

The "elderly," aged 65 and over, represent a rapidly growing proportion of the American population. Their percentage among the neurosurgical admissions at Henry Ford Hospital increased from 14.4% in 1978 to 22.4% in 1984. Occlusive cerebrovascular disease was the most frequent pathology seen, representing 40% of the population studied. Spinal degenerative myeloradiculopathy represented 14%, tumors 7%, trauma 5.4%, and intracranial hematomas represented 4% of the population. Vascular anomalies represented 3.3% of the patient group, with almost the same number of patients presenting with intracr

Geriatric headaches.

Headaches, although common occurrences in the elderly, can be the symptoms of a serious problem. Specific headaches found in this population need careful consideration and diagnosis. When the diagnosis is elusive, or the headache is sudden, severe, and incapacitating, a neurologist or neurosurgeon should be consulted.

[Geriatric medicine].

2017 highlights benefits of prevention. Better control of cardiovascular risk reduces the incidence of dementia and monthly high-dose vitamin D the incidence of respiratory infections in nursing home. Pre-operative geriatric assessment lowers by 20% the rate of delirium after hip-fracture surgery and complications in vascular surgery. Deleterious effects are also reported. High-dose vitamin D triples the rate of falls in supplemented residents and doesn't improve gait speed in sedentary men. Widely used in cardiovascular prevention, antithrombotic therapy is associated with an astonishing risk

Geriatric gynecology.

Most postmenopausal vaginal bleeding is benign; however, it merits diagnostic evaluation by transvaginal ultrasound or endometrial biopsy after emergency department evaluation. Patients and physicians may treat menopausal symptoms with hormone replacement therapy or other agents, such as venlafaxine or gabapentin. Hormone replacement therapy, when initiated close to the start of menopause and continued at the lowest possible dose for the shortest possible duration, carries less risk than previously believed. Pelvic organ prolapse affects millions of women and may contribute to poor body image

Geriatric pharmacology.

When faced with the geriatric dog or cat, the practitioner should consider the following: 1. Avoid using any drugs at all unless there are definite therapeutic indications. If the patient has some degree of renal insufficiency, try to select drugs that are hepatically metabolized and excreted in bile rather than eliminated by the kidneys (eg, doxycycline, tolfenamic acid). If hepatic insufficiency is present, select drugs that do not undergo metabolism before renal excretion (eg, penicillins, cephalosporins). 2. If therapeutic drug monitoring is available, tailor the drug dosage regimen to tha

Geriatric polytrauma.

BACKGROUND: compared to a younger population, the treatment of geriatric trauma victims is known to be associated with a higher mortality and morbidity. The objectives of this study are to assess the clinical course and outcome of multitrauma patients aged 65 years and over. In addition, a direct comparison between the geriatric trauma patients and the younger collective is performed. METHOD: our study includes all multitrauma patients treated between 1992 and 2001 at a major urban trauma center. The major issues of this analysis are: injury severity, injury pattern, preclinical hemodynamics

Geriatric dysthymia.

This article reviews what is known about the epidemiology, clinical characteristics, and treatment of dysthymia in the geriatric age group. Although less common in the elderly than in young adults, dysthymia may have its onset in middle or late life. Geriatric dysthymia appears to have less associated psychiatric comorbidity and closer links to severe life stresses, particularly medical illnesses, than dysthymia with early-age onset. Preliminary reports of response to antidepressant medications are encouraging in the elderly, but randomized, placebo-controlled clinical trials are needed in sam

[Geriatric dermatopharmacology].

Skin aging is associated with alterations regarding its barrier function, the amount and composition of lipids, as well as the cutaneous blood flow. All these factors affect the absorptive capacity of the skin. As a major clinical problem after topical application in aged patients, we find increased toxicity of salicylates (strong absorption, permeation and resorption, elevated free serum fraction, increased renal toxicity) and coffein (decreased hepatic clearance). Sun-ray filter creams protecting against UVB only will decrease the synthesis of vitamin D, and a prolonged exposure to the sun w

Geriatric ophthalmology.

The threat of vision loss increases with age owing to a combination of the accumulation of untreated or partially managed eye disease (for example, immune-mediated keratoconjunctivitis) as well as the late-in-life onset of certain ocular disorders such as lens subluxation/luxation syndrome and senile/age-related cataracts. In addition, the ophthalmic effects of age-related systemic diseases such as systemic hypertension can be vision threatening as well. The veterinarian must always weigh the potential ophthalmic benefits of contemporary therapy against the potential patient risk, not only whe

Geriatric gynaecology.

The menopause is the physiologic cessation of normal, cyclic ovarian function. The development of vasomotor symptoms, atrophic changes in the genito-urinary system and osteoporosis are associated with oestrogen deficiency. Osteoporosis is the most important consequence of ovarian failure because it causes considerable morbidity and mortality. There is no simple screening test for detecting postmenopausal women who are at risk of developing osteoporosis. Oestrogen replacement therapy affects lipid metabolism and may be associated with the risk of developing endometrial cancer, breast cancer and

Geriatric hypertension.

Hypertension in the elderly is a frequent occurrence and is associated with an increase in cardiovascular complications. Therapeutic drug intervention in diastolic-systolic hypertension is both safe and effective in decreasing cardiovascular morbidity and mortality. Convincing evidence supporting the value of therapeutic intervention in isolated systolic hypertension is not yet available. At present, selection of specific antihypertensive drugs remains empirical because of insufficient controlled trials in elderly hypertensives. Antihypertensive agents should be initiated in smaller dosages an

The quality of life of the postmenopausal woman can be significantly improved by active investigation and treatment of gynecologic disorders, such as postmenopausal osteoporosis, in this age group. Surgical intervention should never be excluded simply on the basis of chronologic age.
