Bisalbuminemia: A Rare Incidental Finding in Monoclonal Gammopathy.

Bisalbuminemia is a rare, benign, protein anomaly presenting with two distinct peaks of albumin on serum protein electrophoresis. It reflects the presence of a normal albumin and a modified albumin in the same individual. This condition can be either hereditary or acquired. Bisalbuminemias are more frequently encountered when serum protein electrophoresis is performed with capillary technique, because this offers better resolution compared to the conventional gel electrophoresis. There are very few case reports available in the literature, showing the presence of a bifid albumin peak along wit

An Incidental Finding of Retrocaval Ureter Causing Hydroureteronephrosis.

Retrocaval ureter is a rare, congenital anomaly of the inferior vena cava. Due to an abnormal process in embryogenesis, the ureter descends posterior to the inferior vena cava. Although the anomaly can be found in pediatric patients, it most commonly manifests symptoms between the third and fourth decade of life that are typically due to ureteric obstruction, such as hydronephrosis. Retrocaval ureter can be asymptomatic and may be the reason for unreported cases and low incidence in the world. In this case, we reviewed the record of a patient with the diagnosis of retrocaval ureter incidentall

Solitary fibrous tumour of the pleura: an incidental finding.

We herein report a relatively rare case of a woman in her 30s with an incidental lung finding. She subsequently underwent resection for what was considered to be a solitary fibrous tumour of the pleura (SFTP). SFTPs are rare, slow-growing neoplasms from mesenchymal origin. These tumours are histologically characterised by proliferation of bland-looking spindled cells, arranged in a patternless pattern, with hypocellular and hypercellular areas set in a hyalinised stroma. Complete en bloc surgical excision (with margin negativity) is the treatment of choice for both benign and malignant SFTPs.

Incidental finding of non-alcoholic steatohepatitis-cirrhosis.

SUMMARY: A 72-year-old man with type 2 diabetes volunteered to participate in the control group of a clinical study. The study evaluated non-alcoholic fatty liver disease in patients with kidney disease. The patient was followed at a gastroenterology department due to Crohn's disease and post-operative bile acid malabsorption. The patient had no symptoms or biochemical findings suggesting liver disease. Surprisingly, a transient elastography (FibroScan®) suggested advanced fibrosis with a median of 16.1 kPa. A liver biopsy showed non-alcoholic steatohepatitis (NASH)-cirrhosis. The diagnosis wa

Concomitant thrombosis in patients with cellulitis as incidental finding.

BACKGROUND: Although inflammation and thrombosis are tightly connected, only 45% of patients with lower leg cellulitis receive anticoagulant therapy. Available data about the prevalence of concomitant deep venous thrombosis (DVT) in patients with cellulitis of the lower extremity is scarce and general guidelines regarding diagnosis and prevention of venous thromboembolism are missing. OBJECTIVE: We sought to determine how frequently DVT occurs as an incidental finding in patients with cellulitis and to provide recommendations for diagnostics and anticoagulant therapy. METHODS: Patients' reco

Incidental Finding of Synovial Osteochondromatosis: A Case Report.

Synovial osteochondromatosis (SOC) is a rare, benign condition of unknown etiology characterized by the formation of cartilaginous loose bodies. This often leads to early osteoarthritis with decreased range of motion and pain. Clinical presentation most often is reported as monoarticular pain affecting the knee or less commonly the hip shoulder or elbow. Diagnosis can be confirmed on x-ray imaging where the characteristic synovial bodies will be identified. Management with early debridement of the synovium will often be curative but more complex cases may require further surgical management. W

[Endosalpingiosis - an Irrelevant Incidental Finding During Laparoscopy?].

Endosalpingiosis - an Irrelevant Incidental Finding During Laparoscopy? Abstract. Endosalpingiosis refers to the ectopic presence of tubal epithelium. However, this incidental finding received little attention, although it is the second most common benign peritoneal pathology in women following endometriosis. In contrast to endometriosis, endosalpingiosis shows an increase in prevalence with age beyond the menopause. Furthermore, it does not appear to be chronically inflammatory and, according to research to this date, does not cause chronic pain or infertility. Recent epidemiological and mole

Prolidase deficiency in an infant with an incidental finding of methaemoglobinaemia.

A 4-week-old boy presented to the hospital with symptoms of diarrhoea and vomiting initially thought to be due to cow's milk allergy. He was discharged with extensively hydrolysed formula. The patient represented with worsening of symptoms with metabolic acidosis and was screened and treated for sepsis. However, his condition deteriorated further and he developed methaemoglobinaemia. He was transferred to the high dependency unit and was given two doses of methylene blue. Further investigations were carried out, including rapid trio exome sequencing, which identified a homozygous pathogenic Pe

Mucoepidermoid carcinoma of retromolar area: an incidental finding.

Salivary gland tumours can arise in any location in the oral cavity but are rather rare in the retromolar region. The majority of the salivary neoplasms arising here are malignant in nature. Mucoepidermoid carcinoma is more commonly seen in the palate. Tumours arising in the retromolar region tend to show symptoms late in onset and may be greater in extent than evident clinically. Imaging studies will help in understanding the full extent of these lesions. We report a case of mucoepidermoid carcinoma arising in a young female patient who reported for orthodontic correction. Incidental finding of thymic cyst in a newborn baby.

Thymic hyperplasia is a common finding among newborns, but cyst formation might produce mediastinal symptoms. Lethal outcome is reported when accompanied by massive intrathoracic hemorrhage. The clinical picture in our case was characterized from profuse multiple hemorrhagic foci and respiratory distress syndrome, with an unclear role of the thymic cyst.

Neuroendocrine Tumor of the Gallbladder, a Rare Incidental Finding.

BACKGROUND: Neuroendocrine tumors are a rare finding in the gallbladder. The incidence of this entity in the gallbladder is roughly 1.13 cases per 100,000 in the US, with a steady increase in the last decade. Gallbladder neuroendocrine tumors are generally asymptomatic; the majority of cases have been reported as incidental findings in specimens, resected due to secondary symptomatology, such as chronic cholecystitis. Treatment strategies are limited, in part due to the often advanced stage in which this disease presents. Furthermore, there is a restricted utility of diagnostic tools for early…

Microscopic polyangiitis: an incidental finding in a patient with stroke.

Microscopic polyangiitis (MPA) is a primary systemic vasculitis characterized by inflammation of small-sized vessels associated with the presence of anti-neutrophilic cytoplasmic antibodies. We report a case of a 39-year-old female diagnosed with microscopic polyangiitis as an incidental finding who presented with signs and symptoms of a stroke at a young age. Usually, it presents with fever, malaise, skin rash, weight loss, mononeuritis multiplex, and arthralgia/myalgia. Very rarely, it can involve meninges to cause meningeal vasculitis which can present as a febrile seizure. The most frequen

Multicystic Mesothelioma of the Omentum Presenting as an Incidental Finding.

Multicystic mesotheliomas of peritoneal origin (also termed benign multicystic mesotheliomas) are uncommon and are exceptionally rare in the omentum. Lack of familiarity of this entity could lead to overinterpretation as a malignancy or present a challenge at frozen section. We describe a case of multicystic mesothelioma arising in the omentum found incidentally at laparotomy for endometriosis. Frozen section evaluation presented a challenge. Grossly, the mass in the omentum was multicystic and mucoid, raising the concern for a mucinous neoplasm. The cysts, however, were lined by flattened cel

The Significance of Incidental Findings on Computed Tomography of the Chest.

BACKGROUND: Computed tomography (CT) of the chest has replaced lung scans and pulmonary angiography as the criterion standard for the diagnosis of pulmonary embolism (PE). Most of these examinations are negative for PE, but they frequently have incidental findings that may require further evaluation. OBJECTIVE: In order to examine common incidental findings and their possible clinical ramifications and required workup, we reviewed data from relevant studies in which chest CTs were performed and incidental findings discovered. DISCUSSION: The most common incidental findings on chest CT are pu…

Evaluation of an incidental cardiac finding in a patient with bronchitis.

UNLABELLED: CLINICAL INTRODUCTION: A 63-year-old man with HIV presented with 1 month of dyspnoea and productive cough without orthopnoea. He was afebrile with normal blood pressure, borderline tachycardia and mild pulmonary wheezing. He had exertional hypoxia requiring 4 L per minute of oxygen. No murmurs, jugular venous distention or lower extremity oedema was noted. Clinical evaluation included transthoracic echocardiography (TTE), which showed mild left ventricular hypertrophy with normal size and systolic function. The right ventricle was normal. There was mild aortic insufficiency; other

An incidental finding of severe hyperferritinaemia: a lesson to be learned.

Haemophagocytic lymphohistiocytosis is a rare, under-recognised and often misdiagnosed condition, characterised by a hyperinflammatory response to malignancy or infection. In this case, the cause was a bone marrow isolated anaplastic large cell lymphoma without radiological evidence of systemic disease, a phenomenon rarely described. We present the case of a previously fit and well 64-year-old female who presented on multiple occasions to primary and secondary care in a stable condition with an undifferentiated illness with the only consistent feature being a marked, unexplained hyperferritina

Incidental findings in the skeletal musculature on computed tomography.

OBJECTIVE: The purpose of this study was to analyze intramuscular incidental findings identified on CT in a large patient cohort. METHODS: In the time period from 2010 to 2015, a total of 44,794 patients with several diagnoses were investigated by CT. Only those patients who underwent body CT including the neck, thorax, abdomen and pelvic regions after the i.v. application of a contrast medium were involved in the study. There were 4085 patients. On further analysis, only patients with intramuscular findings (IFs) incidentally detected on CT were included. Osseous or soft-tissue lesions with …

[Hyperostosis of the internal auditory canal : An incidental finding?].

In contrast to the fairly common exostoses in the external auditory canal, hyperostoses and osteomas of the internal auditory canal are extremely rare. In this case report we present a patient with sudden right-sided sensorineural hearing loss, in whom imaging revealed hyperostosis with bilateral stenosis of the internal auditory canal. Whether the connection of such radiological findings with dysfunction of cranial nerves VII and VIII be causal or coincidental is controversially discussed in the literature. Therefore, the indication for surgical intervention should be considered with extreme
