The ageing meniscus: Pathophysiology, epidemiology and management.

The meniscus is a fibrocartilaginous structure crucial for load distribution, shock absorption and knee stability. With advancing age, its biomechanical and biochemical integrity declines through progressive cellular senescence, accumulation of advanced glycation end-products, reduced vascularity and altered collagen cross-linking. These changes predispose to structural damage most often seen as horizontal cleavage lesions-the hallmark of meniscal ageing-best understood as 'wrinkles' of the meniscus rather than true pathology. Such lesions are often asymptomatic and incidental on MRI. However,…

MR imaging evaluation of the postoperative meniscus.

MR imaging has been widely evaluated in the assessment of patients with recurrent or residual symptoms following meniscal surgery. Importantly, the causes of such symptoms may relate to failure or complication of the surgical procedure, a possible recurrent or residual meniscal tear, or may be related to other causes of joint symptoms, including tears of the contralateral meniscus, or local hyaline cartilage, or marrow abnormalities subjacent to or distant to the meniscal surgical site. The complex diagnostic issues involved in the MR imaging evaluation of the postoperative meniscus were ident…

Magnetic resonance imaging of the postoperative meniscus.

Imaging of the postoperative meniscus is a challenge. Nevertheless, magnetic resonance imaging (MRI) of the symptomatic knee after meniscal surgery is a valuable diagnostic study of both the menisci and the entire joint. At present, symptomatic patients who have had partial meniscectomy of less than 25% may be evaluated by MRI. For those with partial meniscectomy of greater than 25% or after meniscal repair, direct or indirect magnetic resonance arthrography (MRA) should be considered. Currently, the decision of whether to perform direct (intra-articular) versus indirect (intravenous) MRA must…

Dislocating anterior horn of the medial meniscus.

Dislocating anterior horn of the medial meniscus was found in 15 knees of 13 patients during arthroscopic examinations done between 1992 and 1995. All of them were available for follow-up evaluation (4 by telephone). There were 11 men and 2 women (average age, 28 years; range, 17 to 49 years). Nine knees had a history of trauma. Only 1 knee had had trauma in two bilateral cases. Duration of symptoms was an average of 3.3 years (range, 3 months to 10 years). The knees were stable clinically. Arthroscopy revealed associated lesions in 13 knees; hypertrophic medial plicae, meniscal, chondral and …

Development and internal validation of the hypermobile lateral meniscus score (HLMS): A diagnostic model for isolated hypermobile lateral meniscus.

BACKGROUND: A subset of patients presenting with persistent rotational knee symptoms demonstrates normal or equivocal magnetic resonance imaging despite clinically significant lateral meniscal instability, making preoperative diagnosis of isolated hypermobile lateral meniscus (HLM) challenging. PURPOSE: To develop and internally validate the Hypermobile Lateral Meniscus Score (HLMS), a multivariable diagnostic model for the preoperative identification of arthroscopically confirmed isolated HLM. METHODS: A retrospective multicenter diagnostic cohort study screened 214 consecutive patients und…

Prevalence and correlation analysis of lateral discoid Meniscus with degeneration or injury of the Meniscus and articular cartilage in the entire tibiofemoral joint.

BACKGROUND: The current discussion comprehensively addresses the injury and treatment of discoid lateral meniscus. However, it is important to note that an unfavorable biomechanical environment caused by the discoid lateral meniscus not only leads to self-injury but also has a significant impact on adjacent structures. Therefore, the aim of this study was to investigate the relationship between discoid lateral meniscus and injuries in both the meniscus and cartilage within the total tibiofemoral joint. METHODS: Patients undergoing knee magnetic resonance imaging examinations at our outpatient …

Effect of cell seeding density on matrix-forming capacity of meniscus fibrochondrocytes and nasal chondrocytes in meniscus tissue engineering.

The presence of intact menisci is imperative for the proper function of the knee joint. Meniscus injuries are often treated by the surgical removal of the damaged tissue, which increases the likelihood of post-traumatic osteoarthritis. Tissue engineering holds great promise in producing viable engineered meniscal tissue for implantation using the patient's own cells; however, the cell source for producing the engineered tissue is unclear. Nasal chondrocytes (NC) possess many attractive features for engineering meniscus. However, in order to validate the use of NC for engineering meniscus fibro…

A new arthroscopic classification of degenerative medial meniscus root tear that correlates with meniscus extrusion on magnetic resonance imaging.

BACKGROUND: To determine a new classification system for medial meniscus root tears (MMRT) based on arthroscopic findings. METHODS: 24 knees (55%) belonged to the nondisplaced or overlapped group, and 20 knees (45%) to the widely displaced group. Absolute meniscal extrusion was defined as distance between outer edge of the articular cartilage of tibial plateau and meniscal outer edge. Relative extrusion was defined as extruded meniscus width divided by entire meniscal width, multiplied by 100. The proportion of knees with major (>3 mm) extrusion were compared in two groups, as were the severi…

New meniscus repair technique for peripheral tears near the posterior tibial attachment of the posterior horn of the medial meniscus.

We introduce a suture technique to repair a peripheral tear near the posterior tibial attachment of the posterior horn. A suture hook was in … m the anteromedial portal; it was passed through the joint space until it penetrated the inner torn meniscus. The PDS suture loaded within the needle was pushed into the joint and picked up through the posteromedial portal. The needle was pulled out of the torn meniscus and readvanced over it while the suture was kept loaded. The other limb of the suture f …

Novel Meniscus Transfer Technique: A Case Report.

The meniscus is an essential component of the knee joint, acting as a shock absorber as well as assisting in the transmission of forces. Due to the meniscus importance of the knee, much of the current literature focuses on treatment techniques that can spare and repair the meniscus when it is torn. The unique vasculature of the meniscus often makes repair difficult or, in many cases, impossible. A current focus within orthopedics has been on meniscal allograft transplantation to fill this gap. The lack of a universal surgical technique for graft fixation, along with the current failure rates, …

Tibial Tuberosity Fracture Combined With Entrapped Meniscus.

Tibial tubercle fractures are rare injuries that occur in adolescents most commonly during athletic activities. Fractures are typically identified by physical examination and radiographs alone; however, this often underestimates the extent of injury. Soft tissue injuries are often associated with tibial tubercle fractures and can be missed by plain radiographs. The authors report on a patient whose meniscus was entrapped in the fracture site during the initial operative reduction and fixation. Magnetic resonance imaging performed 1 year after continued symptoms revealed that a tear of the ante…

Examination of refractory discoid lateral meniscus injury.

BACKGROUND: Lateral discoid meniscus (LDM) should be treated and preserved with saucerization and/or suture repair. However, repair of the meniscal hoop structure is sometimes difficult due to displacement or large defects. In this study, we aimed to examine tear patterns based on the Ahn classification in those requiring meniscal repair and those undergoing subtotal meniscectomy. METHODS: Twenty-three patients were evaluated (mean age, 27.4 years; mean follow-up period, 2.5 years). The following were evaluated: displacement morphology based on the Ahn classification, site of tear under arthr…

CT diagnosis of bucket handle meniscus tear.

Bucket handle meniscus tears are difficult to diagnosis and are one of the most frequently missed tear patterns on magnetic resonance imaging (MR). However, this meniscal injury often requires arthroscopic intervention and preoperative characterization of the tear is important to orthopedic surgeons for treatment planning. MR is currently the best imaging modality for evaluating meniscal tears, but trauma patients in the emergency room more often undergo radiographs and further evaluation with computed tomography (CT) exams. To date, there is one study from 1988 that reported the value of CT i…

Discoid lateral meniscus: importance, diagnosis, and treatment.

Discoid lateral meniscus (DLM) is a common anatomic variant in the knee typically presented in young populations, with a greater incidence in the Asian population than in other populations. As DLM is a congenital anomaly, the ultrastructural features and morphology differ from those of the normal meniscus, potentially leading to meniscal tears. Snapping and pain are common symptoms, with occasional limitations of extension, in patients with DLM. Examination of the contralateral knee is necessary as DLM affects both knees. While simple radiographs may provide indirect signs of a DLM, magnetic r…

High Tibial Osteotomy and Medial Meniscus Transplant.

Concomitant high tibial osteotomy and meniscal allograft transplant is indicated in patients with medial meniscus deficiency and varus deformity. A synergistic relationship likely exists between knee realignment and restoration of meniscal and chondral tissue. Therefore, it is important to address all sources of pathology, such as meniscal or chondral defects, when addressing axis malalignment. In this article, we review the indications, preoperative considerations, technique, postoperative rehabilitative protocol, and outcomes in patients undergoing high tibial osteotomy with medial meniscal …
