OBJECTIVE: The skepticism regarding the existence of dissociative identity disorder as well as the abuse that engenders it persists for lack of objective documentation. This is doubly so for the disorder in murderers because of issues of suspected malingering. By detection of muscular fibrillation, a sign of motor nerve impairment, through electromyography, diseases of the motor nervous system can be distinguished from muscular disorders, malingering, and psychoneurotic states. The site of lesion can be determined and the degree of impairment and progress of recovery can be estimated. The method has been used successfully in amyotrophic lateral sclerosis, poliomyelitis, peripheral neuritis and neuronitis, and root compression syndrome, as also for indirect evidence in muscular dystrophy and other myopathic conditions.

With proper spectacle correction, the healthy human eye is capable of seeing 20/20. Common causes of reduced vision, such as cataracts and macular degeneration, are easily detected during routine examination. In many cases, however, patients present with reduced vision for which no obvious etiology is immediately apparent. This paper deals with the most common of such presentations. Entities to be discussed include amblyopia, hysteria/malingering, monofixation syndrome, partial albinism, subtle macula disease, and occult optic nerve disease.

This article discusses the nature of psychiatric evidence in Icelandic criminal proceedings and presents findings from an investigation of the role of amnesia, malingering and overcontrolled hostility among different types of offenders. Amnesia was most common in homicide cases, and was almost invariably associated with alcohol intoxication. We report a 25-year-old woman with gradual loss of visual acuteness, during puerperium and blindness in fifteen days. Two months latter she remain steady. Visual evoked potentials using monocular checkerboard pattern-reversal, were abnormal. Clinical suspicion of voluntary alteration of VEP was considered. A second VEP exploration with binocular stimulation and maneuvers of distraction were carried out. Normal VEP were recorded. Deliberate alteration of the visual evoked potential should be considered in patients suspicious of hysteria or malingering.
