General anesthesia with isoflurane for diaphragmatic hernia repair during ECMO.

Isoflurane has been administered via the oxygenator of the extracorporeal membrane oxygenation (ECMO) circuit to provide anesthesia and prevent hemodynamic instability during surgical repair of congenital diaphragmatic hernia. A comparison of a series of patients treated with isoflurane demonstrates that there is less fluctuation in heart rate and blood pressure than when surgery is performed under fentanyl analgesia alone.

Isoflurane and enflurane-induced hepatic necrosis in triiodothyronine-pretreated rats.

Exposure of triiodothyronine (T3)-pretreated rats to 1.3% isoflurane, 1.8% enflurane, or 1% halothane in 21% oxygen (air) for two hours resulted in hepatic centrilobular necrosis. The incidence of the liver lesion was 28, 24, and 92% after exposure to isoflurane, enflurane, and halothane, respectively. Histopathologic grading indicated that the necrosis was more severe after halothane than after isoflurane or enflurane anesthesia. No lesion was observed in livers prepared from non-anesthetized T3-pretreated rats or in livers prepared from rats which were pretreated with the vehicle for T3 and

Hepatic Injury following halothane, enflurane, and isoflurane anesthesia in rats.

Halothane anesthesia administered to enzyme-induced animals in a hypoxic atmosphere consistently produced hepatic necrosis. Rats pretreated with phenobarbital were exposed to hypoxia at varying intervals after administration of halothane, enflurane, or isoflurane anesthesia. Anesthetics were administered at 1 MAC for 2 h. For each agent, hypoxia consisting of 8 per cent oxygen-balance nitrogen for 1 h was imposed at the end of anesthesia. In other groups of rats, we also used a 15-, 30-, 60-, and 120-min interval of 100 per cent oxygen between 2 h of halothane anesthesia and the imposition of

Differential impact of isoflurane on the topological organization of frontoparietal network in macaques.

The lateral frontoparietal (FP) network, also referred to as the central executive network, is critical for goal-directed behavior in primates. Resting-state functional MRI (rs-fMRI) studies have revealed functional homologies between humans and macaques; however, methodological disparities, particularly the use of anesthesia in non-human primates, raise questions about the validity of interspecies comparisons. Anesthetic agents such as isoflurane have been shown to alter functional connectivity (FC), but whether they differentially affect lateral FP subnetworks remains unclear. Here, we inves

Bidirectional modulation of isoflurane potency by intrathecal tetrodotoxin and veratridine in rats.

BACKGROUND AND PURPOSE: Results from several studies point to voltage-gated Na(+) channels as potential mediators of the immobility produced by inhaled anaesthetics. We hypothesized that the intrathecal administration of tetrodotoxin, a drug that blocks Na(+) channels, should enhance anaesthetic potency, and that concurrent administration of veratridine, a drug that augments Na(+) channel opening, should reverse the increase in potency. EXPERIMENTAL APPROACH: We measured the change in isoflurane potency for reducing movement in response to a painful stimulus as defined by MAC (minimum alveola

The influence of endotoxemia on the electroencephalographic and antinociceptive effects of isoflurane in a swine model.

BACKGROUND: We have previously reported that hemorrhagic shock decreases the minimum alveolar anesthetic concentration (MAC) of isoflurane but minimally alters the electroencephalographic (EEG) effect. In this study, we investigated the influence of endotoxemia on the EEG effect and the MAC of isoflurane. METHODS: Eighteen swine (25.7 +/- 2.3 kg) were anesthetized by inhalation of isoflurane. The inhaled concentration was decreased to 0.5% and maintained for 20 min, before being returned to 2% and maintained for a further 20 min. End-tidal isoflurane concentrations and spectral edge frequenci

An opioid basis for early-phase isoflurane-induced hypotension in rats.

This study was conducted to more clearly delineate the possible role of endogenous opioid receptors and opioid peptides in general anesthesia-associated hypotension in rats. Exposure to 2% isoflurane in oxygen produced a triphasic change in mean arterial pressure (MAP), including an early phase in which MAP fell by -28.4 +/- 2.2%. The magnitude of this early-phase hypotension was attenuated in rats pretreated with intravenous (i.v.) mu-subtype-selective doses of either naloxone or methylnaloxone but not central doses of the selective mu-opioid antagonist beta-funaltrexamine. This early hypoten

The epileptogenic properties of the volatile anesthetics sevoflurane and isoflurane in patients with epilepsy.

UNLABELLED: No study comparing epileptogenicity of sevoflurane to other volatile anesthetics has been performed. We compared the epileptogenic properties of sevoflurane to isoflurane in patients with epilepsy. In 24 mentally and/or physically disabled patients, 12 with epilepsy and 12 without epilepsy, electroencephalograms were recorded under anesthesia with 1.0 minimum alveolar anesthetic concentration (MAC), 1.5 MAC, and then 2.0 MAC sevoflurane or isoflurane under three ventilatory conditions: (A) 100% oxygen, and end-tidal CO(2) partial pressure (ETCO(2)) = 40 mm Hg, (B) 50% oxygen, 50% n

[A case of status asthmatics relieved by long-term inhalation of isoflurane].

A 58 year old man in status asthmatics refractory to drug therapy, was relieved by long-term isoflurane inhalation. In status asthmatics, it takes long time to recover from airway-damage, and inhalation-time will be prolonged in many cases. It is appropriate to use isoflurane because it is devoid of significant adverse effects compared with halothane and enflurane. Especially, isoflurane did not cause an increase in the serum fluoride revel, and will not lead to renal insufficiency in spite of its inhalation for 120 hours. It seems that isoflurane is suitable for long-term inhalation.

Ventilation with isoflurane in the Triservice anaesthetic apparatus: a comparison with halothane and trichlorethylene.

Recovery from anaesthesia using the Triservice anaesthetic apparatus with either isoflurane alone or halothane and trichlorethylene, was assessed by measurement of reaction time. There was no statistically significant improvement in recovery between patients who received low concentrations of isoflurane and those who received halothane and trichlorethylene. The mean profiles of systolic pressure and heart rate were significantly greater in the isoflurane group (P less than 0.05) than in the halothane/trichlorethylene group. Cardiovascular stability was maintained into the recovery period. The

Levels of consciousness in volunteers breathing sub-MAC concentrations of isoflurane.

Eight volunteers inhaled isoflurane in concentrations of 0.1, 0.2 and 0.4 MAC, and 100% oxygen as control in separate sessions more than 1 week apart. When the end-expiratory isoflurane concentration was stable, response to verbal commands was tested, and the subjects were read 30 words in two lists. Response to the commands was impaired at 0.1 MAC in three subjects and lost at 0.2 MAC in two subjects. No subject responded at 0.4 MAC. When the subjects were questioned 1 h after exposure, memory of response to commands was lost also at these concentrations. Frequently, those who responded to th

Isoflurane and halothane inhibit tetanic contractions in rabbit myocardium in vitro.

Rabbit right ventricular papillary muscles were tetanized by rapid stimulation in the presence of 1 microM ryanodine, an inhibitor of sarcoplasmic reticular function. Tetanic contractions elicited in this manner increased in strength as extracellular calcium concentration [Ca+2]ext was raised from 0.5 to 5 mM, exhibited saturation behavior above [Ca+2]ext = 5 mM, and were blocked by nifedipine. Accompanying membrane potentials depolarized to +10 mV and repolarized to -60 mV between stimuli. These data suggest that rabbit myocardial tetany is supported in large part by extracellular calcium inf

Thiopental and epinephrine-induced dysrhythmias in dogs anesthetized with enflurane or isoflurane.

Epinephrine-induced dysrhythmias were studied in 19 dogs anesthetized with 1.25 MAC enflurane or isoflurane, or the same preceded by thiopental (20 mg/kg). In 11 (group 1) dogs, thiopental reduced the dose of epinephrine required for production of ventricular ectopy, bigeminy and tachycardia with enflurane, and only ventricular tachycardia with isoflurane (P less than 0.05). Thiopental potentiation of epinephrine-induced dysrhythmias with enflurane lasted 4 hr after induction. Evaluation of spectral entropy monitor with different concentrations of isoflurane in Horses.

Spectral entropy and bispectral index (BIS) can differentiate between conscious and unconscious levels in horses. This study aimed to determine whether spectral entropy and BIS parameters can distinguish between different end-tidal isoflurane concentrations (EtIso, %), and whether these parameters change when a nociceptive stimulus is applied. Ten clinically healthy horses were anaesthetized using xylazine, diazepam, ketamine, and isoflurane. The following parameters were monitored: EtIso, response entropy (RE), state entropy (SE), RE-SE difference, BIS, and electromyography (EMG). Three EtIso
