BACKGROUND: People with diabetes mellitus (DM) sometimes present with acute or subacute, progressive, asymmetrical pain and weakness of the proximal lower limb muscles. The various names for the condition include diabetic amyotrophy, or diabetic lumbosacral radiculoplexus neuropathies. Mesothelin is a tumour differentiation antigen that is normally present on the mesothelial cells lining the pleura, peritoneum and pericardium. It is, however, highly expressed in several human cancers including malignant mesothelioma, pancreatic, ovarian and lung adenocarcinoma. The normal biologic function of mesothelin is unknown but recent studies have shown that it binds to CA-125 and may play a role in the peritoneal spread of ovarian cancer. The Wilms' tumor gene WT1 is overexpressed in leukemia and various types of solid cancers and its product is a tumor rejection antigen. A phase I clinical trials of WT1 peptide-based cancer immunotherapy were performed with good clinical response but with only skin reaction at the injection sites of WT1 vaccine. The number of effective anti-viral agents is still limited. Therefore immunotherapy remains of importance in the treatment of viral infections. Immunoglobulins and interferons as well as a variety of immune stimulators or immune modulators are available. The efficacy of such therapy strongly depends on the kind of the virus causing the infection as well as the time of initiation of the treatment. The prophylactic and therapeutic effects of immunoglobulin preparations are well documented, whereas the efficacy of interferons and especially of immune stimulators awaits further evaluation.
