PURPOSE OF REVIEW: Gynecomastia is a common finding in adolescent men. The primary care provider should feel equipped to thoroughly evaluate this condition and to differentiate physiologic from pathologic breast enlargement. Gynecomastia is a common problem during puberty as well as later adulthood, and is caused by hormonal imbalance at the breast tissue level. Various medications and medical conditions can cause gynecomastia and when the drug is discontinued or medical condition cured, it will frequently resolve. Medical therapy can be tried for patients with persistent gynecomastia associated-tenderness or social embarrassment prior to contemplating surgical removal of the breast tissue.

Gynecomastia is a very common symptom of an underlying hormonal imbalance in men. In addition to the three age peaks, a number of pathological conditions can also lead to an increase in the ratio of estrogen to androgen in the male, and thus provoke gynecomastia. While it is not necessary to carry out a thorough diagnostic investigation in every case of gynecomastia, the presence of an underlying tumor needs to be excluded. Should appropriate treatment of associated or causal pathology fail to resolve the problem, drug treatment or surgical options are available.

The experience with treatment of 34 children with gynecomastia is summarized. The examination included roentgenography of the Turkish saddle, definition of bone age, investigation of buccal sex chromatin, excretion of 17-ketosteroids, excretory urography against the background of retropneumoperitoneum, ultrasound investigation of the mammary glands. In juvenile gynecomastia, the mammary glands were removed, the expectant tactic being considered unwarranted. The long-term results of the surgical treatment are good.
