Vaginal microbiota in women with spontaneous preterm labor versus those with term labor in Kenya: a case control study.

BACKGROUND: Preterm birth is a global problem with about 12% of births in sub-Saharan Africa occurring before 37 weeks of gestation. Several studies have explored a potential association between vaginal microbiota and preterm birth, and some have found an association while others have not. We performed a study designed to determine whether there is an association with vaginal microbiota and/or placental microbiota and preterm birth in an African setting. METHODS: Women presenting to the study hospital in labor with a gestational age of 26 to 36 weeks plus six days were prospectively enrolled

Efficacy of venoarterial extracorporeal membrane oxygenation with and without intra-aortic balloon pump in adult cardiogenic shock.

INTRODUCTION: Intra-aortic balloon pump (IABP) is sometimes coupled with Venoarterial extracorporeal membrane oxygenation (VA-ECMO) to treat patients with cardiogenic shock. In this study, we attempted to evaluate the association of the IABP approach on survival and vascular complication rates in adults with cardiogenic shock undergoing VA-ECMO. METHODS: We performed a systematic search of original studies on VA-ECMO with and without IABP in PubMed, EMBASE, and the Cochrane Library. RESULTS: A total of 42 studies with 8,759 patients were included. The pooled in-hospital deaths of patients on

Tolvaptan add-on therapy and its effects on efficacy parameters and outcomes in patients hospitalized with heart failure.

INTRODUCTION: Even with the adequate use of diuretics and vasodilators, volume overload and congestion are the major causes of morbidity and mortality in patients hospitalized with acute heart failure (HF). We aim to evaluate the additive effect of tolvaptan on efficacy parameters as well as outcomes in hospitalized patients with HF. METHODS: We searched PubMed, EMBASE, Cochrane library, and Web of Science databases for randomized controlled trials that studied the effects of tolvaptan versus placebo in hospitalized patients with HF. Studies were included if they had any of the following endp

The behavior of preschoolers during and after brief surgical hospitalizations.

This qualitative study describes the behavior of preschool children during and after very brief hospitalizations for tonsillectomies and/or adenoidectomies. Structured, open-ended home interviews were conducted with 23 parents 5 to 7 days after their children's hospital discharge. Parents were asked to describe their children's behaviors during hospitalization and the first week at home. Approximately half of the parents and children had attended an optional surgical preadmission teaching program. The remaining parents had prepared their children for hospitalization using toy doctor kits and l

New-Onset Type 2 Diabetes Mellitus in Critical Illness Survivors and after Hospitalization.

OBJECTIVES: Recent literature suggests that hospitalization may lead to new-onset type 2 diabetes mellitus (NODM2). Understanding this relationship is crucial for developing interventions that could reduce long-term complications and healthcare costs associated with DM2 and major adverse cardiovascular events (MACE). This systematic review and meta-analysis aims to assess the incidence of NODM2 and MACE in patients after hospitalization. METHODS: With the assistance of the Mayo Clinic Libraries, we searched MEDLINE, Cochrane, and Scopus for medical literature reporting the incidence of NODM2

Meta-analysis of the need for nasogastric or nasojejunal decompression after gastrectomy for gastric cancer.

BACKGROUND: Nasogastric or nasojejunal decompression has been used routinely to prevent anastomotic leakage, hasten the return of bowel function and shorten hospital stay after gastrectomy for gastric cancer. This meta-analysis evaluates the necessity for such routine decompression. METHODS: Medline, Embase and The Cochrane Library were searched. Only prospective randomized controlled trials (RCTs) that compared individuals with and without nasogastric or nasojejunal decompression after gastrectomy for gastric cancer were included. Outcomes evaluated were time to flatus, time to starting oral

A systematic review and meta-analysis: surgical outcomes comparing transoral robotic and open approaches to thyroidectomy.

Thyroid cancer incidence is rising, especially in young women. Conventional open thyroidectomy (OT) is effective but leaves a neck scar, affecting mental health. Transoral robotic thyroidectomy (TORT) offers scarless surgery, but its safety and completeness need evaluation. We systematically searched PubMed, Embase, Cochrane Library, and Web of Science for trials comparing TORT and OT. Outcomes included lymph node yield, complications (recurrent laryngeal nerve injury, hypoparathyroidism, hematoma, seroma, flap perforation), operation time, hospital stay, and postoperative pain. Henry R. Viets, MD, and the History of Myasthenia Gravis.

Henry R. Viets (1890-1969) was both a noted neurologist and medical historian. While at Harvard Medical School, from which he graduated in 1916, he attracted the attention of Harvey Cushing who directed Viets into these disciplines. Cushing arranged for Viets to take a fellowship in Oxford in the year after his graduation. With Cushing's recommendation, he lived with Sir William and Lady Osler and did research with the famous neurologist Sir Charles Sherrington. Viets was in London in 1935 when he heard about the remarkable success of Mary Walker in treating myasthenia gravis, first with physo

In Vitro Screening of a 1280 FDA-Approved Drugs Library against Multidrug-Resistant and Extensively Drug-Resistant Bacteria.

Alternative strategies against multidrug-resistant (MDR) bacterial infections are suggested to clinicians, such as drug repurposing, which uses rapidly available and marketed drugs. We gathered a collection of MDR bacteria from our hospital and performed a phenotypic high-throughput screening with a 1280 FDA-approved drug library. We used two Gram positive (Enterococcus faecium P5014 and Staphylococcus aureus P1943) and six Gram negative (Acinetobacter baumannii P1887, Klebsiella pneumoniae P9495, Pseudomonas aeruginosa P6540, Burkholderia multivorans P6539, Pandoraea nosoerga P8103, and Esche

A clinical decision support system for the diagnosis of probable migraine and probable tension-type headache based on case-based reasoning.

BACKGROUND: The overlap between probable migraine (PM) and probable tension-type headache (PTTH) often confuses physicians in clinical practice. Although clinical decision support systems (CDSSs) have been proven to be helpful in the diagnosis of primary headaches, the existing guideline-based headache disorder CDSSs do not perform adequately due to this overlapping issue. Thus, in this study, a CDSS based on case-based reasoning (CBR) was developed in order to solve this problem. METHODS: First, a case library consisting of 676 clinical cases, 56.95% of which had been diagnosed with PM and 4

Comparative effectiveness of robotic and video assisted thoracic surgery for lung cancer across key outcomes.

Robotic-assisted thoracic surgery (RATS) and video-assisted thoracoscopic surgery (VATS) are widely used for minimally invasive resection of pulmonary tumors, but they differ in technical capabilities and resource demands. We conducted a PRISMA-based systematic review and meta-analysis of randomized and cohort studies identified by searching PubMed, Embase, Web of Science, Scopus, and the Cochrane Library through June 2025. Across 25 studies including 41,417 patients, RATS was associated with lower conversion to thoracotomy rates and less blood loss, more extensive lymphadenectomy, higher R0 r

Historical Perspectives: Low Birthweight and Preterm Infants in Indonesia.

The incidence of infants with low birthweight (LBW), prematurity, and small for gestational age (SGA) in Indonesia remains high, and the morbidity and mortality in these infants are significant. To study these groups of infants in Indonesia, the author searched PubMed/Medline, the Cochrane Library, Google, Indonesian reports, local publications, and doctoral and master theses in English and Indonesian from 1987 to February 2019. In this review, the development of local reference growth charts of infants born in Indonesia is described, as well as the importance of these curves in defining the p

Risk factors for depression in patients with chronic obstructive pulmonary disease.

Depression, anxiety, and panic disorders are common in chronic obstructive pulmonary disease (COPD) and important for the further course of the disease, as they are associated with increased hospital admissions, longer hospital stays, more frequent doctor visits, and a worsened quality of life. There are also indications of premature death in affected patients. Therefore, knowledge of the risk factors for depression in COPD patients is all the more important for early detection and treatment. Hence, Embase, the Cochrane Library, and the MEDLINE/PubMed databases were analyzed for studies on the

Validation of a Natural Language Processing-Assisted Chart Review Method for Identifying Advance Care Planning Documentation in Pediatric Critical Illness.

OBJECTIVES: Reliable measurement of advance care planning (ACP) is essential for evaluating both communication practices and quality of care in pediatric critical illness. However, most ACP documentation is embedded in free-text clinical notes and cannot be captured using administrative data, while manual chart review is time-intensive and difficult to scale. We sought to validate a rule-based natural language processing (NLP) approach to identify ACP documentation in the electronic health record (EHR) compared with manual chart review. DESIGN: Retrospective cohort study. SETTING: Single-cen
