Autoimmune hepatitis: evolving concepts.

The liver is continuously exposed to a large antigenic load that includes pathogens, toxins, tumor cells and dietary antigens. A loss of tolerance against its own antigens may result in autoimmune hepatitis (AIH). The current paradigm holds that the disease is the result of self-perpetuating autoimmune process triggered by yet unknown factors (infections, chemicals, drugs) in a genetically susceptible host. To date, several putative hepatocellular surface antigens have been identified: P450-IID6 (recognized by the anti-LKM-1 autoantibodies) a membrane bound asialoglycoprotein receptor (a liver

Autoimmune hepatitis in children.

Autoimmune hepatitis (AIH) remains an enigmatic condition that affects children of all ages. Although much knowledge has emerged over the past four decades regarding autoimmune diseases, the lack of a spontaneous animal model has hindered a more complete understanding of the pathophysiology of AIH. Serum autoimmune markers, combined with biochemical and histologic evidence of hepatocellular injury and absent other diagnostic considerations, are necessary to diagnose AIH. The clinical spectrum ranges from asymptomatic elevation of aminotransferase levels to acute liver failure. Patients with AI

Autoimmune hepatitis in children.

AIH, ASC, and de novo AIH after liver transplantation are childhood liver diseases of an autoimmune nature. The mode of presentation of AIH in childhood is variable, and the disease should be suspected and excluded in all children presenting with symptoms and signs of prolonged or severe acute liver disease. Although corticosteroids are effective in all types of childhood AIH, patients with LKM1 have a higher frequency of acute hepatic failure and relapse after corticosteroid withdrawal than do patients with ANA/SMA. ASC occurs commonly in the absence of inflammatory bowel disease, requires ch

Genetics in autoimmune hepatitis.

Current hypotheses suggest that autoimmune hepatitis (AIH) is triggered by an environmental factor in a genetically susceptible host. Multiple genes may interact to produce a "permissive gene pool" that determines both disease risk and phenotype. Studies of type 1 AIH have focused on the major histocompatibility complex (MHC), mapping susceptibility to the DRB1 region. Three different molecular models have been proposed based on histidine at DRbeta13, lysine at DRbeta71, and valine at DRbeta86. Although the lysine-71 model has been adapted to explain data from several other studies, the DRbeta

Transplantation for autoimmune hepatitis.

Autoimmune hepatitis (AIH) is a good indication for liver transplantation, with 5- and 10-year survival rates approaching 75%. Determining the timing for transplantation remains difficult because progression to end-stage disease may be difficult to predict. After transplantation, the patients are at risk of recurrent AIH. This syndrome is not well-characterized and requires clearer definition. Introduction of corticosteroids is not always associated with arrest of disease. De novo AIH may also develop in the allograft. Although there is usually a good response to the reintroduction of corticos

Ornidazole-induced autoimmune hepatitis.

Ornidazole is a commonly prescribed antiparasitic drug for parasitic infestations, including amoebiasis, giardiasis and Trichomonas vaginalis. Several cases of antibiotic-induced autoimmune hepatitis (AIH) or AIH-like syndrome have been reported recently. In this report, we describe a 35-year-old woman with two relapses of AIH induced by ornidazole prescribed for diarrhoea and vaginal infection.

[Autoimmune hepatitis in children].

UNLABELLED: Autoimmune hepatitis is a rare chronic disease of unknown aetiology with female predominance, circulating autoantibodies, hypergammaglobulinaemia, typical major human leukocyte antigen types (HLA A1/B8/DR3) and a good response to immunosuppressive therapy. It seems that the main features in autoimmune hepatitis are a primary or secondary defective suppressor-T-cell function. Recently, we are realizing the recommendations of the International Autoimmune Hepatitis Study Group, using a combination of clinical, biochemical and serological features. METHODS: Two hundred and twenty one

Pathogenesis of autoimmune hepatitis.

Autoimmune hepatitis (AIH) is an idiopathic disorder affecting the hepatic parenchyma. There are no morphological features that are pathognomonic of the condition but the characteristic histological picture is that of an interface hepatitis without other changes that are more typical of other liver diseases. It is associated with hypergammaglobulinaemia, high titres of a wide range of circulating auto-antibodies, often a family history of other disorders that are thought to have an autoimmune basis, and a striking response to immunosuppressive therapy. The pathogenetic mechanisms are not yet f

Management of autoimmune hepatitis.

AIH is an organ-specific autoimmune disease with a variable course. It is important to distinguish AIH from other liver diseases. Early diagnosis with appropriate management improves the quality of life as well as survival rate.

Autoimmune chronic active hepatitis.

Autoimmune chronic active hepatitis, now often refereed to as simply autoimmune hepatitis has been recognised for nearly 50 years. Typically, the disease arises in a young woman who presents with an acute hepatitic illness and complains of lethargy, arthralgia, oligomenorrhoea, and fluctuating jaundice. For the purposes of clinical trials and research, guidelines for establishing the diagnosis have been published recently, but in clinical practise it is diagnosed when there is a histological picture of chronic active hepatitis together with immunological features (high levels of immunoglobulin

Autoimmune hepatitis in children.

Autoimmune hepatitis (AIH) is usually diagnosed after other viral, metabolic or drug-induced etiologies have been ruled out. Although uncommon in children, the disease can manifest itself with severe hepatocellular damage and early fibrosis. Early diagnosis and aggressive treatment may improve morbidity and decrease mortality. In other cases, liver transplantation is the final rescue for these children. We describe our experience with five children who were diagnosed with autoimmune hepatitis. Their diagnosis, medical treatment and clinical outcome are presented with a review of the literature

[Autoimmune hepatitis: pediatric aspects].

Autoimmune hepatitis (AIH), a liver inflammatory condition, is characterized by hypergammaglobulinemia, hypertransaminasemia, presence of autoantibody and periportal hepatitis revealed by histology. Usually it comes with acute onset and with bad prognosis. We describe recent updated literature for clinical manifestation and pathogenesis, principally referring to adult disease. We described the data obtained from a multicenter investigation (10 Italian center) included in the Gastroenterology and Hepatology Italian Society. The investigation collected 42 AIH cases in 18 years, this confirms tha

Late autoimmune hepatitis after hepatitis C therapy.

BACKGROUND: De-novo or reactivated autoimmune hepatitis (AIH) has been reported during or a short time after administration of interferon (IFN) in patients treated for hepatitis C (HCV). Reports on AIH during long-term follow-up after IFN treatment are scarce. PATIENTS AND METHODS: Patients diagnosed with both HCV and AIH were identified in clinical databases of four gastroenterology departments. The medical records of patients diagnosed with AIH after IFN therapy were retrospectively assessed. RESULTS: Five patients (four female, one male) with a mean age of 50 years (range: 34-59) were ide
