This study investigated plasma steroid concentrations 1 week after total hypophysectomy in adult female sea lampreys. Plasma estradiol significantly decreased (to 1.19 +/- 0.17 SE ng/ml) following hypophysectomy and was elevated (to 1.91 +/- 0.11 ng/ml) to a value similar to controls by a single injection of two pituitary equivalents in adult female sea lampreys in the stage of final maturation. Pregnant rhesus monkeys (Macaca mulatta) were hypophysectomized at 8-10 weeks gestation to determine effects on plasma levels of estrone (E1), estradiol-17beta (E2), and progesterone (p). HISTORY AND ADMISSION FINDINGS: A seventy-year-old woman was admitted to our hospital under suspicion of an adrenal Cushings's syndrome. Initial laboratory values showed elevated cortisol (834 nmol/l; normal: 180-640) which could not be suppressed after administration of 2 mg dexamethasone (632 nmol/l). Computed tomography of the abdomen showed a 19 x 34 mm mass in the region of the left adrenal gland.

A case is reported in which a large false aneurysm and small associated fistula formed in the cavernous carotid artery as the result of laceration at the time of transsphenoidal surgery. The clinical syndrome associated with the enlarging mass and the surgical management of the lesion itself are described. The value of the midline approach to the sella is stressed, along with anatomical variants of the carotid artery.

A 63-years-old man was found dead with the body soaking in water lying face up on a riverbank. Autopsy and diatom examination demonstrated that the cause of death was drowning. Hypophysectomy was performed in six patients with advanced carcinoma other than from breast and prostate gland to alleviate pain. Two patients received significant and lasting relief of pain; one achieved relief but died from progression of disease 5 weeks after surgery; one patient, initially relieved, had recurrence of pain 3 months later; one had about 50% relief; and one received no benefit. Growth was studied in young metamorphosed frogs, Xenopus laevis, following hypophysectomy and substitution with mammalian growth hormone (bGH or pGH), mammalian prolactin (oPRL), and thyroxine. Transsphenoidal hypophysectomy was performed in 212 consecutive patients with metastatic breast cancer: 11 died within 30 days, two of surgical complications and nine of advanced metastatic disease. A study was made in rats of the histological changes occurring in the kidney after temporary application of clamps to the renal pedicle following oestrogen, ACTH and cortisone treatments and removal of the pituitary. It was found that the tubular damage is increased to a small extent by oestrogen administration and to a more significant degree by ACTH and cortisone treatments. In rats hypophysectomized for several weeks the tubular necrosis either did not occur at all or only to an extremely mild degree.
