Sexual and marital trajectories and HIV infection among ever-married women in rural Malawi.

OBJECTIVE: To explore how sexual and marital trajectories are associated with HIV infection among ever-married women in rural Malawi. METHO … e, however, are also at considerable risk. CONCLUSIONS: These findings identify the potential of a life course perspective for understanding why some women become infected with HIV and others do not, as well as the differentials in HIV prevalence that originate from the sequence of sexual and marital transitions in one's life. The analysis suggests, however, the need fo …

"Deadly", "fierce", "shameful": notions of antiretroviral therapy, stigma and masculinities intersecting men's life-course in Blantyre, Malawi.

BACKGROUND: Stigma and masculinity represent persistent barriers in delivering successful HIV interventions to men. Our study examined community perceptions of HIV and anti-retroviral therapy (ART) and their implications for men on ART across the life course in Blantyre, Malawi. METHODS: Our qualitative study is based on 72 face-to-face semi-structured interviews. Participants were selected purposively and included men on ART (with suppressed and unsuppressed viral loads), adult male community members irrespective of HIV status and other HIV stakeholders such as health personnel and program i

Homelessness and aging: The contradictory ordering of 'house' and 'home'.

The concepts of 'house' and 'home' are compelling and contradictory. They are compelling because they elicit the desired sentiments of perma … g in undesirable locations. In our conclusion, we draw attention to inequalities and the need for a life course perspective to ground future thinking. Our conceptual discussion is based on insights gained from a 3-year project on older homelessness in Montreal, Canada.

Evaluating the cost-effectiveness of rabies post-exposure prophylaxis: a case study in Tanzania.

Although fatal if untreated, human rabies can be prevented through post-exposure prophylaxis (PEP), which involves a course of vaccination and immunoglobulin administered immediately after exposure. However, high costs and frequent lack of rabies vaccine and immunoglobulin lead to about 55,000 deaths per year worldwide. Using data from a detailed study of rabies in Tanzania, we calculate a cost-effectiveness ratio for PEP when the WHO-recommended Essen regimen, a 5-dose intramuscular vaccination schedule, is adopted. Our analyses indicate a cost-effectiveness ratio for PEP of $27/quality-adjus

Geroscience: A call for nursing engagement.

BACKGROUND: To date, most work in the emerging field of geroscience has been conducted by basic scientists and geriatricians, rather than nurses. However, many of the fundamental mechanisms targeted by geroscience are influenced by a range of nursing-relevant factors. With a few exceptions, the nursing profession has yet to fully engage in geroscience at a time when nursing's voice and perspective are especially critical to developing the geroscience agenda. PURPOSE: To provide a high-level overview of the emerging field of geroscience, delineate its relevance to nursing, and propose holistic

Nutrition, Other Environmental Influences, and Genetics in the Determination of Human Stature.

Linear growth during three distinct stages of life determines attained stature in adulthood: namely, in utero, early postnatal life, and puberty and the adolescent period. Individual host factors, genetics, and the environment, including nutrition, influence attained human stature. Each period of physical growth has its specific biological and environmental considerations. Recent epidemiologic investigations reveal a strong influence of prenatal factors on linear size at birth that in turn influence the postnatal growth trajectory. Although average population height changes have been documente

Gender Differences in Sexually Active Life Expectancy in France.

INTRODUCTION: A life course perspective on sexual activity is gaining attention due to increased longevity and the relationship between health and sexual behavior. Building on Lindau's and Gavrilova's concept of sexually active life expectancy, we examine how including non-partnered sexual behaviors and satisfaction influences sexual life expectancy by gender in France. METHODS: We estimate sexually active life expectancy using Sullivan's method, which integrates life table data from the 2023 vital registration with sexual activity prevalence from the CSF-2023 French national survey. We also …

Examining the Consequences of the "Prevalent Life Events" of Arrest and Incarceration among an Urban African-American Cohort.

The life course perspective has traditionally examined prevalent adult life events, such as marriage and employment, and their potential to redirect offending trajectories. However, for African Americans, the life events of arrest and incarceration are becoming equally prevalent in young adulthood. Therefore, it is critical to understand how these "standard" criminal justice practices, which are designed to deter as well as punish, affect deviance among this population. This study evaluates the long-term consequences of criminal justice intervention on substance use and offending into midlife …

Neighbourhood environments and later life flourishing: A systematic scoping review.

BACKGROUND: Enabling older people to live comfortably in their own neighbourhoods for longer, known as ageing-in-place, has become a policy priority to reduce pressures on health and social care systems amid global population ageing. However, research on place and health remains predominantly deficit-focused, emphasising illness and decline (e.g. frailty, depression, and anxiety). This review adopts a positive psychology perspective and a life-course framework to synthesise evidence on how neighbourhood environments support flourishing in later life, a multidimensional state encompassing happi

Patient-reported data informing early benefit assessment of rare diseases in Germany: A systematic review.

BackgroundSince the implementation of the Regulation on Patient Integration (2003), the Act on the Reorganization of the Pharmaceutical Market (2011), and the Patient Rights Law (2013), the inclusion of patient perspectives has been further anchored in the German early benefit assessment process. During the assessment of rare disease interventions, patient perspectives are particularly important, as clinical studies are often designed acknowledging small samples and patients suffering from severe symptoms and the chronic course of the disease. Therefore, our research question is whether patien

[Pegfilgrastim vs filgrastim in primary prophylaxis of febrile neutropenia in patients with breast cancer after chemotherapy: a cost-effectiveness analysis for Germany].

OBJECTIVE: Febrile neutropenia (FN) is a common toxic side effect of myelosuppressive chemotherapy. The cost-effectiveness of primary prophylaxis (PP) of FN with granulocyte colony stimulating growth factor (G-CSF) filgrastim for six or eleven days was compared to single dose pegfilgrastim in patients with early breast cancer receiving chemotherapy (>or= 20 % FN risk) as simulated in a model. METHODS: Based on a decision-analytical model we conducted a cost-effectiveness analysis (CEA) and a cost-utility analysis (CUA) from the perspective of the Statutory Health Insurance (SHI) in Germany. A quantitative assessment of pre-dental antibiotic prophylaxis for patients with mitral-valve prolapse.

Because antibiotic prophylaxis for dental procedures in patients with mitral-valve prolapse (MVP) has been controversial, we performed a decision analysis to assess the costs and effects of the oral and parenteral penicillin regimens currently recommended for patients at high risk for bacterial endocarditis. The analysis suggests that there is a very small risk of post-dental endocarditis in MVP (4.1 cases per 10(6) procedures) which is outweighed by a greater risk of fatal reactions to parenteral penicillin (15 deaths per 10(6) courses). Parenteral penicillin prophylaxis therefore causes a ne

Marital life course events and smoking behaviour in Sweden 1980-2000.

The protective effect of marriage on smoking has been extensively established in the literature. However, less is known about the dynamics of how smoking behaviour is connected to various marital life course events, and whether there are any gender discrepancies in this respect. In this article the connection between the marital life course and smoking is analysed from a stress-related perspective controlling for other socio-economic characteristics. We use information on 81,000 individuals from the Swedish longitudinal micro-level ULF (Survey of Living Conditions) database 1980-2000, which is

When health systems invest matters: temporal imbalance and the case for life-course reorientation.

Health systems worldwide face growing pressure from population ageing and the rising burden of noncommunicable diseases. Yet policy debates still focus mainly on how much is spent, while paying much less attention to when health systems invest. In many settings, spending remains concentrated in late-stage curative care, whereas earlier phases of the life course, when risks are still developing and remain modifiable, receive less sustained investment. We describe this mismatch as a temporal imbalance: a misalignment between when health risks accumulate and when resources are allocated. We argue

The role of positive spirituality in preventing child maltreatment and promoting resilience: a perspective on policy and practice.

Child maltreatment remains a pervasive global issue with profound impacts on health, development, and societal well-being. While evidence-based strategies for violence prevention have expanded, the role of spirituality as a protective and rehabilitative factor remains underexplored in mainstream policy and practice. This perspective article examines how nurturing spiritual values across the life course-such as empathy, emotional regulation, and ethical decision-making-can contribute to violence prevention and resilience building. Drawing on recent empirical findings and global policy examples,

Developmental Issues and Mental Health.

The trajectory of human development follows a predictable course of milestones. As genomic research has brought us more understanding of the brain, the etiology of mental illness is progressing from psychodynamic origins postulated by Freud in 1960 to organic pathways and epigenetic derivations. Humans develop on normal pathways that can be derailed by poverty, illness, and trauma. When infants begin life from a healthy perspective, conditions that may follow such as a mental disorder may be moderated by healthy starts. In this paper, selected developmental theories and mental disorders are pr

Irritable bowel syndrome and recurrent abdominal pain. A comparative review.

Clinical findings on irritable bowel syndrome in adults and recurrent abdominal pain in children are reviewed to determine if what is known about each disorder can illuminate our understanding of the other. The evidence reveals striking similarities between the disorders in prevalence, course, medical and psychiatric comorbidity, family medical and psychiatric history, and association with life events. Continuity between the two disorders is also suggested by the results of follow-up and follow-back studies. The review shows the importance of a developmental perspective for understanding funct

A Systematic Review of Interventions to Improve Mental Health and Substance Use Outcomes for Individuals with Prenatal Alcohol Exposure and Fetal Alcohol Spectrum Disorder.

Individuals with fetal alcohol spectrum disorder (FASD) experience remarkably high rates of mental health and substance use challenges, beginning early in life and extending throughout adulthood. Proactive intervention can help to mitigate some of these negative experiences. Although the literature on FASD intervention is growing, there is currently a lack of consolidated evidence on interventions that may improve mental health and substance use outcomes in this population. Informed by a life course perspective, we undertook a systematic review of the literature to identify interventions that …
