A Swedish knee-cage for stabilizing short below-knee stumps.

Stump length is an important factor in attaining successful prosthetic rehabilitation in below-knee (BK) amputees. Stability of the stump-prosthesis complex is impaired in the case of a stump shorter than 10 cm. Thus, fitting a prosthesis to a BK amputee with a stump which is very short often requires the use of different prosthetic techniques. In this work, the authors suggest the use of a Swedish knee-cage attached to a conventional patellar-tendon-bearing prosthesis as an alternative solution in the case of a short BK stump. Objective evaluation was performed by an analysis of gait and the …

Below-knee amputee knee extension force-time and moment characteristics.

This study, utilizing isometric knee extension, was undertaken to determine if significant differences existed between extremities of below-knee amputees and matched normal subjects in minimal time to production of peak force, maximal rate of tension development, and maximal extension moment. Results showed no significant differences for rate of tension development. Significance at the .05 level was established for moments produced by left amputees and their contralateral limb, and right and left limbs of normal subjects. Differences in time to 95 percent of peak force for the amputated leg we…

Functional improvement of unicompartmental knee arthroplasty compared with total knee arthroplasty for subchondral insufficiency fracture of the knee.

Subchondral insufficiency fracture of the knee (SIFK) causes acute knee pain in adults and often requires surgical management. Unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) are the two most common surgical treatments for SIFK. While both UKA and TKA have their advantages, there is no consensus for SIFK localized on the medial compartment. We hypothesized that patients with SIFK treated with UKA would show superior patient-reported outcomes compared to those who underwent TKA. A total of 90 patients with SIFK located medially were included in the TKA (n = 45) and UK…

Effect of knee arthroscopic debridement combined with peripatellar denervation on restoration of knee function in patients with knee osteoarthritis.

BACKGROUND: This research examines knee osteoarthritis (OA), a prevalent orthopedic disease marked by cartilage degeneration and chronic synovitis, leading to pain, restricted mobility, and eventual loss of knee function. Notably, patellofemoral osteoarthritis constitutes a significant proportion of knee OA cases. Our study aims to assess the impact of knee arthroscopic debridement coupled with peripatellar denervation on restoring knee function in OA patients and analyze the risk factors affecting treatment outcomes. By doing so, we hope to contribute to the informed selection of clinical tre…

The subjective knee value is a valid single-item survey to assess knee function in common knee disorders.

INTRODUCTION: The patient's perspective plays a key role in judging the effect of knee disorders on physical function. We have introduced the Subjective Knee Value (SKV) to simplify the evaluation of individual's knee function by providing one simple question. The purpose of this prospective study was to validate the SKV with accepted multiple-item knee surveys across patients with orthopaedic knee disorders. MATERIALS AND METHODS: Between January through March 2020, consecutive patients (n = 160; mean age 51 ± 18 years, range from 18 to 85 years, 54% women) attending the outpatient clinic fo…

Size of Medial Knee Osteophytes Correlates With Knee Alignment But Not With Coronal Laxity in Patients With Medial Knee Osteoarthritis.

The severity of osteoarthritis (OA) has been related to osteophyte size. However, the effects on osteophyte size of repeated and increased loading associated with joint laxity and varus misalignment remain unclear. We investigated these relationships in patients with medial knee OA and compared the performances of computed tomography (CT) and radiography for assessing osteophyte parameters. We examined knee joint alignment on radiographs and knee laxity using arthrometry in 191 patients with medial knee OA who were undergoing total knee arthroplasty. We also measured femur and tibia osteophyte…

The association of knee structural pathology with pain at the knee is modified by pain at other sites in those with knee osteoarthritis.

The objective of this study was to investigate the associations of knee structural abnormalities with different patterns of pain. A total of 891 participants (average age 63 years; range 50 to 80 years) participated in this study. Presence of pain at the neck, back, hands, shoulders, hips, knees, and feet was assessed by questionnaire. Participants were categorized as having no pain at any site (no pain), pain only at the knee (KP), pain at other sites but not the knee (OP), and pain at the knee and other sites (KOP). T1-weighted or T2-weighted MRI of the right knee was performed to measure ca…

Crosswalk and correlation between the Oxford Knee Score (OKS), Knee Society Score (KSS) and Knee Injury and Osteoarthritis Outcome Score (KOOS-12) in knee osteoarthritis.

BACKGROUND: Significant heterogeneity exists in the patient-reported outcome measures (PROMs) used to assess outcomes in knee osteoarthritis (KOA), with limited evidence exploring the correlation and conversion among the various PROMs. Our study aims to explore the correlation and conversion between the Oxford Knee Score (OKS), Knee Injury and Osteoarthritis Outcome Score (KOOS-12) and Knee Society Score (KSS) in patients with non-operatively treated KOA. METHODOLOGY: This was a prospective cohort study of 131 patients who received non-operative treatment (standardized physical therapy and an…

Advances in knee arthroplasty for younger patients: traditional knee arthroplasty is prologue, the future for knee arthroplasty is prescient.

Total knee arthroplasty (TKA) was a remarkable development in orthopedic surgery. Joint arthroplasty and arthroscopy were perhaps the greatest innovations in orthopedics in the 20th century and occurred without the advantages of today's technology. Initially, TKA was performed only on elderly patients and those with advanced rheumatoid arthritis because of concerns with long-term wear of polyethylene. Surgeons strongly discouraged this surgery for patients younger than age 60 years because both patients and many orthopedic surgeons believed that knee implants would last only for approximately …

Paraffinoma of the knee.

We present the case of a 90-year-old man with a swollen, painful knee and a history of paraffin injections. Radiography showed soft tissue streaky and nodular calcifications without erosions. MR imaging showed globular areas of low signal intensity on T1- and T2-weighted images which became even more hypointense on STIR sequences, and an infiltrative hypointense region containing areas of high signal on T1-weighted, T2-weighted and STIR images. Biopsy confirmed soft tissue foreign body granulomata, fibrous reaction and areas of inflammation. We believe this to be the first report of MR imaging…

Tuberculosis of the knee.

Fifty-two children with tuberculosis of the knee treated from 1979 to 1999 were reviewed retrospectively. The radiologic appearance of the joint at presentation was predictive of the outcome. Ninety-two percent of the patients had Stage 1 or Stage 2 involvement (synovitis) with or without bony erosions, but had a normal joint space. Treatment with antituberculous chemotherapy without synovectomy had an excellent or good result in all patients with Stage 1 or Stage 2 disease, and there was no difference in outcome whether the knee was immobilized or mobilized. Patients with Stage 3 and Stage 4 …

Ultrasonography of the knee.

Despite the increasing use of MRI in knee disorders, ultrasonography remains the first choice examination technique in para-articular disease. The technique is fast, cheap, easily accessible and usable in the acutely injured patient. The choice of equipment, positioning of the patient and some characteristics of normal anatomy are described. The routine applications of ultrasonography include different types of tendon disease, trauma of the collateral ligaments, bursal pathology, joint effusions and synovitis, evaluation of the popliteal fossa and para-articular swellings. Other possible appli…

The postoperative knee.

Magnetic resonance (MR) has established itself as the gold standard for noninvasive imaging of the knee. As in arthroscopy, assessment of injuries to the menisci, articular surfaces, synovium, and cruciate ligaments can be made. Additionally, MR has an advantage over arthroscopy in being able to assess the para articular soft tissues and cortical and medullary bony compartments. Until recently, the role of MR in the postoperative knee has been less clear. New information, however, suggests that MR is capable of assessing the status of the partially resected meniscus, the adjacent articular sur…

[Tendinitis of the knee].

The diagnostic criteria for tendinitis of the knee are: (1) strict localization of the pain in the whole length of the tendon from its body to its end; (2) recrudescence of pain during movements that stretch or rub the tendon, and (3) positive local anaesthesia test if needed. Only this approach makes it possible not to miss a tendinitis of the extensor system in a so-called femoro-patellar syndrome, or to misdiagnose a tendinitis of the sartorius, gracilis and semitendinosus muscles when confronted with an osteoarticular pathology of the medial femoro-tibial compartment.
