Hospice Clinicians' Approaches to Terminal Restlessness: A Qualitative Analysis.

CONTEXT: Terminal restlessness is frequently observed in hospice, yet it lacks a consistent definition, diagnostic framework, and treatment approach. This ambiguity complicates care by increasing caregiver distress, delaying diagnosis, and creating uncertainty around appropriate treatment. OBJECTIVES: This qualitative study explored how hospice clinicians define, diagnose, and pharmacologically manage terminal restlessness in practice. METHODS: We conducted semi-structured interviews with hospice clinicians from diverse settings across Michigan. Participants were asked how they define termin

Veterinary Hospice-Enhancing Quality Without Prolonging Suffering.

Veterinary hospice provides a compassionate, patient-centered approach to end-of-life care that emphasizes comfort, dignity, and quality of life over curative intent. This article outlines the principles of hospice care, including patient selection, multimodal pain and symptom management, environmental adaptations, and integration with geriatric care. It highlights the importance of proactive communication, quality-of-life assessment tools, and euthanasia planning to guide families through difficult decisions. By supporting both patients and caregivers, veterinary hospice strengthens the human

The State of Pediatric Concurrent Hospice Care in the United States.

The Patient Protection and Affordable Care Act (ACA) required all state Medicaid programs to pay for both curative and hospice services for children and adolescents. The purpose of this Special Article report is to quantify and describe the use of concurrent care for children, including a depiction of the barriers and benefits according to community-based hospice organizations in the United States. A total of 295 hospice organizations from 50 states and Washington, DC responded to the National Alliance for Care at Home call for engagement. Almost three-quarters of responding organizations admi

Subcutaneous Dexmedetomidine for Refractory Symptoms in a Hospice Inpatient Unit.

CONTEXT: Pain, agitation, and other refractory symptoms occur commonly in palliative patients. Dexmedetomidine has anxiolytic, analgesic, and sedating effects; however, its use remains uncommon in palliative care. In New Zealand, the Hospice Waikato Inpatient Unit has increasingly used dexmedetomidine since 2019 when symptoms are refractory and deep sedation is not desirable due to patient and prognostic factors. OBJECTIVES: To describe use of subcutaneous dexmedetomidine infusions in the Hospice Waikato Inpatient Unit, noting patient demographics, treatment characteristics, safety factors, a

Exploration of delirium assessment and management in a hospice inpatient unit.

Background: Delirium is common across all palliative care settings. Guidelines exist to support the care of terminally ill people who develop delirium; yet the evidence base is limited. Recent surveys of palliative care specialists have suggested clinical practice is variable. Aim: To explore delirium assessment and management in a hospice inpatient setting. Methods: A mixed-methods study comprising a retrospective case note review of 21 patients admitted to a hospice inpatient unit and semi-structured interviews with seven hospice inpatient doctors and nurses. Results: A total of 62% of patie

Hospice Care Experiences Among Decedents With Huntington's Disease.

CONTEXT: Little is known about the hospice care experiences of those with Huntington's Disease (HD). OBJECTIVES: Our objective is to provide the first national characterization of hospice care quality for patients with HD and their families. METHODS: We used national Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey data to examine caregiver-reported experiences of hospice care. We analyzed data from 550 caregivers of patients with HD and 1,098,819 caregivers of patients without HD who died January 2016-June 2019 while receiving hospice care from 3,845 hospices n…

Utilization of Hospice Services in a Population of Patients With Huntington's Disease.

CONTEXT: Although the early and middle stages of Huntington's disease (HD) and its complications have been well described, less is known about the course of late-stage illness. In particular, little is known about the population of patients who enroll in hospice. OBJECTIVES: Our goal is to describe the characteristics of patients with HD who enrolled in hospice. METHODS: This is a retrospective cohort study of electronic medical record data from 12 not-for-profit hospices in the United States from 2008 to 2012. RESULTS: Of the 164,032 patients admitted to these hospices, 101 (0.06%) had a p…

Safe Use of Subcutaneous Diphenhydramine in the Inpatient Hospice Unit.

BACKGROUND: Diphenhydramine (DPH) is most commonly used via oral, topical, intramuscular (IM) and intravenous (IV) routes for the palliation of pruritus, treatment of extrapyramidal symptoms, management of parkinsonism and for allergic reactions. However, many hospice patients are unable to take oral medications and/or do not have IV access. Moreover, topical administration has a relatively slow rate of absorption. For this reason, in the hospice setting it is not uncommon for diphenhydramine to be administered via the subcutaneous (SC) route secondary to its ease of access, low infection rate

Managing mobility challenges in palliative and hospice care patients.

Some pet owners may have more difficulty managing a pet's mobility challenges than any other disorder. This problem is especially frustrating because the pet is often otherwise healthy. The decline in mobility is also connected to many disease processes, such as the neuropathies seen in poorly regulated diabetes and the weakness associated with degenerative myelopathy. As death nears, a decline in mobility toward becoming recumbent or moribund is expected. The progression of the mobility disorder will vary according to the disease process. As the pet's mobility declines, the burden of care wil

Unrecognised cognitive impairment in hospice patients: a pilot study.

The presence of cognitive impairment in patients who are receiving hospice care can affect numerous practical, ethical and legal aspects of their healthcare. A number of factors can contribute to cognitive impairment in these patients. Prevalence rates of cognitive impairment vary widely, but it remains under-recognised and under-treated. The aims of this pilot study were to evaluate the presence and nature of cognitive deficits in patients receiving inpatient hospice care who did not have a known current or past diagnosis of a cognitive disorder or any obvious cognitive impairments. A conveni

Should we screen hospice inpatients for orthostatic hypotension?

Orthostatic hypotension (OH) is defined as a reduction in systolic blood pressure of at least 20 mm Hg or a reduction in diastolic blood pressure of at least 10 mm Hg within three minutes of standing, with a reported prevalence of 40% in the hospice setting. Dizziness, falls and fractures have been attributed to OH and some cases of symptomatic OH might be reversible. This study explored the case for screening hospice inpatients for OH. Fifty-nine patients were screened up to five times and, whilst the detection rate was 27%, in no case was it reproducible. There was no statistical association

Predicting hospice appropriateness for patients with dementia of the Alzheimer's type.

The appropriateness of admitting individuals to hospice services is determined by assessing the individual's 6-month survival prognosis. Clinical parameters that guide clinicians in assessing prognosis, however, are not well defined in cases of dementia of the Alzheimer's type (DAT) when compared to other illnesses. The Alzheimer's-Hospice Placement Evaluation Scale (AHOPE) was developed to assess the 6-month prognosis of individuals with late-stage DAT. The purposes of this study were to estimate the reliability and predictive validity of AHOPE and to test additional demographic and clinical
