Pelvis and hip joint injuries as a reconstructive factors in car-to-pedestrian accidents.

The pelvic girdles of 371 pedestrian victims of road traffic accidents were evaluated during postmortem examinations. Additionally, 144 hip joints were opened. The pelvic injuries were found in 28% of the pedestrians hit exclusively in their upright position and 52% of the victims run over by a vehicle. The side of the body hit by a car was determined on the basis of the location of blood suffusions within the subcutaneous tissues and muscles of lower limbs as well as the character of injuries found in the knee and upper ankle joints. The findings were verified with the data from court records

Recovery of sciatic nerve injuries in association with total hip arthroplasty in 27 patients.

We analyzed 27 sciatic nerve injuries associated with total hip arthroplasty (THA). The patients were 23 women and 4 men, and their median age was 55 years (range, 28-75 years). In 1987 to 1995, 4,339 THAs were performed. Primary arthroplasties accounted for 3,471 and 868 were revisions. Nine patients had developmental dysplasia of the hip. Six operations were revisions. Radiologic lengthening was median 1.4 cm (range, -1 to 4.1 cm); in 8 cases, lengthening was greater than 2 cm. The median follow-up period was 58 months (range, 24-110 months). Eight patients recovered fully, the recovery of 7

Distribution and severity of injuries associated with hip dislocations secondary to motor vehicle accidents.

The injuries associated with traumatic hip dislocations and their quantitated morbidities were evaluated in this retrospective study of 38 patients. Ninety-five per cent of the patients had associated injuries severe enough for hospitalization on their own merit. Head, chest, and abdominal injuries were seen most frequently. Central dislocations were associated with the most morbidity, but were not different in distribution of injuries. Using eight different measurements of morbidity, including the Injury Severity Score, hip dislocations, in general, had more morbidity than the national injury

Hip Range of Motion Is Lower in Professional Soccer Players With Hip and Groin Symptoms or Previous Injuries, Independent of Cam Deformities.

BACKGROUND: Soccer (football) players often have hip and groin symptoms (HGS), and a previous groin injury is a risk factor for a relapse. Decreased hip range of motion (HROM) has been related to both hip and groin pain and the presence of a cam deformity. How these factors interact is unknown. PURPOSE: The first aim was to study whether HGS are associated with HROM. The second aim was to study the association of the presence of a cam deformity with HROM. Additionally, the influence of a cam deformity on the relationship between HGS and HROM was examined. STUDY DESIGN: Cross-sectional study;

The irreducible floating hip: a unique presentation of a rare injury.

A floating hip injury occurs in the setting of poly-trauma and is a rare and difficult problem to manage. Floating hip injuries require vigilant attention not only to the osseous injuries but also the surrounding compartments and soft tissue envelope. We report the case of a 35-year-old male with a lower extremity posterior wall acetabular fracture, ipsilateral femoral shaft fracture and a postero-superior hip dislocation. Closed reduction failed, necessitating an open reduction internal fixation of his hip dislocation and acetabular fracture. The patient then developed a thigh compartment syn…

Obturator artery injury after hip fracture surgery.

We present the case of a 55-year-old woman who sustained a left femoral neck fracture which was managed by insertion of a dynamic hip screw. Six hours after surgery, distension appeared in the left lower quadrant of her abdomen associated with a decline in haemoglobin concentration and clinical signs of ongoing bleeding. Computed tomography showed a left retroperitoneal mass without concomitant extravasation of contrast material. Exploratory laparotomy revealed a damaged left obturator artery with a large haematoma in its vicinity. After arterial ligation and retroperitoneal space decompressio

Injury classifications and operative approaches in hip dislocation and fractures.

During the last few decades, traumatic injuries of the hip joint have significantly increased in number, and traffic accidents have also increased. Depending on the speed, direction of the forces and the position of the femur at the moment of impact, different patterns of injury occur. Basically, they are classified as hip dislocations, dislocation fractures and acetabular fractures. These injuries have in common a high rate of concomitant lesions. Several classification systems have been developed for these injuries. Commonly, Stewart and Milford's or Levin's classification is used for disloc

[Injury to pelvic vessels in total hip replacement surgery].

Massive bleeding of the pelvic vessels during an alloarthroplastic operation on the hip joint should be suspected in cases of abnormal anatomic conditions. Therefore, the operative procedure should be meticulously planned including extensive X-ray investigations such as angiography and computer-tomography. Surgical preparation and procedures in case of vascular complications are described. Hints on operative techniques are given to prevent vascular complications during and after surgical intervention of the hip joint.

The mechanisms of severe arterial injury in surgery of the hip joint.

Vascular accidents occurring in the course of hip surgery may reach potentially catastrophic dimensions by posing an immediate and sudden threat to life and limb. This is a report of 15 cases with severe arterial injury representing 0.2--0.3% of all reconstructive hip operations performed during an 8 year period. In 6 cases perforation of either the external iliac artery, the common femoral artery of main branches of the lateral and medial circumflex femoral artery were caused by the tip of a narrow-pointed Hohmann retractor used to expose the hip joint. Other mechanisms were: intimal tear wit

The incidence and pattern of knee injury associated with dislocation of the hip.

A retrospective survey of 135 posterior dislocations and fracture-dislocations of the hip was carried out in order to define the pattern of associated knee injuries. Thirty-five patients had sustained a significant injury to the knee, of which twenty-five were clearly attributable to a direct blow on the front of the knee (fractured patella, traumatic chondromalacia, fractures of femoral and tibial condyles) and ten were compatible with valgus, varus or rotational forces (medial, lateral and cruciate ligament tears). The second type of injury has not been widely recognised but it is important

Risk of Arterial Injury During Hip Internal Fixation.

BACKGROUND: Injury to femoral arterial vessels is a risk with internal fixation of the hip. Understanding the anatomy of proximate vessels, particularly medial vessels not directly visualized, may reduce intraoperative and postoperative complications. METHODS: We analyzed 47 patients (29 men and 18 women) using a computed tomographic (CT) angiogram of the lower limbs. The mean age of our patients was 69 years (range, 46 to 88 years). The distance from the tip of the greater trochanter to the profunda femoris and its perforators within 5 mm of the medial femoral shaft was measured along the le

Imaging of hip and thigh muscle injury: a pictorial review.

Muscle injuries of the hip and thigh are a highly relevant issue in competitive sports imaging. The gold standard in diagnostic imaging of muscle injuries is magnetic resonance imaging (MRI). Radiologists need to be familiar with typical MRI findings in order to accurately detect and classify muscle injuries. Proper interpretation of the findings is crucial, especially in elite athletes. In soccer players, muscle injuries of the hip and thigh are the most common reason for missing a game. The present pictorial review deals with the diagnostic assessment, especially MRI, of muscle injuries of th

Heterotopic ossification of the hip after spinal cord injury.

OBJECTIVE: To analyse the risk factors of heterotopic ossification after spinal cord injury. METHODS: Of 104 patients with spinal cord injury, 47 (45.2%) were complicated by heterotopic ossification of the hip. These patients with heterotopic ossification were compared with 57 patients without heterotopic ossification. RESULTS: Statistical analysis showed that complete paraplegia, spasticity and pressure sore were significantly (P < 0.05) or highly significantly (P < 0.01) related to heterotopic ossification formation. The coexistence of two or three of these factors in the paraplegic patien

Total hip arthroplasty for head and neck of femur fractures secondary to civilian gunshot injuries.

BACKGROUND: The optimal treatment for intracapsular neck of femur fractures secondary to civilian gunshot injuries (GSI) remains a challenge. Surgical fixation is associated with a high failure rate due to avascular necrosis and non-union. This manuscript reports on the largest series of patients who underwent total hip arthroplasty (THA) for civilian GSI involving the hip joint. The objectives are to assess clinical outcomes and to report on complications as well as associated injuries. METHODS: All patients who had undergone THA for a civilian GSI to the hip joint at a single Level 1 Trauma
