Preoperative Predictors for Acute Pain After Photorefractive Keratectomy.

PURPOSE: The aim of this study was to identify preoperative predictors for the occurrence of early severe postoperative pain in patients undergoing photorefractive keratectomy (PRK). The implementation of preoperative screening methods may facilitate more specific or aggressive pain therapies specifically targeted to individuals at a high risk of experiencing severe postoperative pain. METHODS: This was exploratory research that included patients who underwent PRK. Before PRK, patients were administered a sociodemographic questionnaire, the Pain Catastrophizing Scale, and the State-Trait Anxi…

Severe bilateral paralimbal sterile infiltrates after photorefractive keratectomy.

This study presents a case report of the clinical presentation and management of a 47-year-old male myope who underwent photorefractive keratectomy (PRK) and developed bilateral sterile corneal infiltrates at 1 day post-operatively. The patient was successfully treated with aggressive topical antibiotic and topical steroid therapy. The final corrected distance visual acuity (CDVA) was 20/25 with faint corneal scarring. Peripheral sterile corneal infiltrate can occur after PRK with excellent prognosis. Infectious causes should be suspected in all cases of corneal infiltrate. The most likely cau…

Causes of decentration after laser-assisted subepithelial keratectomy.

BACKGROUND AND OBJECTIVE: To determine important factors causing decentration after laser-assisted subepithelial keratectomy (LASEK). PATIENTS AND METHODS: A total of 223 cases (446 eyes) were evaluated after LASEK using corneal topography to determine the amount of decentration. Decentration and pupillary offset values and their components on the nasal, temporal, superior, and inferior axes were analyzed with the t test and the two-way repeated measures analysis of variance test in patients with myopia or hypermetropia. Correlation analysis between the amount of decentration and the possible…

Mycobacterium chelonae keratitis after laser-assisted subepithelial keratectomy.

A 24-year-old man with myopic astigmatism had uncomplicated laser-assisted subepithelial keratectomy (LASEK) in October 2004. Approximately 4 weeks later, a foreign-body sensation and redness developed in the left eye. A paracentral anterior stromal infiltrate was observed, and Gram stain on the day of presentation showed branching filamentous gram-positive rods. Cultures subsequently grew Mycobacterium chelonae. Topical tobramycin 15 mg/mL, azithromycin 2 mg/mL, amikacin 25 mg/mL, and cefazolin 50 mg/mL were initiated based on Gram stain results. We believe this is the first documented case o…

[Phototherapeutic keratectomy. Undesirable effects, complications, and preventive strategies].

The conceptual advantage of phototherapeutic keratectomy (PTK) include the option to remove diseased tissue on a micrometer scale. Although less invasive than other forms of corneal surgery, it remains a surgical procedure which carries inherent risks. In phototherapeutic indications this risk is higher than in otherwise healthy eyes undergoing photorefractive surgery. Potential general complications include wound-healing problems, infection, inflammation, or a recurrence of the underlying disease. More specifically, PTK is associated with unwanted refractive changes, such as a hyperopic shift…

Haze after photorefractive keratectomy caused by iatrogenic lagophthalmos.

A 22-year-old man had shortening of the levator muscle for a congenital blepharoptosis in the right eye in 2000. In September 2004, he was successfully treated by bilateral photorefractive keratectomy (PRK) for myopia and was asymptomatic with an uncorrected visual acuity of 20/20. In October 2005, right eye visual acuity decreased because of an inferiorly localized haze caused by nocturnal lagophthalmos. Fluorometholone eyedrops and lubrication induced full visual recovery after 2 months, but corresponding topographical abnormalities were only partially improved. Corneal exposure can induce h…

Early bacterial keratitis after laser-assisted subepithelial keratectomy.

We report a case of bacterial keratitis that occurred after laser-assisted subepithelial keratectomy. The patient presented with a decrease in visual acuity and pain 2 days after the procedure. Culture was positive for Staphylococcus haemolyticus. The infiltrate slowly resolved with topical antibiotics, and the best corrected visual acuity improved to 20/20. Although bacterial keratitis occurs rarely after refractive surgery, patients should be informed of the potential risk for visual loss caused by this infection.

Corneal epithelial dots following excimer laser photorefractive keratectomy.

BACKGROUND: Map-dot-fingerprint epithelial changes of the cornea have been reported to occur as a consequence of trauma or surgical procedures, such as radial keratotomy. METHODS: We describe a case of dot-like changes in the corneal epithelium following excimer laser photorefractive keratectomy for the correction of myopia. Because the lesions were located centrally, possibly reducing visual acuity, the epithelium was removed mechanically. RESULTS: Dot-like changes recurred in the same areas 4 weeks after epithelial debridement. Best spectacle-corrected visual acuity improved from 20/200 to…
