BACKGROUND: Cervical lymph nodes are frequently involved in patients with lung cancer and indicate inoperability. Some guidelines recommend neck ultrasound (NUS) in patients with bulky mediastinal lymphadenopathy. Positron emission tomography (PET) is indicated for patients with potentially curable disease.

In this study, the clinical and laboratory features of children with lymphadenopathy were evaluated. Over a 3-year period, 126 patients were referred to the clinic for lymphadenopathy. Twenty-eight of cases have diseases mimicking lymphadenopathy; 98 (mean age: 86+/-55 months) have lymphadenopathy. We report a case of a patient suffering from enlargement of multiple lymph nodes and low back pain for one year. The diagnosis of ankylosing spondylitis was made by bilateral sacroilitis and HLA-B27 positivity. Lymph nodes biopsy revealed lymphoid hyperplasia. No other cause for the lymphadenopathy was found after a thorough study. Furthermore, high serum IgA and C-reactive protein which were most likely related to active ankylosing spondylitis existed together with lymph node enlargement. Dermatomyositis (DM) in adults is frequently associated with cancer. In contrast, juvenile DM (JDM) is predominantly idiopathic and rarely reported with occult neoplasm. We describe a patient who presented with DM that was found to be a paraneoplastic manifestation of nasopharyngeal carcinoma. Although rare, paraneoplastic JDM must be suspected in the presence of unusual features such as elevated inflammatory markers or lymphadenopathy. Accurate clinical assessment including appropriate biopsies is needed before starting glucocorticoid therapy.
