Successful management of gingival recession with interdental attachment loss using gingival unit grafts.

BACKGROUND: Gingival recession leads to root surface exposure devoid of periodontal attachment. Recession defects with interdental attachment loss (RT2, RT3), further increase the avascular area which is critical for survival of soft-tissue grafts. Nonsubmerged grafts such as free gingival grafts and gingival unit grafts (GUGs) rely primarily on plasmatic circulation from the recipient area during the initial stages of healing for its survival. METHODS: Nineteen isolated RT2 recession defects, in the mandibular anterior region, were treated using GUG. The clinical parameters of recession dept…

Gingival Unit Graft Versus Free Gingival Graft for Treatment of Gingival Recession: A Randomized Controlled Clinical Trial.

OBJECTIVES: Gingival recession can lead to root exposure and discomfort for patients. There are various techniques for root coverage. The aim of this study was to compare the use of gingival unit graft (palatal graft including the marginal gingiva and papillae) with free gingival graft for treatment of localized gingival recession. MATERIALS AND METHODS: In this randomized controlled clinical trial, 18 bilateral localized recessions of Miller class I and II were treated in nine systemically healthy patients. Recessions were randomly treated with gingival unit graft in one side and conventiona…

The possible pathogenesis of gingival recession. A histological study of induced recession in the rat.

Stages in the pathogenesis of gingival recession were observed in rats in which pocketing had been induced by replacement of natural incisors with dental implants. Suitable conditions were thus created on the palatal aspect of the implant sockets for recession to occur. The recession process was examined at intervals by taking transverse serial sections. In the epithelial and connective tissues deep to the receding margin, morphological changes were seen which apparently lead to cleft formation and recession. These appeared to be associated with mononuclear cell infiltration of the connective …

Assessment of Creeping Attachment after Free Gingival Graft in Treatment of Isolated Gingival Recession.

BACKGROUND: This longitudinal clinical study aimed to determine the amount of creeping attachment and its relation to baseline recession depth after placement of free gingival grafts (FGG) apical to class I, II, and III Miller's recession defects. MATERIALS AND METHODS: Twenty subjects with Miller class I, II, and III gingival recession defects requiring FGG were recruited into this longitudinal clinical study. Site-specific clinical parameters (pocket depth, depth and width of gingival recession and width of keratinized gingiva apical to the recession) were recorded at baseline, 3 month and …

A novel treatment of gingival recession using a botanical topical gingival patch and mouthrinse.

BACKGROUND AND AIM: Current treatment of gingival recession (GR) is limited to surgical procedures. We describe a case series of 18 patients with GR who were treated with a botanical patch and rinse following standard conservative therapy. CASE SERIES DESCRIPTION: A total of 22 sites with GR > 1 mm were studied. Following scaling and root planing (SRP) and oral hygiene instruction, patients received two courses of patch treatment (3 days each) and botanical rinse administered twice daily throughout the treatment period. Outcome measures (GR, gingival index (GI) and gingival thickness (GT) were…

Accurate gingival recession quantification using 3D digital dental models.

OBJECTIVES: To develop and validate a method for accurate quantitative assessment of gingival recessions based on superimposition of serial 3D digital models. MATERIALS AND METHODS: Gingival recessions of mild (0.5-2 mm) and increased (3-7 mm) severity were simulated on stone casts and surface models were created. The outlines of the gingival margins of the mild (A) and severe recessions (B) were compared to the original gingival margins following 3D best fit superimposition through a gold standard technique (GS), which used intact adjacent structures, and the tested method (CC), which used s…

Restoration of gingival recession with periodontal preformed composite veneers.

Recovery from periodontal disease is not completed until the esthetic sequelae of the disease have been restored. This paper introduces a new technique to restore the natural appearance of the smile in a periodontal patient in a single appointment.

No association between gingival labial recession and facial type.

OBJECTIVE: To evaluate if facial type is a predictor of the development of gingival recession. METHODS: A cohort of 179 orthodontic patients (76 males, 101 females; age before treatment T S = 12.4 years, SD = 0.8) were followed until 5 years post-treatment (T 5 = 20.7 years, SD = 1.2). The presence of recessions was scored ('Yes' or 'No') by two raters on initial (T S), end of treatment (T 0), and post-treatment (T 5) plaster models. A recession was noted (scored 'Yes') if the labial cemento-enamel junction was exposed. The clinical crown heights were measured at T S, T 0, and T 5 as the dist…

Etiology and occurrence of gingival recession - An epidemiological study.

OBJECTIVES: Gingival recession is the term used to characterize the apical shift of the marginal gingiva from its normal position on the crown of the tooth. It is frequently observed in adult subjects. The occurrence and severity of the gingival recession present considerable differences between populations. To prevent gingival recession from occurring, it is essential to detect the underlying etiology. The aim of the present study was to determine the occurrence of gingival recession and to identify the most common factor associated with the cause of gingival recession. METHODS: A total of 7…

On the minimally invasive approach to the gingival recession.

Some aspects of pathogenesis and therapeutic approach to the gingival recession are discussed in this short communication with the example of a typical case from Russia, where excessive socket curettage after a tooth extraction resulted in a marked gingival recession. Subgingival plaque and calculus can be secondary to recession. An argument about plaque as a source of microorganisms might be plausible in case of inflammation, although various microorganisms are normal for the oral cavity. From the viewpoint of general pathology, being an atrophic condition, recession can progress due to repea…

Gingival recession in postmenopausal women with and without osteoporosis.

BACKGROUND: The periodontal disease is a complex chronic progressive inflammatory and destructive process of the tooth attachment apparatus: gingiva, alveolar bone, desmodontium, cementum. Systemic osteoporosis has a potential influence on both the periodontal and gingival inflammation indices, on the gingival recession (GR) and teeth mobility. The aim of this study was to investigate the possible relationship between the menopause osteoporosis and gingival recession, by studying the correlations between osteoporosis and gingival recession, and between the bone mineral density (BMD) at the lev…

Microsurgical treatment of gingival recession: a controlled clinical study.

When maximum precision is required in performing surgery to satisfy particular esthetic demands, a surgical microscope that enhances complete visualization of the operative field may represent a useful tool. Twenty-four cases of gingival recession (depth 2 to 5 mm) were treated by different mucogingival surgical techniques in 24 patients: 12 procedures were performed with the aid of a surgical microscope (test group), whereas the other 12 patients were treated without the microscope (control group). Recession depth, probing depth, periodontal attachment loss, and keratinized gingival tissue wi…

Gingival recession defects and guided tissue regeneration: a review.

The last decade has seen an increasing number of clinical reports on guided tissue regeneration (GTR) for reconstruction of gingival recession defects. This article reviews the value of GTR in the management of gingival recession defects based on records from such reports. Studies and case-series using nonresorbable and bioresorbable membranes, studies comparing GTR to the subepithelial connective tissue graft (CTG) procedure, and histologic reports of healing following GTR, published in the English language from 1985 to 2000, were identified using a Medline search and were included in the dat…

Alternative method of conservative esthetic treatment for gingival recession.

The health and appearance of gingival tissue play an essential role in esthetics. The esthetic function of the tissue is enhanced when it frames the restoration. With gingival recession, the effect of natural gingiva is created with pink composites or ceramics on the cervical part of the crown, as described in this clinical report. This goal depends on the absence of adverse effects on the color, shape, and health of the surrounding tissue. The new composites are more color stable and wear resistant, and the latest generation of dental bonding agents allows the bonding of composites to dentin,…
