Innovative Strategies in Management of Pediatric Mandibular Fractures.

OBJECTIVES: To employ three novel techniques and evaluate their results in the management of pediatric mandibular fractures. CASE DISCUSSION: Three cases of pediatric mandible fracture were chosen. The use of impression compound as a cap splint, use of hooks for maxillomandibular fixation, and circummandibular wiring with stainless steel wire were evaluated for their results. Preoperative computed tomography (CT) scans and postoperative orthopantomograms were carried out to evaluate the results. Patients were evaluated for fracture site healing and their ability to perform functional movement…

Traumatic Brain Injury in Patients With Mandibular Fractures.

BACKGROUND: Traumatic brain injury (TBI) associated with facial fractures is a major public health concern worldwide. The rate of TBI in patients with mandibular fractures ranges from 21.3% to 39.6%. However, the risk factors for TBI in patients with mandibular fractures remain unknown. Our study evaluates these risk factors. METHODS: We retrospectively reviewed patients who presented with traumatic mandibular fractures in 2018 and 2019. Excluded were patients with no documentation of Glasgow Coma Scale. Our primary outcomes were: (1) prevalence of concomitant TBI on presentation defined as h…

Update on ladder plates for mandibular angle fractures.

PURPOSE OF REVIEW: Fractures of the mandibular angle are surgically challenging with high rates of postoperative complications. Among established fixation techniques for these injuries, Champy's tension band approach with miniplate fixation has held prominence. Rigid fixation, using two plates, also remains commonly used. More recently, geometric ladder plates, which confer greater three-dimensional stability have been developed to overcome the shortcomings of conventional fixation approaches. Herein, we aim to summarize the recent evidence surrounding the use of ladder plates and offer our ow

Unifocal versus multifocal mandibular fractures and injury location.

Mandibular fractures are frequently encountered in the trauma setting and comprise a significant number of facial injuries. The purpose of this study was to evaluate the prevalence and injury patterns of unifocal and multifocal mandibular fractures using thin-section imaging. Following IRB approval, 220 patients with mandibular fractures identified on maxillofacial CT scans performed between October 2008 and February 2011 were retrospectively reviewed. Examinations were performed on 64-multidetector row CT scanners with axial images acquired at 1.25-mm slice thickness. The location and number …

The retromandibular transparotid approach to mandibular subcondylar fractures.

The aim of this study was to evaluate the safety, utility and morbidity associated with the treatment of mandibular subcondylar fractures using the retromandibular transparotid approach and to evaluate the stability of a single 2mm miniplate fixation system for such fractures. Forty-two cases with 48 mandibular subcondylar fractures were analysed prospectively for 12 months and evaluated for functional results, scar, postoperative complications and stability of fixation. There were three cases of suboptimal occlusal status, two cases of haematoma that were drained and resolved, eight patients

Pitfalls in endoscopic treatment of mandibular subcondylar fractures.

OBJECTIVES: The facial trauma surgeon should be able to list indications for endoscopic treatment of mandibular condyle fractures and discuss the limitations and complications of the technique. BACKGROUND: The ideal treatment of mandibular subcondylar fractures continues to be debated. Acceptable results are often obtained with conservative measures such as mandibular maxillary fixation followed by elastics. On the other hand, an open approaches result in potential risk injury to the facial nerve. These 2 arguments have cautioned many surgeons from open treatment of condylar fractures. Recent

Evaluation of sensitivity of teeth after mandibular fractures.

The sensitivity of teeth anterior to a fracture between the mental and mandibular foramina has been tested and followed up until reinnervation or 3 years has passed. This study assessed the reinnervation period, the number of denervated teeth, and their clinical importance. Fifty patients and 459 teeth were examined. Two hundred and seventy-three teeth were affected and had potentially impaired innervation. Tests after injury showed non-responsive teeth in 81% of affected teeth. Six weeks after injury, 19% of teeth were reinnervated; by 1 year after injury, 92% of initially non-responsive teet

Infant mandibular fractures: are you considering child abuse?

Mandibular fractures in infants are rare and difficult to detect. Causes include motor vehicle accidents, falls, and physical abuse. Historical, physical, and imaging aids to diagnose mandibular fractures and to differentiate abuse from accident are discussed. Three-dimensional computed tomographic scans are the preferred method of imaging. In addition, a thorough workup for physical abuse is needed.

Radiographic changes during bone healing after mandibular fractures.

The study aimed to find out the best time to undertake radiological follow-up examinations and remove fixation materials after fractures of the mandible through a retrospective study of radiographs. Serial radiographs of 325 fracture sites in 231 patients over a 10-year period were examined. Outcome was measured by radiographic features of healing at less than 2, 2-3, 3-4, and 4 or more months. Osteogenic change (osteogenesis and union) was the best radiographic criterion for evaluating follow-up radiographs. This change started to predominate 1-2 months after injury in patients less than 18 y

[Mandibular condyle fractures: conservative treatment versus surgical treatment].

A review of medical literature on the various principles of medical care of condylar fractures confirms that the treatment is still controversial. For a long period of time, the conservative functional treatment of condylar fractures was a dominant philosophy. However, rigid internal fixation of condylar fractures is increasingly accepted as an alternative which presents an improved stability provided by the use of plate and screw fixation techniques. The purpose of this study is to compare the clinical and radiographic long-term results of patients with condylar fractures treated between 1984

Complications of nonrigid fixation of mandibular angle fractures.

This retrospective study analyzed complications in 96 patients with 99 mandibular angle fractures treated during a 3-year period with either closed reduction or nonrigid means of fixation combined with maxillomandibular fixation. An overall complication rate of 17% occurred. Infection was the most common complication, occurring in 17 fractures. Thirteen fractures had infection as the only complication; in the remaining four patients, infection was combined with malunion/malocclusion. The results of this study show that mandibular angle fractures in an inner-city population are associated with
