On the nature of the oscillations of the membrane potential (slow waves) produced by acetylcholine or carbachol in intestinal smooth muscle.

1. Intracellular recording was made with glass micro-electrodes from cells of the longitudinal muscle of the guinea-pig ileum in isotonic and in hypertonic solution.2. In isotonic solution spontaneous bursts of electrical activity occurred; these consisted of a slow potential component which carried a burst of spike action potentials. Acetylcholine increased the size (and the frequency) of the slow potential component. This had the effect of first reducing and then abolishing the spike potentials; continuous slow wave activity was thus produced. Slow waves were about 1 sec in duration and up t

Early Resuscitation of Patients With Non-exsanguinating Trauma Using Packed Red Blood Cells Versus Low-Volume Crystalloids: Have We Gone Too Far?

The early resuscitation of patients with mild to moderate non-exsanguinating trauma has shifted from the conventional use of one to two liters of crystalloids to the use of one to two units of PRBC. This evolution assumes that the transfusion of PRBC is superior to the administration of any volume of crystalloids because of the propensity of crystalloids to migrate from the intravascular to the interstitial space leading to organ dysfunction, organ failure, and worse outcomes. However, the premise of the fluid migration relies on Starling original model of fluid exchange between the hydrostati

[Fluid and Volume Therapy with Crystalloid and Colloidal Infusion Solutions].

A rational infusion therapy orchestrates fluid- and volume therapy based on the individual indication and situation. The principle of fluid replacement is to substitute ongoing fluid losses such as insensible perspiration and urine output or to treat dehydration with balanced crystalloid solutions. Volume therapy in contrast is the quick restoration of intravascular losses such as an acute blood loss through application of balanced colloids or crystalloids. The goal of volume therapy is to maintain normal cardiac output and oxygen delivery by restoring intravascular normovolemia and cardiac pr

Arthrocentesis of the temporomandibular joint: a review.

BACKGROUND: Arthrocentesis is a very gentle method for lavage of the joint space. The principle consists in the introduction of a pair of needles into the upper joint space and subsequent lavage using physiological saline or Ringer's solution. Arthrocentesis of the temporomandibular joint is used in both cases of acute closed lock and treatment of various temporomandibular disorders. METHODS: A literature search in Pubmed database, using key words: Temporomandibular joint (TMJ), Arthrocentesis, indications, technique, results. CONCLUSION: Arthrocentesis of the temporomandibular joint is a mi

Ammonia as a stimulant to ventilation in rainbow trout Oncorhynchus mykiss.

Ammonia is the third most important respiratory gas in ammoniotelic fish after oxygen and carbon dioxide. We here investigated the effects of elevated plasma ammonia on ventilation in freshwater rainbow trout. Intact trout fitted with indwelling dorsal aortic catheters were given injections (over 5 min) of Cortland saline, isotonic high ammonia solutions (NH(4)HCO(3), (NH(4))(2)SO(4), NH(4)OH at pH 8.0, and NH(4)OH at pH 9.0), and other solutions as controls for acid-base effects, while ventilatory rate (VR) and buccal pressure amplitude (DeltaP(buccal)) were recorded. All high ammonia solutio

Hypertonic saline in critical illness - A systematic review.

INTRODUCTION: The optimal approach to fluid management in critically ill patients is highly debated. Fluid resuscitation using hypertonic saline was used in the past for more than thirty years, but has recently disappeared from clinical practice. Here we provide an overview on the currently available literature on effects of hypertonic saline infusion for fluid resuscitation in the critically ill. METHODS: Systematic analysis of reports of clinical trials comparing effects of hypertonic saline as resuscitation fluid to other available crystalloid solutions. A literature search of MEDLINE and

Hypertonic Saline in Human Sepsis: A Systematic Review of Randomized Controlled Trials.

The role of hypertonic saline in sepsis remains unclear because clinical data are limited and the balance between beneficial and adverse eff … ls with 381 patients who had received hypertonic saline. Lower volumes of hypertonic saline than of isotonic solutions were needed to achieve the desired hemodynamic goals (standardized mean difference, -0.702; 95% CI, -1.066 to -0.337; P < .001; moderate-quality evidence). Hypertonic saline administration was associated with a transient increase in sodium and chlor …

Compatibility of nitroglycerin, diazepam and chlorpromazine with a new multilayer material for infusion containers.

The stability and compatibility of three drugs: nitroglycerin, diazepam and chlorpromazine, with a new multilayer infusion bag were studied. The study was carried out comparatively with PVC bags with which these drugs are incompatible. The drugs were diluted in 5% dextrose or in 0.9% sodium chloride isotonic solutions. Solutions were stored during 8 or 48 h with or without any protection against light. Remaining concentrations of drug were determined by high-performance liquid chromatography (HPLC) during the storage. The admixtures were also monitored for precipitation, color change and pH. W…

Parenteral fluid regimens for improving functional outcome in people with acute stroke.

BACKGROUND: Parenteral fluids are commonly used in people with acute stroke with poor oral fluid intake. However, the balance between benefit and harm for different fluid regimens is unclear. OBJECTIVES: To assess whether different parenteral fluid regimens lead to differences in death, or death or dependence, after stroke based on fluid type, fluid volume, duration of fluid administration, and mode of delivery. SEARCH METHODS: We searched the Cochrane Stroke Group Trials Register (May 2015), the Cochrane Central Register of Controlled Trials (CENTRAL), the Cochrane Database of Systematic Re
