[Skeletal fluorosis: a case report].

The authors report a case of skeletal fluorosis described in a 60 years old man living in south-west Tunisian. The main clinical sign is a severe functional legs disability. Radiographic examination has shown a diffused osteocondensation, with cervical spine degenerative discopathy, a pelvis bilateral osteoarthritis and an interosseous membrane ossification of forearms and legs. The skeletal fluorosis diagnosis has been confirmed by high serum and urinary fluoride levels.

Evolution of the Ring Concept for the Forearm and Its Implication on Treatment: From Galeazzi, Monteggia, Essex-Lopresti, and Darrach to the Current Era.

Geometrically, rings distribute their stress along their arc instead of concentrating at any one point. The forearm ring is composed of the radius, ulna, proximal radioulnar joint, and distal radioulnar joint. The annular ligament, interosseous membrane, and triangular fibrocartilage complex link and stabilize the ring. Injuries to the forearm occur along a continuum with recognized patterns of ring disruption, including Galeazzi, Monteggia, and Essex-Lopresti injuries. The Darrach procedure causes a disruption to the forearm ring and can lead to painful convergence between the radius and dist

The effect of fibular malreduction on contact pressures in an ankle fracture malunion model.

Nine fresh-frozen cadaveric specimens were disarticulated through the knee, and the soft tissues, except for the interosseous ligaments and interosseous membrane, were removed to the level of the ankle. The subtalar joint was secured with screws in neutral position (approximately 5 degrees of valgus). Contact pressures in the tibiotalar joint were measured with use of low-grade pressure-sensitive film, which was placed through an anterior capsulotomy. For each measurement, 700 newtons of load was applied to the specimen for one minute. The film imprints were scanned, and the contact pressures

Delayed posttraumatic pseudoaneurysm of the anterior tibial artery mimicking a malignant tumor.

The anterior tibial artery arises at the lower border of the popliteus muscle, and passes anteriorly to lie on the interosseous membrane medial to the fibular neck. Injury to the artery results in extravasation of blood, and the tamponade effect of the surrounding tissue may contain acute hemorrhage. The blood clot gradually liquefies resulting in the formation of a fibrous capsule. Consequently, a pseudoaneurysm contains no normal elements of arterial wall, as opposed to a true arterial aneurysm. Pseudoaneurysm of the anterior tibial artery has been reported as a complication of fracture surg

Fibula pro tibia and cancellous allograft vitalised with autologous bone for non-union of the distal tibia diaphysis: Surgical technique.

BACKGROUND: The management of medium-sized osseous defects and pseudoarthrosis of the tibia is challenging. This surgical technique aims to bridge medium-sized tibial defects using the fibula as a lead structure. The fibula and cancellous bone graft fuse with the tibia leading to synostosis. METHODS: Cortico-cancellous bone is harvested from the anterior iliac crest. The bone is shredded into small pieces of about 5 mm and mixed with stem cell-rich blood from the situs. Additionally, cancellous allografts can be used for expansion. The approach is extended along the interosseous membrane to r

The Essex-Lopresti Injury.

The Essex-Lopresti injury is caused by a high energy mechanism and consists of a characteristic triad: a comminuted radial head fracture, disruption of the distal radioulnar joint, and tearing of the interosseous membrane. These injuries are often difficult to diagnosis on initial evaluation, and the majority are missed acutely. Chronic Essex-Lopresti injuries lead to radioulnar longitudinal instability, proximal radius migration, ulnocarpal impaction, and chronic elbow pain. These injuries present a challenging problem for the treating surgeon.

Decision-making in the treatment of radial head fractures: Delphi methodology.

BACKGROUND: The treatment of partially displaced radial head fractures is determined not only by the type of fracture but also by patient characteristics such as age, occupation, hand dominance, mechanism of injury, and concomitant injuries and comorbidities. The goal of this study was to employ the Delphi method to achieve consensus on the management of patients with radial head fractures, utilizing the experience of the ASES Elbow Fracture-Dislocation Multicenter Study Group and Mayo Elbow Club surgeons. METHODS: The initial survey was sent to participants, which included consent to partici

Upper tibial corticocancellous flap anatomy study (19.2.93).

A cortico-cancellous flap from the anterolateral aspect of the upper third of the tibia was presented. Sixty lower limbs of "fresh" cadavers were dissected. The vascular bundle includes the anterior tibial artery and its recurrent collateral branches and recurrent and musculoperiosteal arteries. The flap is harvested with the interosseous membrane and can be used either free or predicled. We used this flap for two patients suffering from pseudarthrosis. Long range clinical and radiological results are good.

Avoiding Neurovascular Risk During Percutaneous Clamp Reduction of Spiral Tibial Shaft Fractures: An Anatomic Correlation With Computed Tomography.

The use of percutaneous clamps is often a helpful tool to aid reduction and intramedullary nailing of distal tibial spiral diaphyseal fractures. However, the anterior and posterior neurovascular bundles are at risk without careful clamp placement. We describe our preferred technique of percutaneous clamp reduction for distal spiral tibial fractures with a distal posterolateral fracture spike, with care to protect the adjacent neurovascular structures. We also investigated the relationship between these neurovascular structures and the site of common percutaneous clamp placement. Preoperative c

[ESTABLISHMENT OF A NEW RADIUS DEFECT MODEL BASED ON ULNA ANATOMICAL MEASUREMENT IN RABBITS].

OBJECTIVE: To introduce a new bone defect model based on the anatomical measurement of radius and ulna in rabbits for offering a standard model for further tissue engineering research. METHODS: Fifteen healthy 4-month-old New Zealand rabbits were selected for anatomic measurement and radiological measurement of the radius and ulna. Another 30 healthy 4-month-old New Zealand rabbits were randomly divided into groups A, B, and C (n=10). The radius bone defect was created bilaterally in 3 groups. In group A, the periosteum and interosseous membranes were fully removed with jig-saw by approach be

Understanding Acute Distal Radioulnar Joint Instability.

Acute instability of the distal radioulnar joint (DRUJ) is often underestimated but can cause devastating loss of forearm and wrist form and function. These traumatic injuries can be caused by disruption of the triangular fibrocartilage complex, the interosseous membrane, or fractures of the radius or ulna, and a comprehensive understanding of the anatomy involved is paramount to the adequate treatment of these injuries. While stable DRUJ instability can be treated with immobilization, often surgical intervention is required and is based on the anatomical restraints to the DRUJ that are disrup

Radial Shaft Convergence in Distal Radius Fractures: Diagnosis and Treatment.

Radial shaft convergence in distal radius fractures is often misdiagnosed. This common deformation is often associated with a radial translation of the distal fragment. This parameter has to be corrected because of the increased risk of distal radioulnar joint instability due to detensioning of the distal interosseous membrane if there is an associated triangular fibrocartilage complex lesion. A new radiologic sign for diagnosis of proximal radius convergence during distal radius fracture is presented as well as technical tips for correction of this deformity.

Transfer of tibialis posterior tendon in cerebral palsy.

Posterior tendon transfer to the dorsum of the foot through the interosseous membrans is an effective procedure for spastic equinovarus deformity. The complications of excessive calcaneal or excessive valgus deformity can be avoided if the transfer is not inserted too tightly with the foot in dorsiflexion and if it is not transferred to a cuboid insertion. Electromyographic activity was demonstrated in some of the transferred muscles during the swing phase of gait. Tendo achillis lengthening in association with posterior tibial transfer must be done judiciously to avoid deformity of the calcan

The Bridle procedure in the treatment of paralysis of the foot.

The Bridle procedure consists of a posterior tibial tendon transfer through the interosseous membrane to the dorsum of the foot with a dual anastomosis to the tendon of the anterior tibial and a rerouted peroneus longus in front of the lateral malleolus. This procedure is performed to restore loss of dorsiflexion and to balance the foot, preventing a secondary varus or valgus deformity. The procedure was performed in 10 patients with 11 feet with paralysis of dorsiflexion of the foot. All are brace-free. The results are better in patients with peroneal nerve palsy secondary to trauma.

Locked Scapholunate Instability Presenting as a Progressive Injury.

BACKGROUND: Locked scapholunate instability is described as scapholunate instability occurring with a dislocated proximal pole of the scaphoid. Scapholunate instability is an early stage of perilunate instability. While being rare, complete or proximal pole scaphoid dislocations usually occur as an acute injury and often missed. CASE DESCRIPTION: We present a case of locked scapholunate instability where the proximal pole dislocation developed over a period of 6 weeks following injury. In addition, the membranous part of the scapholunate interosseous ligament was avulsed and interposed betwee

[The treatment of spastic equinovarus in the adult by transplantation of peroneus brevis and lengthening of the tendo-calcaneus (author's transl)].

A new technique is proposed for the treatment of spastic equinovarus foot. After lengthening the tendo calcaneus, the peroneus brevis tendon is divided at the junction with its muscle belly and the distal end of the tendon is transferred through the interosseous membrane to be sutured to the tendo calcaneus. This technique aims to correct the varus as well as the equinus deformity. It was performed in seven cases with good results. The technique is compared with other methods designed to treat this condition. The method is worthwhile in cases without bony deformity.

Scintigraphy of injuries to the distal tibiofibular syndesmosis.

Ankle sprains are a common injury and may occur in as many as 75% of sporting participants. More complex injuries involving a rotational component may lead to injury of the interosseous membrane, its supporting ligaments, or the tibial plafond, compromising the integrity of the ankle mortice. Four such cases that show the scintigraphic manifestation of the injury are presented. In addition to the standard planar views of the ankles, special magnified anterior views were obtained with the feet in internal rotation of 25 degrees to 35 degrees. These special views facilitated the diagnoses.

Forearm instability.

Forearm instability results from trauma, which disrupts the radial head, the interosseous membrane, and the triangular fibrocartilage complex. Inadequate treatment of injuries to these forearm stabilizers may result in the complex problem of chronic longitudinal forearm instability. Delayed recognition and/or treatment of injuries producing forearm dissociation has led to poor patient outcomes, which makes timely recognition of the injury pattern imperative. This article discusses relevant aspects of forearm anatomy and current concepts in the diagnosis and treatment options for this complex i

Skeletal fluorosis causing high cervical myelopathy.

Skeletal fluorosis is endemic in some parts of the world and is the result of life-long ingestion of high amounts of fluoride in drinking water. Its clinical presentation is characterized mostly by bone and dental changes with later ossification of many ligaments and interosseous membranes. We present a rare case of high cervical myelopathy caused by ossification of the posterior longitudinal ligament and ligamentum flavum in a patient from an area endemic for skeletal fluorosis. The clinical presentation of skeletal fluorosis and treatment options are discussed.

Endemic fluorosis of the skeleton: radiographic features in 127 patients.

OBJECTIVE: A wide range of radiographic appearances have been reported in skeletal fluorosis, but little has been written about the spectrum of radiographic features. We evaluated the spectrum of radiographic appearances in this disorder to help with its diagnosis and differentiation from other metabolic skeletal disorders. MATERIALS AND METHODS: One hundred twenty-seven patients with clinically proved endemic fluorosis had radiographs of the chest, spine, pelvis, elbow, forearm, and knee obtained. The radiographic findings were classified as osteosclerosis, osteopenia, intermittent growth li
