In a study of over 32 pyelocaliceal tumors greater than 1 cm in diameter, the authors review retrospectively the reports of the pyelographic and sonographic documents obtained in these patients before the definitive diagnosis was confirmed by the pathological examination. They observed that pyelography has a much greater diagnostic accuracy than does sonography: 31 confirmed (27) or suspected (4) tumors whereas only one tumor was overlooked (silent kidney) on pyelography and 12 diagnosed tumors, 15 overlooked and 5 diagnostic errors on ultrasonography. However, it should be noted that the resu

A rare congenital malformation of the kidneys, consisting of renal agenesis on the left and a small kidney on the right with only a single calyx, abnormal renal vasculature and impaired medullary function, is described. This was associated with long phocomelia of the right arm. The coexistence of these rare malformations has not been reported before.

Six patients in this series of 543 renal transplants (1.10%) suffered a post-transplant renal segmental infarct of the donor kidney because of occlusion of an accessory renal artery. Five grafted kidneys had multiple renal arteries. Patients presented with symptoms of a caliceal fistula and were treated by partial (25 to 40%) transplant nephrectomy, followed by closure and tissue coverage with either parietal peritoneum (4 patients) or lyophilized human dura mater sealed with fibrin (2). In 2 cases the renal ischemia and necrosis involved the ureter, and a pyelo-pyelostomy was performed. One p

BACKGROUND AND PURPOSE: Horseshoe kidneys are a complex anatomic variant of fused kidneys, with a 20% reported incidence of associated calculi. Anatomic causes such as high insertion of the ureter on the renal pelvis and obstruction of the ureteropelvic junction are thought to contribute to stone formation via impaired drainage, with urinary stasis, and an increased incidence of infection. In this multi-institutional study, we evaluated whether metabolic factors contributed to stone development in patients with horseshoe kidneys.
PATIENTS AND METHODS: A retrospective review of 37 patients wit

Urothelial carcinoma is usually characterized by severe symptoms including macroscopic haematuria. We here report the case of a female patient without a history of smoking or urothelial carcinoma, making it, to our knowledge, a unique case and a first case of urothelial carcinoma revealed by brain metastasis.

A 39-year-old white man with a history of right renal pelvic stones treated 1 year before by extracorporeal shock wave lithotripsy, but with no history of major surgery, alcoholism, pancreatitis, hyperparathyroidism or trauma, was admitted, suffering from an abdominal mass. Abdominal and pelvic computed tomography revealed an enlarged pancreatic head (9 cm in transverse diameter) with, inside it, a heterogeneous, cyst-like structure, measuring 7 cm in diameter. It was suspected that this lesion was a cystic neoplasm and the patient underwent a proximal pancreaticoduodenectomy and a cholecystec

The authors present their results with the treatment of nephrolithiasis of the lower calix using the method of extracorporeal lithotripsy with the Czech apparatus Medilit M-5. In 310 patients they made 452 operations and checked the therapeutic results three months after the last session. For checking they used a native nephrogram and examination by ultrasound. With the increasing size of concrements the effectiveness declined and thus also the success of ESWL(in concrements above 20 mm in size it declined to 30%). This is not due only to the size of the concrement but also its site in the low

As it is well known, the gold-standard to treat ureteric and pelvi-caliceal transitional cell carcinoma (TCC) is nephroureterectomy. Nevertheless, in particular circumstances, such as solitary kidney, a more conservative treatment is needed. Conservative treatment has been reported for low-grade TCC, also. In this paper we discuss the laparoscopic approach to low-grade ureteric TCC in patients with otherwise normal urinary tract. After reporting a case of a patient treated with this approach, we deal with some technical aspect of the surgical procedure and with the pre-operative and post-opera

The most frequent initial clinical symptom is flank pain, which occurs in approximately half of affected patients with stones of the upper urinary tract. Of these patients approximately half require treatment and 50% will again have new stones in the future. Only 8% of urinary stones are clinically inapparent with 25-50% of the stones located in the lower calyx group and are incidentally discovered during the diagnostics of symptomatic stones. Stones in the lower calyx are more difficult to treat than other kidney stones due to the localization and the resulting access possibilities. The treat

The failure of a stone-containing calix to visualize on an imaging study (that is a missing calix) is often the initial and only sign of an associated pyocalix, since the patient may present without fever and with sterile urine. Extracorporeal shock wave lithotripsy (ESWL*) in these patients may lead to life-threatening systemic sepsis. Therefore, the treatment of choice for the stone-laden missing calix is percutaneous drainage of the affected calix followed by percutaneous stone removal and, when necessary, an endoinfundibulotomy to reestablish communication between the obstructed calix and

OBJECTIVES: To show the pathologic characteristics of upper urinary tract tumors with special emphasis on the peculiarities due to anatomical site and secondary to endoscopic treatment.
METHODS: Bibliographic review and presentation of our ten-year experience at the "Hospital Ramon y Cajal" Pathology Department in Madrid studying 203 urothelial tumors of the ureter, renal pelvis and calyces.
RESULTS: More than 95% of the upper urinary tract tumors in adults have their origin in the urothelium, and most of them are either malignant or potentially malignant. Their biological behaviour is simil

Six adult Ascaris lumbricoides were found in the pelvis and calyces of the left kidney of a 25-year-old woman in Mexico. Prior to death three worms had emerged from a subcutaneous abscess which at autopsy was found to communicate with the colon through fistulas to the upper ureter above an occluding calculus, and from the renal capsule to the skin near the left iliac crest.

Objectives: To compare the safety and effectiveness of a novel flexible vacuum-assisted ureteral access sheath (FV-UAS) and traditional ureteral access sheath (UAS) in simulating retrograde intrarenal surgery (RIRS). Materials and Methods: A manometric model was established in porcine kidneys to observe the change in intrarenal pressure (IRP) in the FV-UAS and traditional UAS groups at different irrigation fluid velocities of 30, 50, 80, and 100 mL/min. Establish a kidney stone model (with 0.2 g, dry, ≤5 mm stones) to simulate RIRS. A total of 20 porcine kidneys were randomly numbered from 1 t

Transitional cell carcinoma of the upper urinary tract (UUT-TCC) is relatively uncommon, accounting for 2-5% of all urothelial tumors. Its incidence appears to be increasing as a result of progress in imaging, endoscopy, and improved survival from bladder cancer. Renal pelvis tumors represent 10% of all renal cancers. Pyelic neoplasms occur at a rate twice to four times the incidence of tumors in the ureter, where the common site is the distal tract (about 70%). One third of UUT-TCC ore multifocal, and about 1% are simultaneous and bilateral. The introduction of lasers represented a big step i

Examination of the renal pelvis of late fetal and neonatal kidneys shows that in about one quarter of the calyces examined the arrangement is similat to that in the rat, the outer medulla being separated from the calyceal cavity by a very thin epithelium. It is suggested that this presents a vulnerable area through which intrarenal damage can occur as a result of vesico-ureteric reflux in the immature kidney.

PURPOSE: To present a simplified surgical technique to access the kidney percutaneously with low exposure to radiation.
MATERIAL AND METHODS: This percutaneous approach to the kidney "creates" a plane of coincidence between the C-arm beam axis and the needle axis. Both axes always have the same angle running in opposite directions with the predetermined calix located in the medial vertical plane at 0 degrees. Twenty-one percutaneous access tracts were made into the lower calix and 7 into the middle and upper calices. The 3-dimensional position of the inserted needle was determined.
RESULTS:

CONTEXT: Percutaneous nephrolithotomy (PCNL) in renal fusion, ectopia, renal rotational anomalies, renal hypoplasia, and pelvicaliceal aberration for complex calculi is challenging for fear of inaccessibility, vascular, colonic, and splanchnic injury. We report our experience with 48 such patients.
PATIENTS AND METHODS: From 2002 to July 2007, 48 patients (mean age, 28 years) underwent PCNL for mean stone size of 39 mm. Fourteen patients had malrotated kidneys, 12 patients had bifid renal pelvis with complete or incomplete double ureter, 10 horseshoe kidneys, 8 hypo plastic kidneys, 3 with cr

Congenital megacalyces is a calyceal anomaly characterized by nonobstructive dilatations of the calyces, associated with hypoplasia of the medullary pyramids. In the past three years, seven patients with congenital megacalyces were found incidentally during renal ultrasonic examination. There were five boys and two girls who were asymptomatic, except for a case with hematuria. Three cases presented bilateral and four unilateral, ages ranged from 7 to 14 years. In this series, all cases with dilatations of the calyces were initially detected by sonography, and then confirmed by other radiologic
