[Analysis of lactase activities of small intestine mucous membrane by double labeled stable isotope technique in subjects with lactase deficiency].

OBJECTIVE: Low lactase activity in small intestine mucosa is the main reason for the occurrence of lactose malabsorption (LM) and lactose intolerance (LI). It would be the basis for the research on LM and LI to find an accurate method to analyze the activity of lactase. METHODS: In this study, 43 volunteers were selected and divided into LM and LI group according to the results of H2 breath test and symptoms record. Twenty-five grams of 13C-lactose and 0.5 g 2H-glucose in 250ml solution were consumed by all the volunteers. The concentration of total plasma glucose, 13C- glucose and 2H-glucose…

[The role of a combined probiotic in the treatment of lactase deficiency].

AIM: To study the effectiveness of the combined probiotic Bifiform and the enzyme lactase in the treatment of secondary lactase deficiency (SLD). MATERIALS AND METHODS: We examined 79 patients with SLD, isolated from the group of patients with post-infectious irritable bowel syndrome (IBS) with a predominance of diarrhea. The age of patients reached 273.5 years, women predominated by gender 62%. Diagnosis of SLD was carried out in biopsies of the small intestine mucosa using a color rapid test. To diagnose small intestinal bacterial overgrowth (SIBO) all patients underwent lactulose breath te…

Lactose digestion and the evolutionary genetics of lactase persistence.

It has been known for some 40 years that lactase production persists into adult life in some people but not in others. However, the mechanism and evolutionary significance of this variation have proved more elusive, and continue to excite the interest of investigators from different disciplines. This genetically determined trait differs in frequency worldwide and is due to cis-acting polymorphism of regulation of lactase gene expression. A single nucleotide polymorphism located 13.9 kb upstream from the lactase gene (C-13910 > T) was proposed to be the cause, and the -13910*T allele, which is …

Hypolactasia and lactase persistence. Historical review and the terminology.

Lactase (more precisely lactase-phlorizin hydrolase) is located in the brush border of the small intestinal enterocytes and is responsible for the hydrolysis of dietary lactose. The earliest studies on lactase activity in mammals were published around the turn of the century. In 1903, it was found that the dog had a very low lactase activity and therefore lactose remained unhydrolysed, causing diarrhoea. Human hypolactasia was demonstrated in 1963, and it was soon found that it is very common, commoner than lactase persistence in most parts of the world. In 1973, adult-type hypolactasia was sh…

Distribution of adult lactase phenotypes in the Tuareg of Niger.

The adult lactase phenotype, lactose absorber or malabsorber, was determined using the lactose tolerance test with breath hydrogen assay in a group of Tuareg, a traditionally nomadic pastoralist population in the central Sahara. Out of a total of 118 subjects, 103 (87.3%) were lactose absorbers and 15 (12.7%) lactose malabsorbers. The frequency of the "lactase suppression gene" in this population sample was .357 (SD .043). The low frequency of lactase suppression in the Tuareg supports the hypothesis of natural selection in favor of the "lactase persistence gene" in milk-dependent nomadic past…

Persistence of high intestinal lactase activity in Pakistan.

In order to determine the incidence of persistence of high intestinal lactase activity (PLA) in a Pakistani human population, 53 probands belonging to the Punjabi ethnic group were examined using a lactose tolerance test. The incidence of PLA in the sample was found to be 55%, which has been compared with the incidence in other ethnic groups racially or geographically related to the Pakistani population. Various possible explanations for the fairly high incidence of PLA in the Punjabi ethnic group have been discussed.

Effect of pancreatic proteases on intestinal lactase activity.

The sensitivity of human intestinal lactase to pancreatic proteases was tested both in vitro and in vivo. Lactase specific activity in brush border membranes was decreased by 26%-27% during incubation with trypsin at pH 7.0 in patients with normal intestinal lactase levels, whereas in patients with lactase deficiency the inactivation was 75%. However, when lactase levels from deficient patients' mucosa were increased relative to trypsin during incubation so that they were comparable to the levels of activity in normal mucosa, inactivation of lactase in deficient patients was only 45%. Therefor…

[Exhaled hydrogen test in the detection of lactase deficiency].

To determine the validity of breath H2 measurements in detecting lactase deficiency, capillary blood glucose and breath H2 were measured after ingestion of 50 g lactose in 34 patients with abdominal symptoms or diarrhea. 18 patients had biopsy-proven lactase deficiency and 16 normal lactase activity. Blood glucose determination produced 2 false-positive results while breath H2 measurements provided 2 false-negative and 4 false-positive results.
