[Treatment of meconium ileus equivalent].

A 12-year old boy with mucoviscidosis developed a mechanical obstruction of the small intestine. The diagnosis of a meconium ileus equivalent was made and a conservative treatment was started. After this treatment failed, a laparotomy was performed which verified the diagnosis of a meconium ileus equivalent. It was possible to move the inspissated feces inside the terminal ileum into the ascending colon. The postoperative course was uneventful. The etiology, differential diagnosis, and both conservative and operative treatment of this complication of the mucoviscidosis are discussed.

Plain film diagnosis in meconium plug syndrome, meconium ileus and neonatal Hirschsprung's disease. A scoring system.

Abdominal plain films of 133 neonates, with 82 cases of meconium plug syndrome (MPS), 27 cases of meconium ileus (MI) and 24 cases of neonatal Hirschsprung's disease (HD), were reviewed to assess the value of such radiographs for diagnosis. The radiographs were examined according to a list of 11 parameters. By using multivariate discriminant analysis, it appeared that 4 parameters i.e. dilatation of bowel loops, varying loop calibre, fluid levels and colonic gas were most important in discriminating among the three disorders. For each parameter the weight (in points) was derived. To classify p…

Meconium is not enough: look for the hole!

Anorectal malformations are usually diagnosed at birth, and the diagnosis is usually readily apparent in boys and girls by direct inspection of the perineal region. However, some of the less complex lesions, often referred to as 'low lesions', such as rectovestibular fistula in girls, may not be diagnosed at birth. These children are able to pass meconium, and it is only when the narrow fistula fails to pass formed stool later in infancy or childhood does the child become symptomatic. It is important to emphasise that the diagnosis of these defects should be made during neonatal predischarge e…

Diagnosis of meconium peritonitis by orchidopexy.

Meconium peritonitis is an uncommon condition almost invariably associated with structural ileal obstruction or meconium ileus. Rarely meconium peritonitis may occur in an isolated form when the clinical course may be benign. We report a case where evidence of meconium peritonitis was only discovered during orchidopexy.

The incidence of meconium-aspiration in Hawaii.

Meconium in the amniotic fluid was found in 2,633 obstetrical patients and meconium-aspiration occurred in 77 cases out of 14,527 deliveries. Although the incidence of meconium in the amniotic fluid increased significantly at 39 weeks, a corresponding significant increase in meconium-aspiration did not occur until 41 weeks gestation. All deaths associated with meconium, as well as 84% of the cases of severe meconium-aspiration syndrome, occurred in infants born of patients with oligohydramnios and a gestational age of 41 weeks or greater.

The pulmonary vasculature in meconium aspiration.

Some studies have suggested that pulmonary hypertension in the newborn with meconium aspiration can be attributed to a primary prenatal increase in pulmonary arterial musculature; but this concept has been controversial. To examine this question, we reviewed 62 infants autopsied at The Johns Hopkins Hospital, 24 of whom demonstrated meconium aspiration, 20 with meconium staining but no aspiration, and 18 with abruptio placentae without meconium aspiration or staining. Clinical and pathologic features were evaluated and cross-sectional arterial medial area was determined at the junction of the …

[The meconium ileus equivalent in mucoviscidosis].

Meconium ileus equivalent (MIE) complicating cystic fibrosis of the pancreas (CF) increases in frequently with increasing age of patients. In the present paper the course of 11 children and adolescents with MIE diagnosed and treated at the University Dept. of Paediatrics in Zürich during the last 15 years, i.e. 9% of 120 CF patients, is analyzed. 9 were successfully managed by medical treatment alone, 1 three months old infant was treated surgically with no later relapse and a 26 years old patient with a chronic deleterious course leading to opiate dependency could be relieved only by ileostom…

The pathology of meconium ileus equivalent.

A case of meconium ileus equivalent in a 25-year-old woman with cystic fibrosis is reported and the pathology of this condition is described. On gross examination the lumen of the terminal ileum was completely obstructed by a mass of inspissated faecal material with a putty-like consistency. The principal microscopic findings were plugging of the mucosal crypts with mucoid secretion, distention of goblet cells and the presence of a thick layer of mucus, admixed with faecal material, adherent to the mucosal surface. Mucin histochemistry demonstrated sulphomucin in the terminal ileum, although a…

The spectrum of meconium disease in infancy.

Meconium diseases in infancy cannot be neatly separated into discrete categories of meconium plug syndrome, meconium ileus, and meconium peritonitis; nor can the therapy of each condition. A patient with any of the three may or may not have cystic fibrosis. All babies with any form of meconium obstruction or atresia must have a sweat chloride test to confirm or rule out this diagnosis. Repeated gastrografin enemas can decrease the need for operative intervention in all babies with meconium plug syndrome and in selected babies with meconium ileus. Babies with meconium diseases can now be manage…

The radiology of Meconium Ileus Equivalent.

Meconium Ileus Equivalent is a condition which occurs in patients with cystic fibrosis where the ileum becomes obstructed by bowel content of abnormal consistency. Intercurrent chest infection or absence of pancreatic extract supplements are often the exacerbating cause. Plain films of the abdomen show small bowel obstruction and in some a nodular pattern in the small bowel, the nodules being larger than those seen in quiescent cystic fibrosis. Gastrografin enemas and/or enemas with acetyl cysteine can be used in the treatment of the condition, the Gastrografin having the advantage of confirmi…

Meconium granulomas of the tunica vaginalis.

Meconium peritonitis results from perforation of the gastrointestinal tract during fetal life. Involvement of the tunica vaginalis may be the sole presenting clinical manifestation of the disease in the unusual event of the gut perforation resolving spontaneously. In such instances radiologically detectable calcification in the abdomen and scrotum is an essential diagnostic point. A case is described in which a baby had hydroceles and bilateral intrascrotal nodules but in which calcification was radiologically undetectable, presumably owing to its having undergon resolution. The typical histol…
