Acute lymphoblastic leukemia.

This is a comprehensive overview on the most recent developments in diagnosis and treatment of acute lymphoblastic leukemia (ALL). Dr. Dieter Hoelzer and colleagues give an overview of current chemotherapy approaches, prognostic factors, risk stratification, and new treatment options such as tyrosine kinase inhibitors and monoclonal antibodies. Furthermore the role of minimal residual disease (MRD) for individual treatment decisions in prospective clinical studies in adult ALL is reviewed. Drs. Ching-Hon Pui and Mary Relling discuss late treatment sequelae in childhood ALL. The relation betwee

Hairy cell leukemia.

The standard therapy for hairy cell leukemia (HCL) is with the nucleoside analogs, 2"-deoxycoformycin (dCF) or 2-chlorodeoxyadenosine (CdA), which produce morphologic complete remissions (CRs) in the majority of patients, although residual hairy cells can frequently be detected by molecular or immunologic techniques. Relapses continue to occur over time, but most patients respond well to retreatment with the same agent. The longest follow-up is for patients treated with dCF, where the 5- and 10-year relapse-free survival rates are 80% to 85% and 67% to 76%, respectively. dCF is usually adminis

Acute myeloid leukemia.

Through the hard work of a large number of investigators, the biology of acute myeloid leukemia (AML) is becoming increasingly well understood, and as a consequence, new therapeutic targets have been identified and new model systems have been developed for testing novel therapies. How these new therapies can be most effectively studied in the clinic and whether they will ultimately improve cure rates are questions of enormous importance. In this article, Dr. Jacob Rowe presents a summary of the current state-of-the-art therapy for adult AML. His contribution emphasizes the fact that AML is not

Acute Lymphoblastic Leukemia.

Acute lymphoblastic leukemia (ALL) accounts for 25% of childhood cancer. Clinical and biological parameters define prognostic categories and therapeutic approaches. For the majority of children with B-progenitor ALL, age (1-9 yrs) and white blood count (WBC<50,000) indicate standard risk disease; WBC>50,000 and age>9 yrs. define eligibility for high-risk protocols. Infants (<1 yr. old) are high-risk, frequently associated with chromosomal t(4;11) translocation and extramedullary disease. T-cell immunophenotype is usually associated with clinical high-risk features, including older age, high WB

[Hairy cell leukemia].

Clinical experiences on seven patients suffering from hairy cell leukaemia are presented. The disease was most often identified at age above 50 and was more frequent in men. Splenomegaly and infiltration of bone marrow or spleen with tartrat resistant acid phosphatase positive lymphoid cells were detected in all cases. Splenectomy resulted complete clinical remission in six and partial remission in one patient. Two patients have died. The mean survival time is five years. The surviving patients are in good clinical remission.

Chronic myelogenous leukemia.

Chronic myelocytic leukemia (CML) is a well-known myeloproliferative disorder, with typical clinical and hematological features, which develops into a lethal blastic crisis within an average of 4 years. In the last few years much has been learned about the cell responsible for the leukemic deviation, chromosomal abnormalities both in chronic phase and in blastic transformation, biochemical and cytochemical behavior of leukemic cells, and the immunology of the disease. On the contrary, polychemotherapy has brought but little progress in CML treatment. However, thorough research on prognostic fa

Acute basophilic leukemia.

Acute basophilic leukemia was diagnosed in a 61-year-old black woman on the basis of 85 to 90 percent basophils in the peripheral blood as well as bone marrow and very high serum histamine level (more than 10,000 ng/ml). These complications occurred as a transformation from essential thrombocythemia. Accompanying this transformation, there was also cytogenetic change from 46XX karyotype to 46XX 2p+ in 66 to 90 percent of cells in the bone marrow. This may be the first reported occurrence of transformation of essential thrombocythemia into acute basophilic leukemia.

Acute nonlymphocytic leukemia.

The discovery of cytosine arabinoside, and then the anthrocycline antibiotics, 6-thioguanine, vincristine, cyclophosphamide, and other drugs, has added to the armamentarium of known effective agents. The use of combination chemotherapy, the recognition of the need during induction for virtual marrow aplasia to obtain a remission, and recognition of the predilection of the disease for the central nervous system requiring prophylaxis constitute major advances. The impediment to long-term survival is the lack of effective maintenance therapy.

Acute ocular leukemia.

A 71-year-old woman had a history of recent weight loss and bilateral decreased visual acuity, bilateral serous detachment, and mental depression. Fluorescein angiograms showed a myriad of retinal pigment epithelial leakage points. Despite extensive evaluation, the cause of her weight loss and ocular process remained uncertain until her death, when postmortem examination revealed leukemia infiltrates of many organs, including the choroid. After death, we correlated the clinical signs and fluorescein angiograms with the histopathologic findings. This case shows that choroidal disease may be a s

Hairy cell leukemia.

The clinical findings in 12 patients with an unusual type of leukemia have been reviewed. The leukemic cell cannot be identified with any of the normal hematopoietic cells or with any of the lymphomas and may best be referred to by the descriptive term "hairy cell", which describes its appearance, most clearly seen on phase contrast microscopy. The patients were all males and the major clinical features were enlargement of the liver and spleen, with little lymph node enlargement. Hematologic findings in most patients have been anemia and thrombocytopenia with the characteristic abnormal cells

Research Progress of A20 in Acute Leukemia: A20 in Acute Leukemia.

Acute leukemia (AL) is a malignant tumor originating from hematopoietic stem cells. Its outstanding feature is the abnormal proliferation and aggregation of clonal leukemia cells in bone marrow and other hematopoietic tissues. Although chemotherapy, targeted immunotherapy and hematopoietic stem cell transplantation have been widely used in clinic, there are still relapse and refractory cases in AL patients. Finding new therapeutic targets and screening prognostic molecules are of great significance for the treatment and prognosis of AL. A20 protein, also known as tumor necrosis factor α-induce

Updates in the management of chronic lymphocytic leukemia/small lymphocytic leukemia.

Chronic lymphocytic leukemia is one of the most common lymphoid malignancies. Often treatment modalities are tailored to individual patients based on age, presence of comorbidities and cytogenetics. The advent of ibrutinib has significantly changed the management of the disease in all patient groups and has had the largest impact on clinical practice to date. Over the last 15 years, a series of trials have established that chemoimmunotherapy improves both progression-free survival and overall survival compared to chemotherapy alone. Despite its proven role, efficacy of ibrutinib has not been w

Are most secondary acute lymphoblastic leukemias mixed acute leukemias?

A case of acute lymphoblastic leukemia (ALL) secondary to X-ray exposure is reported. The lymphoblasts expressed T-cell and myelomonocytic differentiation-related surface antigens. Clonal chromosomal abnormalities including t(2;12)(q25;q13),-10,der(10), del(11)(q14),der(12) and r were observed. A review of the cytogenetic data suggests that most secondary ALLs, with the exception of Burkitt type ALLs, may be biphenotypic or mixed acute leukemias.

Infant Acute Leukemia.

Infant acute leukemia is a rare but aggressive disease. Although infant acute leukemia is cytologically and histologically similar to acute leukemia seen in older children and adults, it displays unique and characteristic clinical and genetic characteristics. The features, as well as the extremely young age of the patients, present multiple challenges for treatment. This review focuses on the unique pathology of acute leukemia of infancy, including the genetic characteristics that are specific for these diseases.

Hairy cell leukemia (HCL) is a well-defined entity. Proliferation with hair cells, morphological aspects of hairy cells are easy to identify. Hairy cells express markers CD11c, CD25, CD103 and CD123. In 80% of cases, a BRAFV600E mutation is highlighted. In the absence of a BRAFV600E mutation, the differential diagnosis with other hair cell proliferations can be difficult, especially with the variant form of hairy leukemia, diffuse lymphoma of the red pulp of the spleen or splenic lymphoma of the marginal zone. Purine analogues (PNA) with or without anti-CD20 antibodies remain the first-line re

Pure erythroid leukemia.

The category of acute erythroid leukemia was significantly revised in the recently published 2016 revision to the World Health Organization (WHO) classification of myeloid neoplasms. In the previous 2008 WHO classification, acute erythroid leukemia was categorized into two subtypes: erythroleukemia and pure erythroid leukemia (PEL), whereas in the 2016 WHO update, erythroleukemia was merged into myelodysplastic syndrome, and PEL becomes the only type of acute erythroid leukemia. PEL is a rare and aggressive form of acute leukemia whose biology remains poorly characterized. In this review, we d

[Plasma cell leukemia].

Plasma cell leukemia (PCL) is a rare disorder which develops spontaneously (primary PCL) or evolves in patients with multiple myeloma (secondary PCL). It is defined by the presence of 2 × 10(9)/L peripheral blood plasma cells or plasmacytosis accounting for more than 20 % of the differential white cell count. PCL presents more often extramedullary involvement, anemia, thrombocytopenia, hypercalcemia, as well as impaired renal function. Cytogenetic abnormalities and mutations observed in PCL lead to escape from immune surveillance and independence from the bone marrow microenvironment with chan

[Chronic lymphocytic leukemia].

Chronic lymphocytic leukemia (CLL) is a lymphoproliferative disorder that accounts for approximately 30 % of adult leukemias and 25 % of Non-Hodgkin lymphomas (NHL). It is the most common form of leukemia in the western world (incidence 3-5/100 000). Elderly people are mainly affected, median age at diagnosis is around 70 years and there is a slight predominance in men. The etiology of the disease is unknown. The initial symptoms are nonspecific. Cervical lymphadenopathy and splenomegaly followed by general fatigue are seen most commonly. Other possible symptoms include night sweats, fever, lo
