Predictors of Rehospitalization and Mortality in Diabetes-Related Hospital Admissions.

The risk factors of rehospitalization and death post-discharge in diabetes-related hospital admissions are not fully understood. To determine them, a population-based retrospective epidemiological survey was performed on diabetes-related admissions from the Polish national database. Logistic regression models were used, in which the dependent variables were rehospitalization due to diabetes complications and death within 90 days after the index hospitalization. In 2017, there were 74,248 hospitalizations related to diabetes. A total of 11.3% ended with readmission. Risk factors for rehospitali

The Mayo Clinic Hospital Mortality Reduction Project: Description and Results.

Quality improvement, regulatory, and payer organizations use various definitions of hospital mortality as clinical outcome measures. In this prospective study, the authors evaluated a multicomponent intervention aimed at reducing inpatient mortality in a multistate healthcare delivery system. The project was initiated because of a statistically nonsignificant upward trend in mortality suggested by a six-quarter rise in the observed/expected mortality ratio generated by the Vizient Clinical Data Base and Resource Manager. The design of the mortality reduction plan was influenced by the known li…

Radiologic predictors of in-hospital mortality after traumatic craniocervical dissociation.

BACKGROUND: Traumatic craniocervical dissociation (CCD) is the forcible dislocation of the skull from the vertebral column. Because most CCD patients die on scene, prognostication for those who arrive alive to hospital is challenging. The study objective was to determine if greater dissociation, based on radiologic measurements of CCD, is predictive of in-hospital mortality among patients surviving to the emergency department. METHODS: All trauma patients arriving to our Level 1 trauma center (January 2008 to April 2019) with CCD were retrospectively identified and included. Transfers and pat…

Prediction Model of In-Hospital Mortality After Hip Fracture Surgery.

OBJECTIVES: Mortality in elderly patients after the surgical treatment of hip fractures remains high. Although individual clinical risk factors have been widely studied, there has been limited research on prediction models in this population. The purpose of this study was to develop a prediction model for in-hospital mortality after hip fracture surgery and to evaluate the performance of this model. METHODS: Using the National Inpatient Sample database from 2012 to 2013, we collected data on 535,475 patients older than 50 years who had hip fracture surgery. Patient characteristics, surgery-sp…

Acute Q fever related in-hospital mortality in the Netherlands.

INTRODUCTION: A large outbreak of acute Q fever has been reported in the Netherlands with over 3500 cases from 2007 to 2009, during which 749 patients were hospitalised. In foreign cohorts, reported mortality rates in patients hospitalised with acute Q fever, ranged from 0.9 to 2.4%. We analysed mortality among hospitalised patients with acute Q fever in the Netherlands. METHODS: Physicians from hospitals in the afflicted region were asked to provide details about patients who died with a diagnosis of acute Q fever between 2007 and 2009. RESULTS: Nine patients (seven males, median age 72 yea

Ischaemic stroke--impact of renal dysfunction on in-hospital mortality.

Renal dysfunction predicts mortality in patients with myocardial infarction but less is known about the impact of renal dysfunction on in-hospital mortality after ischaemic stroke. All 361 patients (185 men, 176 women; mean age 72.1 years) with ischaemic stroke and glomerular filtration rate (GFR) <90 ml/min/1.73 m2 were followed-up. GFR was calculated according to abbreviated modification of diet in renal disease (MDRD) formula. Stroke severity was determined by National Institutes of Health Stroke Scale (NIHSS). The mean GFR was 61.5 +/- 16.6 ml/min/1.73 m2. There were 49 (13.6%) in-hospital…

Demystifying the influence of hospital characteristics on inpatient mortality rates.

Death, although not the only measure of adverse outcome, has been studied more than any other single occurrence. The authors discuss the findings of these studies and propose a conceptual framework to explain the relation between several hospital characteristics (e.g., profit status, RN ratios) and inpatient mortality rates. These studies raise the issue of whether some hospital characteristics are distal contributors and others, such as RN ratios, are more proximal explanatory variables of the mortality rate. Implications for understanding the relation of nursing's role in the array of potent

Improved hospital morbidity, mortality, and survival after the Whipple procedure.

Between 1969 and 1986, 88 patients had a Whipple resection for adenocarcinoma of the pancreas (N = 50), ampulla (N = 19), distal bile duct (N = 10), and duodenum (N = 9). Forty-nine patients were men, 39 were women, and the mean age was 58 years (range: 34-84 years). The patients were divided into two groups on the basis of two different time periods: those operated on from 1969 to 1980 (N = 41) and those operated on from 1981 to 1986 (N = 47). There were no significant differences between the two groups in terms of mean age, sex distribution, duration of symptoms before presentation, or mean

Physician and hospital factors associated with mortality of surgical patients.

Recent studies have found an inverse relationship between hospital-specific mortality rates for selected conditions and the number of patien … of primary surgeons to the discharge abstract records of patients. The study analyzes variation in hospital mortality associated with: the total volume of specific surgical procedures performed in the hospital, the volume of these procedures performed by the patient's primary surgeon, physician board certification, and other factors including patient severity of il …

Mortality from bleeding peptic ulcer. Alfred Hospital, Melbourne, 1976-1980.

Of 682 patients (755 admissions) who were admitted to hospital with bleeding peptic ulcers or erosive gastritis-duodenitis during a five-year period, 92 died (12% of admissions). In 70% of admissions to hospital, the patients were aged 50 years or older. The overall mortality rates were 13% in patients with bleeding gastric ulcers; 12% in those with duodenal ulcers; and 11% in those with gastritis-duodenitis. Fourteen per cent of patients with bleeding gastric ulcers, 21% of those with duodenal ulcers, and 4% of those with erosive gastritis-duodenitis had undergone surgery. Ten of 92 deaths (1

Maternal mortality in a major referral hospital, 1926 to 1980.

This review describes the changes in the causes of maternal deaths in a major referral hospital over a span of 55 years. There has been a significant decline in direct maternal deaths from infection, hemorrhage, and toxemia. Continued vigilance is needed since 58% of direct obstetric deaths in our hospital during the last 30 years were considered to have been preventable. Heart disease and nonobstetric infection as indirect causes of maternal deaths have decreased also. Greater effort is necessary to identify those patients with conditions that predispose to indirect deaths and to provide appr

Perinatal mortality rate at a community hospital, 1956 to 1975.

This paper is a review of the perinatal mortality rate and its components stillbirths and neonatal deaths, for the years 1956 through 1975 at Memorial General Hospital, Las Cruces, New Mexico. It is an attempt to assess major medical factors, such as specialization and cesarean section, and weigh their effect on these indices of the quality of perinatal medical care. Other factors considered in the analysis were: ages of mothers, parity, birth weights, and socioeconomic, educational, and motivational factors. The apparent impact of direct medical care factors is considerably less than would be

In-hospital mortality prognostication for cancer patients with febrile neutropenia.

BACKGROUND: Febrile neutropenia (FN) in cancer patients undergoing chemotherapy can result in life-threatening outcomes. Hence, an evaluation of associated risk factors can enable clinical surveillance as well as inform prophylactic measures. In this retrospective cohort study, we report a mortality prognostication model for chemotherapy-treated cancer patients upon a neutropenic episode. METHODS: Clinical and diagnostic data of 137 febrile neutropenia patients (>18 years) was collected from a cancer hospital, with the primary endpoint of post-hospital admission mortality within 30 days. The

Factors associated with in-hospital mortality among infective endocarditis patients.

INTRODUCTION: Despite recent advancements in the diagnosis and management of infective endocarditis (IE), it is associated with substantial morbidity and mortality. Our study objective is to determine the factors associated with in-hospital mortality in IE patients among the local population. MATERIALS AND METHODS: All IE patients who were diagnosed with definite or possible IE and were treated at Sarawak Heart Centre from 1st January 2020 to 31st December 2022 were recruited. We examined the demographic features of the subjects and the factors that contributed to in-hospital mortality. Multi…

Causes of hypercapnic respiratory failure and associated in-hospital mortality.

BACKGROUND AND OBJECTIVE: Hypercapnic respiratory failure (HRF) can occur due to severe respiratory disease but also because of multiple coexistent causes. There are few data on the prevalence of antecedent causes for HRF and the effect of these causes on prognosis, especially where study inclusion has not been biased with respect to primary diagnosis, interventions received or clinical outcome. We sought to determine the prevalence of pre-specified conditions among patients with HRF and to determine the effect of these causes on in-hospital mortality. METHODS: Cross-sectional study of patien…

Nasal intubation for trauma patients and increased in-hospital mortality.

PURPOSE: Data regarding harm from nasal intubation in trauma patients have been conflicting. This study aims to elucidate whether nasal intubation is associated with increased in-hospital mortality compared with oral intubation. METHODS: A retrospective cohort study on a nationwide trauma registry of 2004-2019 was conducted. Adult trauma patients who underwent nasal or oral intubation during initial resuscitation were selected. In-hospital mortality and lung complications were compared between nasal and oral intubations. Acute Hyperglycemia and In-Hospital Mortality in Spontaneous Intracerebral Hemorrhage.

Hyperglycemia is reported to predict worse outcome in patients with stroke, including intracerebral hemorrhage (ICH). In 83 consecutive cases of ICH at a tertiary stroke center, hyperglycemia (serum glucose >7 mmol/L) compared to normoglycemia at presentation was associated with higher rates of in-hospital mortality (51.2% vs. 26.2%, OR 2.3, CI 1.2-7.6, p = 0.02). The association with in-hospital mortality withstood adjustment for age, ICH volume, intraventricular hemorrhage, and infratentorial ICH location, but not baseline Glasgow Coma Scale. Acute hyperglycemia is associated with in-hospita…
