Social drivers in lactation for mothers of very premature infants.

Provision of mother's milk for very premature infants (≤32 weeks' gestation) is a critical, modifiable factor that improves outcomes in this highly vulnerable population. Despite this, however, longstanding lactation disparities persist. Mothers who are non-Hispanic Black, non-English speaking, have lower income, lower educational attainment, or public insurance are less likely to provide mother's milk, and these inequities have remained largely unchanged over decades. For mothers of very preterm infants, sustaining lactation presents unique challenges. Mothers are typically dependent on breas

Case of COVID-19 in a 5-week-old baby.

On 31 December 2019, the first case of COVID-19, was reported in Wuhan. A public health emergency of international concern was declared on 30 January 2020 and the first case in Scotland, on 2 March. The effect of COVID-19 appears to be less in the paediatric population and there are fewer cases reported in the literature in comparison to the adult population. Here, we report a case of a previously well 5-week-old infant who presented with fever and increased sleepiness. There was no known contact with any unwell individuals. COVID-19 was identified through a septic screen work up. The infant's

[Body plethysmography in infants and small children. Respiratory function diagnosis using a special baby plethysmograph].

Successful body plethysmographic examination of infants and toddlers can now be performed by means of a special baby plethysmograph which is commercially available (Fa. Jaeger, Würzburg, Fed. Rep. of Germany). The apparatus is equipped with a heated rebreathing system and manipulation ports. Examinations require paying attention to certain potential sources of error: the consequences of necessary sedation; possible undefined leaks due to non-airtight manipulation ports; the influence of irregular breathing or increasing hyperventilation; the effect of nasal resistance; the large variation in t

Hybrid stage I palliation for hypo-plastic left heart condition without a hybrid suite: suggestions for developing nations.

Cardiac hybrid procedures are performed in modern, spacious, and highly equipped hybrid suites in developed countries. Organizing such expensive suites in countries with an emerging economy is difficult from both a financial and logistics point of view. We share our experience of safely performing a Hybrid stage I palliation procedure for Aortic atresia with ventricular septal defect on a 2-month-old infant weighing 3.35 kg using minimal resources in a conventional catheterization laboratory.

A New Protocol for Early Intervention in Infants with Cleft Palate: Language and Articulation.

PurposeChildren with cleft conditions are at increased risk of language and speech disorders. Velopharyngeal dysfunction is a common sequela of palate repair, and children may develop cleft-type speech characteristics, with consequences in the academic, social, and ultimately employment, domains. Infants with certain speech behaviors are more likely to present with increased intelligibility at school age. These behaviors could be embedded into general language development strategies and taught to infants by their caregivers. This paper describes a study protocol investigating the effects of ea

Single-pulse transcranial magnetic stimulation for assessment of motor development in infants with early brain injury.

INTRODUCTION: Single-pulse transcranial magnetic stimulation (TMS) has many applications for pediatric clinical populations, including infants with perinatal brain injury. As a noninvasive neuromodulation tool, single-pulse TMS has been used safely in infants and children to assess corticospinal integrity and circuitry patterns. TMS may have important applications in early detection of atypical motor development or cerebral palsy. AREAS COVERED: The authors identified and summarized relevant studies incorporating TMS in infants, including findings related to corticospinal development and circ

Optokinetic nystagmus: a review of pathways, techniques and selected diagnostic applications.

There are a myriad of conditions producing abnormal OKN responses. In most of these conditions the diagnosis can be made without recourse to OKN. However, in the five conditions described in this paper (suspected poor visual acuity in infants and young children, parietal lesions, subclinical internuclear ophthalmoplegia, Parinaud's syndrome, and aberrant regeneration of the third nerve) OKN can greatly facilitate the diagnosis. A review of the neural pathways of the OKN response and an explanation of the various equipment and techniques used to assess OKN are included.

The acute abdomen in the newborn.

The acute abdomen in the newborn provides challenging problems from many aspects, not only with regard to diagnosis, resuscitation and treatment, but also now with prenatal management. Most conditions are uncommon and treatment in specialist centres enables concentration of appropriate resources and expertise. Co-morbidity is common, particularly in the preterm or low birth weight infant. A multi-disciplinary team of surgeons, anaesthetists, neonatologists, radiologists, cardiologists, obstetricians, nurses, physiotherapists and other health professionals experienced in dealing with extremely

[An immediate measurement of intravesical pressure. A natural method of a urodynamic test in children with myelodysplasia].

A comparison of bladder leak point pressure values measured during conventional filling cystometry with 8 F, and then with 4-5 F catheter was performed in 20 infants with myelodysplasia. In 12 (60%) of them, the larger catheter led to artificial findings, especially in children less than one year old. This was observed in 9 (82%) of 11 infants in this age-group. The method of impromptu bladder pressure monitoring has been introduced, in order to eliminate the risk of artificial results. It is based on testing an already spontaneously filled bladder with a fine catheter without any previous ins

The role of bedside functional echocardiography in the assessment and management of pulmonary hypertension.

Pulmonary hypertension is an emergency in neonatal intensive care units with high morbidity and mortality. Its timely assessment and management is crucial for intact survival. Over the last couple of decades, there have been significant advances in management and techniques, which have resulted in improved survival. The use of neonatologist-performed echocardiography (NPE) is now increasingly utilized on neonatal intensive care units to understand the pathophysiology of the disease and to direct the treatment to the underlying cause. Its use is now established not only in cases of congenital d

The Acceptability of Clean Delivery Kits on the Atlantic Coast of Nicaragua: A Focused Ethnography.

Anecdotal reports suggest rates of puerperal sepsis/umbilical cord infection in the Southern Atlantic Autonomous Region (RAAS) of Nicaragua are high, as maternal/infant mortality rates are. Clean delivery kits (CDKs; sealed containers, clean razor blades, soap, string to tie umbilical cords, and clean plastic sheeting) have been shown to decrease perinatal infection rates in low-income countries. Participant observation, focus groups, and key informant interviews with parties involved in delivery practices and policies were conducted in this focused ethnography to determine the cultural accept

Current methods of non-invasive ventilatory support for neonates.

Non-invasive ventilatory support can reduce the adverse effects associated with intubation and mechanical ventilation, such as bronchopulmonary dysplasia, sepsis, and trauma to the upper airways. In the last 4 decades, nasal continuous positive airway pressure (CPAP) has been used to wean preterm infants off mechanical ventilation and, more recently, as a primary mode of respiratory support for preterm infants with respiratory insufficiency. Moreover, new methods of respiratory support have been developed, and the devices used to provide non-invasive ventilation (NIV) have improved technically

Improved diagnosis of classical vs atypical phenylketonuria by liquid chromatography.

Atypical phenylketonuria among infants with hyperphenylalaninemia must be promptly diagnosed and differentiated from classical phenylketonuria, because these patients require different treatment to prevent irreversible neurological damage. Measurement of pteridines in urine by liquid chromatography has been widely used for this purpose. Here we report a rapid, simplified liquid-chromatographic method for simultaneously measuring urinary biopterin, neopterin, and creatinine. Unlike previous methods, this method involves neither tedious cleanup procedures nor special equipment.

An Official American Thoracic Society/European Respiratory Society Workshop Report: Evaluation of Respiratory Mechanics and Function in the Pediatric and Neonatal Intensive Care Units.

Ready access to physiologic measures, including respiratory mechanics, lung volumes, and ventilation/perfusion inhomogeneity, could optimize the clinical management of the critically ill pediatric or neonatal patient and minimize lung injury. There are many techniques for measuring respiratory function in infants and children but very limited information on the technical ease and applicability of these tests in the pediatric and neonatal intensive care unit (PICU, NICU) environments. This report summarizes the proceedings of a 2011 American Thoracic Society Workshop critically reviewing techni

DETERMINATION OF CHOLESTEROL IN HUMAN MILK: AN ALTERNATIVE TO CHROMATOGRAPHIC METHODS.

INTRODUCTION: human milk (HM) is considered the best option for feeding healthy infants. Cholesterol (CHOL) is important for proper development of the nervous system, and for hormone and vitamin synthesis in growing infants. Breastfeeding and dietary CHOL intake during infancy have been suggested to affect blood lipid levels and the risk of cardiovascular disease in adulthood. Gas chromatography is the technique most widely used to determine CHOL in HM. Chromatographic methods are specific for the determination of CHOL and other sterols present in HM, but are extremely time consuming, and the

The usefulness of the Retinomax autorefractor for childhood screening validated against a Danish preterm cohort examined at the age of 4 years.

BACKGROUND AND PURPOSE: Refractometers have gained a foothold in childhood screening for ophthalmic disorders. Given the results of an ophthalmic follow-up of an extremely preterm Danish cohort, the results of the Retinomax autorefractor were further evaluated. MATERIALS AND METHODS: A nationwide cohort of infants born before gestational age 28 weeks (n=178) and 56 term controls were examined at the age of 4 years. Refraction was given as the cycloplegic Retinomax value. For this study, we analysed the equipment's confidence value on the printout and equipment-induced myopization (as the diff

Extra-uterine placental transfusion and intact-cord stabilisation of moderately preterm to term infants in caesarean deliveries - A feasibility study with historical control (INTACT-2).

BACKGROUND: Although delayed umbilical cord clamping (DCC) is universally recommended, implementation has been difficult in caesarean deliveries. The study objective was to test if extra-uterine placental transfusion (delivering the placenta before cord clamping) to facilitate intact-cord stabilisation could be a feasible and safe alternative to DCC (≥ 1 min) for moderately preterm to term infants with caesarean delivery in regional anaesthesia and their mothers. METHODS: This feasibility study included infants with GA 320 to 423 weeks with planned or emergency caesarean delivery. Primary out

Less Invasive Surfactant Administration: A Quality Improvement Project.

PURPOSE: Less invasive surfactant administration (LISA) uses a thin catheter that allows infants to breathe spontaneously throughout surfactant administration. The procedure potentially reduces adverse effects and enhances clinical outcomes compared to the INtubate, SURfactant, Extubate (INSURE) method, which requires transient intubation with an endotracheal tube and positive pressure ventilation. The purpose is to share how the Plan-Do-Study-Act (PDSA) cycle method can identify improvement opportunities, guide interventions, and improve outcomes in a LISA quality improvement (QI) initiative
