Obliteration of persistently discharging open mastoid cavities is one surgical option to achieve a dry ear. All the currently described techniques involve the use of random pattern local tissue flaps or free grafts. Ten patients have undergone obliteration procedures using the vascularized temporoparietal fascia flap. This resulted in rapid epithelialization by six weeks in seven out of 10 cases. The remaining three patients have persistent non healed areas over the medial attic wall, but are not troubled by otorrhoea. The indications, technique and complications of mastoid obliteration by thi…

Zinc sulfate has been used for therapy of granulations of the trachea and ear that have not responded to usual treatment. This paper will report on current indications for zinc sulfate and the resulting success in managing granulomata of the ear canal and mastoid bowl. Some mastoid bowls have been large and hard to clean, especially in the posterior and lateral parts. A method of cleaning using indirect otoscopy and curved aspirators in conjunction with adequate lubrication will be described. Treatment of otalgia in both the ear canal and mastoid bowl due to otitis externa sicca and results of…

Acute mastoiditis is a commonly occurring condition in children and adults, and one that most radiologists will come across at some point during their on-call duties. Acute mastoiditis is usually clinically apparent. However, the complications, especially the intracranial ones, can be more insidious and may have fatal consequences. Therefore, it is imperative that the radiologist is well versed in identifying these. Local spread of infection from the mastoids and middle ear cleft may occur via four routes: bone erosion, thrombophlebitis, periphlebitis, and via the anatomical pathways. The role…

Manifest fungal infection of the middle ear, fungal mastoiditis, is a very rare entity, which is almost exclusively seen in immunocompromised patients. The authors present a case of fungal mastoiditis in a 52-year-old woman without immunocompromise. The patient presented with acutely progressing symptoms of hearing loss and dysequilibrium. Bony fistula of the semicircular canal was noted on CT scans and a marginal perforation of the tympanic membrane was also seen. Her hearing recovered following the surgery, which revealed massive granulations and proliferation of fungi but no cholesteatoma i…

OBJECTIVE: Oral antibiotic use may have changed the incidence and microbiology of otitic intracranial complications. We reviewed cases of acute mastoiditis to document: (1) incidence of intracranial complications; (2) risk factors; and (3) identify pathologic organisms.
METHODS: A retrospective study of children at a tertiary care children's hospital with acute mastoiditis from July, 1986 through June, 1998.
RESULTS: 118 children with acute mastoiditis were identified. Eight patients (6.8%), ages 20 months to 14 years, had intracranial complications related to acute mastoiditis. Three childr…

To estimate the incidence of acute mastoiditis and identify predictors for mastoid surgery, a retrospective case record study of 38 children hospitalised for acute mastoiditis in Oslo from 1989 to 1998 was performed. Median age at diagnosis was 18 months and 13 (34%) of the children received mastoidectomy. Compared to the period 1970-1979, the incidence of mastoidectomy was significantly reduced. Only seven children (18%) had experienced acute otitis media prior to the current episode. Symptom duration of 6 days or more prior to hospitalisation and elevated white blood cell counts and C-reacti…

The environment versus the genetic influence on the size of the mastoid pneumatization has been debated for years. A concrete evidence of the influence of environmental factors (e.g. inflammation) or heredity on pneumatization is still lacking. In this study the extent to mastoid peumatization in thirty otosclerotic ears was compared with control ears. The size of mastoid pneumatization was measured by the use of Law lateral x-ray projection with the help of planimetry. It was found that the average pneumatized area in otosclerotic ears was 14.73+ -3.55 cm2 compared to 10.60+3.62 cm2 in contro…

Lemierre syndrome seldom follows an episode of pharyngotonsillitis. Characteristically, it is comprised of septic thrombosis of the internal jugular vein and bacteremia, leading to lung emboli and metastatic abscess formation. We describe Lemierre syndrome that complicates an acute mastoiditis, with considerations regarding its pathogenesis and management. Despite its sporadic occurrence, awareness of Lemierre syndrome is important, since early recognition reduces both the morbidity and mortality associated with it.

The immunocompromised host is subject to a variety of opportunistic infections. Mycotic infections, including invasive fungal sinusitis, are a dreaded complication in immune deficient children. Fungal mastoiditis has rarely been described in this population. Our experience with 2 cases of fungal mastoiditis in immunocompromised children is reviewed. Case histories describing aggressive medical management with and without surgical intervention and a review of the literature are presented.

Wegener's granulomatosis is an uncommon autoimmune disease. It has a peak incidence in the fifth decade of life, with a slight male predominance. Nasal problems are the predominant presentation, with rare otological manifestations. We report 2 cases of Wegener's granulomatosis presenting as mastoiditis in 2 young female patients. We conclude that young patients presenting with mastoiditis unresponsive to conventional medical and surgical therapy should be investigated for Wegener's granulomatosis.

BACKGROUND: To analyse the late results of mastoiditis and operative intervention as well as critical classification of our own indication for operation we invited 298 operated patients to follow-up examinations.
PATIENTS AND METHODS: 218 of 298 patients operated on, took part in these examinations. The follow-up time ranged between 2 and 18 years.
RESULTS: Altogether 311 ears were operated. Out of these, 69 patients had presented with classical mastoiditis and 242 with "masked" type. Microscopic examination of the operated ears showed that 284 (91.3%) had normal tympanic membranes, 13 (4.2%…

In order to investigate the early diagnosis and proper management of the cholesterol granulomatous tympanomastoiditis, 6 cases verified by pathology and surgery were reviewed. All of these cases had a longterm or a historical otitis media. Among them, different clinical features were presented, such as chronic serous otitis media, idiopathic blue eardrum, or as a primary occupying lesion of the middle ear. Depend on the extension of the disease, surgical procedure varied in the 6 cases, including exploratory tympanotomy, ventilation tube insertion, antroatticotomy, simple mastoidectomy, and ra…

A study was undertaken in 43 patients to determine the role of surgery in tuberculous mastoiditis. Cortical mastoidectomy was performed on five patients (Group I). Incision and drainage of a post-auricular abscess, removal of sequestrum and meatoplasty in eight (Group II). Thirty patients had no ear surgery (Group III). Of the 17 patients with facial palsy, three were in Group I, two in Group II, 12 in Group III. The patients in all three groups were treated with anti-tuberculous drugs for a period of no less than six months. The average time taken for the otorrhoea to subside and granulation …

Thirty children with acute mastoiditis were identified over a 12-year-period and their hospital records were reviewed retrospectively. All had abnormal tympanic membranes and 26 (87%) had swelling above or posterior to the ear that deviated the pinna. Findings on mastoid roentgenograms included clouding (n = 12) and osteitis (n = 7); six were normal. From 13 patients, bacteria were recovered from normally sterile sites and included Pneumococcus (n = 5), group A streptococcus (n = 3), Haemophilus (n = 2), and anaerobes (n = 3). Complications occurred in 13 children, including subperiosteal absc…

To our knowledge, atypical mycobacterium has not previously been implicated in de novo acute mastoiditis. We encountered a case in which Mycobacterium avium was the causative agent in acute mastoiditis, in the absence of any bacterial infection.
