OBJECTIVE: To investigate the relationship between classification and prognosis of hydronephrosis in fetus.
METHODS: 226 pregnant women in their pregnant weeks 20 - 40 who were suggested to be with fetuses suffering from hydronephrosis underwent ultrasonography to observe the configuration of the kidneys, form and size of renal pelvis, extent of calyces, and thickness of renal cortex of their fetuses. The ultrasonography was conducted regularly and the outcome after birth was followed up.
RESULTS: 143 fetuses (186 kidneys) were diagnosed as with hydronephrosis of grade I with the anteroposte…

Hydronephrosis is generally considered a pathologic process, and especially in infancy is widely viewed as caused by obstruction, potentially injurious to the kidney and in need of expeditious surgical treatment. However a number of clinical and experimental studies suggest exactly the opposite: that hydronephrosis is not pathological but actually a compensating mechanism designed to protect the kidney from high pressures and renal damage. Furthermore, because hydronephrosis in the infant involves an already compliant and distensible renal pelvis it appears to be uniquely beneficial. Herein th…

Two 11-year-old boys had similar clinical courses of abdominal actinomycosis complicating ruptured appendicitis and manifesting as abdominal masses causing hydronephrosis. Cure was effected by surgery and prolonged penicillin therapy.

Three hundred and eleven patients diagnosed to have congenital hydronephrosis proven by postnatal ultrasonography in a ten-year period between 1987 and 1996 were studied retrospectively. These were classified into mild degree of hydronephrosis in H (64%), moderate in 94 (30%) and severe in 18 (6%) patients, according to first post natal sonography and depending on the measurement of the anterio-posterior transverse diameter of the pelvi-calyceal system. Significant obstructive uropathy was documented in 66 patients by Technetium-Diethylene triamine penta acetic acid renogram (Tc99m DTPA Renogr…

Renal colic symptoms may be caused by an aberrant vascular bundle that intermittently incarcerates the pyeloureteral junction. The authors describe 6 patients who were complaint free except for the period of fits and routine urological examinations showed normal findings. The patients had no subjective complaints after open surgery.

Experience is recorded with the operative treatment of congenital hydronephrosis in 56 patients, 14 to 59 years of age, over the period from January 1985 through July 1989. Of major importance for establishing correct diagnosis were excretory urography and ultrasound examination of the kidneys. Retrograde ureteropyelography was performed in only 5 patients on the operation day. Constrictions in the area of the pyeloureteral segment were the most common cause of hydronephrotic transformation of the kidneys. Because of the encouraging results, the authors give preference to the plastic operation…

Twenty-six fetuses with dilated urinary tracts were studied with serial sonograms. Eight fetuses with unilateral hydronephrosis were followed without intervention; all are well after postnatal surgical correction. Three cases of bilateral hydronephrosis resolved spontaneously before birth. Eight fetuses with bilateral hydronephrosis had evidence of poor function: Three were not treated and died shortly after birth with small lungs and dysplastic kidneys; three others had diagnostic intervention that demonstrated irreversible disease and allowed termination of the pregnancy; two had obstruction…

Sixteen cases of pyeloureteral obstruction treated in the period of 24 months are reported. The differential diagnosis, specially with Wilms' tumor must be considered. The low frequency of obstruction secondary to aberrant vessels is also important as etiological cause of pyeloureteral obstruction. This situation contributes to obstruction, but is not the main cause; the same is true with the anomalous implantation of the ureter in the renal pelvis. Within the possible etiological causes, the impairment of function of the pyeloureteral junction must be mentioned as responsible of the dilatatio

A case is presented of an unusually large abdominoscrotal hydrocele with secondary left ureterohydronephrosis owing to contiguous pressure. The technique that made the preoperative diagnosis apparent is described.

The author first recalls that the first operation to correct hydronephrosis due to ureter stenosis was performed by Trendelenburg. He then defines the causal lesion of hydronephrosis and describes its etiology. He stresses that the only procedure for correcting hydronephrosis caused by stenosis of the juxtapyelic ureter is the removal of this portion, the lesion of which it is impossible to repair or correct. He criticizes the numerous operations proposed by different authors and even the partial removal of the renal pelvis. He points out that most of the operation procedures described as new,…

INTRODUCTION: Pyeloplasty is the definitive management of ureteropelvic junction obstruction (UPJO). One of the challenging questions is when to perform pyeloplasty. We studied if improvement post-pyeloplasty in the first 3 months of life could show greater improvement in hydronephrosis than surgery at an older age.
PATIENTS AND METHODS: Patients with postnatally diagnosed UPJO and underwent pyeloplasty in the first year of life were retrospectively reviewed. We excluded patients with concomitant vesicoureteral reflux, and patients who had pyeloplasty because of UTI or missed follow-up. Patie…
