Nosocomial infections and antimicrobial resistance are topics that have been intensely studied in human medicine because of their significant impact on human health. In recent years, concerns have been raised that the use of antibiotics in veterinary medicine, animal husbandry, and agriculture may be contributing to the development of resistance in common bacterial species affecting human beings. Although there is inadequate proof at this time that the resistance is transmitted from animals to people, if antibiotics continue to be used indiscriminately in veterinary medicine, veterinarians may

Spinal infections can occur in a variety of clinical situations. Their presentation ranges from the infant with diskitis who is unwilling to crawl or walk to the adult who develops an infection after a spinal procedure. The most common types of spinal infections are hematogenous bacterial or fungal infections, pediatric diskitis, epidural abscess, and postoperative infections. Prompt and accurate diagnosis of spinal infections, the cornerstone of treatment, requires a high index of suspicion in at-risk patients and the appropriate evaluation to identify the organism and determine the extent of

The major hepatobiliary infections (excluding the viral hepatitides) include amebic and pyogenic liver abscess and cholangitis. Little new information has been published in the area of cholangitis during the last several years. In contrast, the clinical presentation and management of liver abscess have evolved considerably, not only in the last several years but also during a more extended period spanning the last two decades. In the United States, amebic liver abscess occurs largely in individuals from endemic areas or in those traveling to endemic areas. Recent data suggest that patients wit

Peritoneal dialysis related infections include infection of the catheter exit site, subcutaneous pathway, or effluent. Exit-site infections, predominately owing to Staphylococcus aureus, are defined as purulent drainage at the exit site, although erythema may be a less serious type of exit-site infection. Tunnel infections are underdiagnosed clinically, and sonography of the tunnel is useful to delineate the extent of the infection and to evaluate response to antibiotic therapy. S aureus infections occur more frequently in S aureus carriers and immunosuppressed patients and can be reduced by m

During and after the smallpox eradication campaign, human cases of monkeypox appeared in West and Central Africa, as isolated cases or as small epidemics. Since inter-human transmission has never or only very exceptionally been documented, monkeypox does not represent a serious threat to humans. The virus reservoir is among tree squirrels living in the tropical rain forests of Africa and humans are infected by hunting, killing and skinning these animals. However, the modernization of society lessens human contact with the virus reservoir. Since the eradication of smallpox, stocks of variola vi

Although cholangitis is the most prevalent hepatobiliary infectious process, liver abscess represents the most serious and conspicuous hepatobiliary infection. Amebic liver abscess typically occurs in individuals from endemic areas or those traveling to endemic areas. It is associated with an excellent prognosis when managed expediently with antiamebic antibiotics. Recent reports emphasize a possible association between amebic liver abscess and HIV infection. Drainage or surgery for amebic liver abscess is rarely necessary. In contrast, pyogenic liver abscess is associated with significant mor

The role of foodborne infections in the health of the population has become of major concern recently. Numerous agents are transmitted in food and water and typically result in acute gastroenteritis, although long-term complications such as reactive arthritis (due to Salmonella, Yersinia, and Shigella organisms), Guillain-Barré syndrome (due to Campylobacter organisms), and renal failure (due to Escherichia coli) are now well recognized. The development of FoodNet to follow the epidemiology of select foodborne infections in the United States has been a major advance in recent years and is now

Major hepatobiliary infections include cholangitis and liver abscess. Liver abscess is typically either of pyogenic or amebic origin. Amebic liver abscess usually occurs in individuals from endemic areas or those traveling to endemic areas and is associated with an excellent prognosis if properly managed, usually with one of several antiamebicidal drugs alone. Pyogenic liver abscess is most often cryptogenic in origin, although sophisticated advanced interventional procedures such as transarterial embolization and cryoablation are leading to a new generation of patients with pyogenic liver abs

Fungal infections have increased in frequency in the last decades because of the growing number of immunocompromised patients who survive longer periods of time than in the past, the widespread use of immunosuppressive drugs, a large aging population with increased numbers of malignancies, and the spread of AIDS. Although fungi are present everywhere, some mycoses predominate in the tropics, not only in view of warm temperature and humid climate, which favor their growth, but also because of inadequate hygienic and working conditions brought about by poverty. Mycotic diseases in the brain are

Non-opportunistic bacterial infections are an important cause of morbidity and mortality for HIV-infected adults and children. Factors associated with increased risk of these include altered B- and T-cell function; altered phagocytic cell function; skin and mucous membrane defects; and use of indwelling vascular catheters, antibiotics, or cytotoxic agents. The pathogens encountered most frequently are S. aureus, S. pneumoniae, H. influenzae, Salmonella sp., and Pseudomonas aeruginosa. Less commonly encountered organisms include Rhodococcus equi, Listeria monocytogenes, Shigella sp., and Nocard

The organisms that produce bacterial infections of the nervous system in tropical regions are similar to those existing in the rest of the world. However, because of poor socio-economic conditions in the former areas, preventing the implementation of appropriate prophylactic and therapeutic measures, the incidence and course of these diseases may vary. In this paper the neuropathological appearances of the main bacterial diseases are reviewed and the main differences between those occurring in developed and developing countries emphasized. Despite great efforts by governments and communities,

Yeasts are unicellular fungi that reproduce by the process of budding in which daughter cells are produced from parents by outpouching of the cell membrane and wall, migration of cytoplasm into the new structure thus formed, and then separation from the parent cell. Yeasts that are pathogenic in humans range in size from 2 to 12 microns in diameter; most, therefore, can be engulfed by phagocytic cells. These pathogens include many of the best known of pathogenic fungi, such as the Candida species, Cryptococcus neoformans, and the lipophilic yeasts of the genus Malassezia.

The genus Yersinia contains three pathogenic species: Y. pestis, Y. enterocolitica and Y. pseudotuberculosis. All pathogenic strains contain a 70 kb plasmid coding for a number of virulence factors, of which outer membrane proteins including an adhesin are the most important. In 65% of the patients the infection is self-limiting, but in the others a chronic local inflammation develops. Persistence of the infection has been associated with the immune status and the HLA-B27 antigen. Y. enterocolitica leads to abdominal complaints in young children that usually subside spontaneously. The infectio

Neonatal infection still carries a high mortality and morbidity. The spectrum of infection is broad and includes congenital syphilis and viral infections. Sadly, nosocomial infection is common, particularly due to coagulase negative staphylococci. Very low birth weight infants are at high risk, particularly following prolonged central venous catheterization; continuous low-dose vancomycin may offer a solution to this important problem. Early identification of infected infants can be facilitated by measurement of interleukin-6 levels. The premature newborn, deficient in white cells and humoral
