[Open knee joint injuries (including vascular and nerve lesions)].

Immediate definitive reconstruction of all damaged structures leads to better results than delayed operations. If direct closure of the joint is not possible, local rotational flaps, muscle flaps or free skin-fat tissue flaps (i.e. a scapular flap) may be used. In injuries of the popliteal artery, the interposition of a saphenic vein graft is preferable to local reconstruction of the artery. Nerve injuries are the only exception to comprehensive primary repair. Reconstruction in these lesions is usually done later when all wounds are healed. The postoperative treatment includes the use of cont

Prevention of hip and knee injuries in ballet dancers.

Hip problems form about 10% (7.0 to 14.2%) of most published series of ballet injuries. The abnormally large range of external rotation needed for a perfect turnout is primarily due to soft tissue adaptation, more readily achieved in the young dancer. Insufficient range of motion at the hip throws considerable stress on the other lower limb segments. The snapping hip syndrome is common (43.8% of hip problems), with about one-third associated with pain. A tight iliotibial band may contribute to this, and balanced flexibility requires special attention to abductor stretching. The external clicki

Multiple Ligament Knee Injuries: Does the Knee Dislocation Classification Predict the Type of Surgical Management?

This study aims to evaluate relationships among multiple ligament knee injury (MLKI) patterns as classified according to the knee dislocation (KD) classification and the types of surgical management pursued. We hypothesized that the KD classification would not be predictive of the types of surgical management, and that categorizing injuries according to additional injury features such as structure, chronicity, grade, and topographic location would be predictive of the types of surgical management. This is a Retrospective cohort study. This study was conducted at a level I trauma center with a

Incidence of Concurrent Peroneal Nerve Injury in Multiligament Knee Injuries and Outcomes after Knee Reconstruction.

The purpose of this study was to determine incidence of concurrent peroneal nerve injury and to compare outcomes in patients with and without peroneal nerve injury after surgical treatment for multiligament knee injuries (MLKIs). A retrospective study of 357 MLKIs was conducted. Patients with two or more knee ligaments requiring surgical reconstruction were included. Mean follow-up was 35 months (0-117). Incidence of concurrent peroneal nerve injury was noted and patients with and without nerve injury were evaluated for outcomes. Concurrent peroneal nerve injury occurred in 68 patients (19%). …

Acute knee injury. How to determine if the knee is stable.

Knee injuries are common in patients of all ages. A detailed history concerning the mechanism of injury and symptoms is helpful in establishing a tentative diagnosis, and the physical examination can easily provide confirmation. X-ray films occasionally are helpful, but stress views and arthrograms should not be relied on for definitive diagnosis. Arthroscopy may be required for definitive evaluation of internal derangement of the knee and can be used therapeutically in meniscal injuries and other problems. Proper rehabilitation is mandatory for a good outcome; therefore, physical therapy is e…

Injury Patterns in Posterolateral Corner Knee Injury.

BACKGROUND: Posterolateral corner (PLC) knee injuries associated with different injury mechanisms are not well known. PURPOSE/HYPOTHESIS: This study sought to assess the patterns of associated injuries in the setting of PLC injury. The hypothesis was that there are recognizable injury patterns in PLC injuries that may correlate with injury mechanism. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Patients who sustained a multiligament knee injury were retrospectively reviewed. Patients who sustained an acute grade 3 PLC injury and underwent surgery were enrolled in this…

Simultaneous bilateral multiligamentous knee injuries are associated with more severe multisystem trauma compared to unilateral injuries.

PURPOSE: To compare the knee ligamentous injury patterns and associated multisystem trauma in patients who have sustained simultaneous bilat … MLIs with similar mechanisms, patients with traumatic simultaneous bilateral knee multiligamentous knee injuries are at high risk of concomitant head, chest and abdominal injuries. Although the ligament injury profile is similar, the post-operative complication rate is higher for simultaneous bilateral injuries. LEVEL OF EVIDENCE: Case control study, Level II …

Non-Anatomic Reconstruction in Multiligament Knee Injuries: A Functional Approach.

Background/Objectives: Multiligament knee injuries, involving damage to multiple stabilizing structures, present a significant challenge in orthopedic surgery, often resulting in knee instability and compromised function. While anatomic ligament reconstruction has been traditionally advocated, non-anatomic techniques may provide effective alternatives, particularly for patients with moderate functional demands who do not require high-level athletic performance. Material and methods: In this study, we assessed the outcomes of a non-anatomic, hybrid surgical approach involving combined arthrosco…

Articular involvement impacts unplanned reoperation rates in floating knee injuries.

OBJECTIVES: To compare the rate of unplanned reoperation to address fracture-related complications between extraarticular floating knee frac … tly associated with requiring surgery to address knee stiffness (p = 0.009). CONCLUSIONS: Floating knee injuries with at least one articular fracture, especially when the patella is involved, had higher rates of surgery for knee stiffness. Intraarticular floating knee injuries are challenging, often requiring reoperation for infection, nonunion, or stiffness. …

Evidence based treatment options for common knee injuries in runners.

The purpose of this paper is to review the current literature regarding conservative treatment options for the three most common knee injuries in runners including patellofemoral pain syndrome (PFPS), iliotibial band friction syndrome (ITBFS), and patellar tendinopathy (PT). Each diagnosis is discussed using current research to describe the pathophysiology, evaluation process, and evidence based effective treatment strategies including therapeutic exercise, manual therapy, neuromuscular re-education, and modalities. The result is a comprehensive overview of each diagnosis and a research-based …

Etiology, Diagnosis and Treatment of Tendinous Knee Extensor Mechanism Injuries.

BACKGROUND AND AIMS: Quadriceps and patella tendon ruptures are uncommon injuries often resulting from minor trauma typically consisting of an eccentric contraction of the quadriceps muscle. Since rupture of a healthy tendon is very rare, such injuries usually represent the end stage of a long process of chronic tendon degeneration and overuse. This review aims to give an overview of the current understanding of the pathophysiology, diagnostic principles, and recommended treatment protocols as supported by the literature and institutional experience. MATERIAL AND METHODS: A non-systematic rev

Concomitant ligamentous and meniscal knee injuries in femoral shaft fracture.

BACKGROUND: Concomitant knee injury is a common finding in femoral fractures but can be easily missed during early management of the initial trauma. Degrees of damage to the articular structures vary considerably; from only a mild effusion to complete ligamentous and meniscal tears. Since previous reports were mostly from developed societies, this study was designed to look into characteristics of associated knee injury in a sample from Iran, to represent a developing country perspective. MATERIALS AND METHODS: Consecutive patients admitted to an orthopedic ward of Baqiyatallah hospital (Tehr

Management of arterial and venous injuries in the dislocated knee.

Although uncommonly encountered, knee dislocation is frequently associated with major vascular injury. Serious injuries resulting in ischemia demand prompt recognition and efficient management to prevent devastating long-term sequelae. In this review, we detail mechanisms of knee dislocation and associated popliteal vascular injuries. Diagnostic modalities used to evaluate the extent of vascular injury are individually discussed. Appropriate initial management of vascular injuries is crucial and an algorithm for diagnosis and management will be reviewed. We elaborate on the salient points of v

[Value of imaging in posterolateral corner injuries of the knee].

Lesions of the posterolateral corner are usually post-traumatic in etiology. They are most frequently associated with tear of the ACL and/or PCL. When unrecognized, they may lead to short-term failure of cruciate ligament reconstruction or long-term knee joint degeneration. Early detection of such lesions, especially in the preoperative period, is important since more severe injuries usually require dedicated early surgical management. The anatomy of the posterolateral corner will be reviewed and the normal and abnormal imaging features on MRI and US will be illustrated. The main clinical and

Musculoskeletal injuries of the knee: are females at greater risk?

Research has shown that the incidence of anterior cruciate ligament injuries is higher in female athletes than in male athletes. And anecdotal evidence points to a higher incidence of patellofemoral disorders in females versus males. This article reviews what is known about the incidence of these injuries, the reasons for sex differences in injury rates, the mechanisms behind the injuries, and strategies to prevent them.

"Floating knee" injuries: ipsilateral fractures of the femur and tibia.

Ipsilateral fractures of the femur and tibia have been called "floating knee" injuries and may include combinations of diaphyseal, metaphyseal, and intra-articular fractures. These are often high-energy injuries and most frequently occur in the polytrauma patient. Many of these fractures are open, with associated vascular injuries. Surgical stabilization of both fractures and early mobilization of the patient and the extremity produce the best clinical outcomes. The use of a radiolucent operating room table and the introduction of retrograde intramedullary fixation of femoral fractures have fa
