The DATTA panelists emphasized the critical importance of patient selection when considering IUDs for contraception. The IUD is an acceptable method of contraception, especially for those women who are in the middle to older reproductive years, unable to take oral contraceptives, in a stable monogamous relationship, and not at risk for sexually transmitted diseases. This paper reports the joint experience of three investigators who used three types of the Multiload (ML) IUD (MLCu250 Short, MLCu250 Standard, and MLCu375) in 1987 consecutive insertions during which an experience of 56,005 woman-months of use was accumulated. Allergological examinations were conducted on 49 women (out of a gynaecological clientele of 982) with different degrees of complaints after wearing an intrauterine device (IUD). At the same time, they and a control group had an histological examination of endometrial biopsies. Nickel allergy was found in 38 of the 49. We analyzed the clinicopathologic aspects of 19 cases of actinomycosis associated with intrauterine devices (IUDs) seen and treated at hospitals affiliated with the State University of New York at Buffalo between 1972 and 1982. Over a 6 year period, 67 women had a translocated intrauterine contraceptive device (IUCD) removed from the peritoneal cavity or uterovesical fold under general anaesthesia. In 40 patients removal of the IUCD at laparoscopy succeeded, whereas in 24 laparotomy proved necessary and three IUCD were removed per vaginam. Compared with the inert plastic device a larger proportion of copper-containing IUCD in the peritoneal cavity required laparotomy for removal, however, 44% could be removed by laparoscopy, the less traumatic procedure.

Ultrasonic localization of missing IUDs was performed on 51 patients with a high degree of accuracy. This method also demonstrated pregnancy where present and other pathologic conditions including fibroids and ovarian cysts. Other methods recommended for localization of missing IUDs are discussed with particular reference to their adverse effects. A protocol for localizing missing IUDs using ultrasound is recommended.

Copper-containing intrauterine devices (IUDs) for contraception are increasingly used within recent years. IUDs in situ provoke a non-specific inflammatory cell reaction, particularly at the glandular epithelium of the endometrium. The cellular changes include swelling of the cytoplasm and of the nuclei, the latter showing chromatin clumping, prominent nuclei and a pseudoeosinophilic reaction. One of the most serious problems among women using the intrauterine device (IUD) has been pelvic inflammatory disease (PID). Subjects for the present investigation were chosen from 15- to 44-year-old women seeking emergency medical attention at a large city hospital.
