Conservative Management of Metatarsalgia and Lesser Toe Deformities.

There are several forefoot conditions that can result in metatarsal head pain. Various points of the gait cycle can predispose the metatarsal heads to pain based on intrinsic and extrinsic imbalances. Metatarsalgia can further be classified according to primary, secondary, or iatrogenic etiologies. Within these groups, conservative management is the first line of treatment and can often obviate surgical intervention. Depending on the cause of pain, proper shoewear, orthoses, and inserts coupled with targeted physical therapy can alleviate most symptoms of metatarsalgia and lesser toe deformiti…

Proximal Gastrocnemius Release in the Treatment of Mechanical Metatarsalgia.

BACKGROUND: Gastrocnemius shortening causes an equinus deformity that may clinically manifest in foot disorders, including metatarsalgia. We use this term to describe pain localized to the metatarsal heads. The purposes of this prospective study were to review the effect of medial gastrocnemius proximal release on ankle dorsiflexion and assess the outcome of this technique on pain and functional limitations in patients who have mechanical metatarsalgia and isolated gastrocnemius shortening. METHODS: We prospectively followed a consecutive series of 78 feet in 52 patients with metatarsalgia wh…

Study of the metatarsal formula in patient with primary metatarsalgia.

OBJECTIVES: The aims of this study were (i) to ascertain the prevalence of different types of metatarsal formula among patients with primary metatarsalgia; (ii) to compare the variable of "shortening of the first metatarsal in relation to the second" (I/II) between the metatarsalgia and control groups; and (iii) to analyze the intra and interobserver concordance by means of Morton's transverse line method and Hardy and Clapham's arc method. METHODS: A cross-sectional observational study was conducted on 56 patients by means of radiographs on their 112 ft, of which 56 were in the metatarsalgia…

MR imaging and ultrasound of metatarsalgia--the lesser metatarsals.

Metatarsalgia is a common problem for many in the community. The condition includes many different entities, such as interdigital neuroma, synovitis or metatarsophalangeal joint instability, Freiberg infarction, stress fractures, and systemic disorders. Many patients presenting with metatarsalgia have a combination of diagnostic abnormalities. The key is to establish the principal pathology and from there construct an appropriate treatment regimen.

Metatarsalgia in hallux valgus deformity: a pedographic analysis.

The purpose of this study was to analyze the plantar loading patterns in patients with hallux valgus (HV) with and without symptoms of metatarsalgia in order to find predictive pressure variabilities which are likely to result in metatarsalgia. Two groups of subjects (100 feet) volunteered for this study. Both groups provided the diagnosis of HV deformity. Group A (50 feet) exhibited a metatarsalgia symptomatology, whereas group B was asymptomatic. Plantar loading patterns were assessed using the two-step method and data were collected by the use of a capacitive pressure measurement platform. …

Plantar fat pad atrophy: a cause of metatarsalgia?

The aim of this prospective study was to evaluate if atrophy of the plantar fat pad in splay-foot deformities was a major cause of metatarsalgia. A sonographic method of measuring the thickness of the plantar fat pad under the second and third metatarsal heads was developed. The method was tested on 25 volunteers and the intraobserver reliability was calculated to be 97.1% for the second metatarsal and 96.7% for the third metatarsal. Sonographic measurement of the plantar fat pad was then performed on 100 symptomatic feet Pain frequency and pain intensity were determined by using a verbal rati…

Metatarsalgia due to insufficient support by the first ray.

Sixty-five feet with metatarsalgia due to inadequate support by the first ray are presented. A classification of different types of deficient anteromedial support is proposed in relation to the morphology of the first ray. When there is deficient anteromedial support and the first ray is normal, the ray is elevated and may or may not be associated with supination of the forefoot. When there is elevation of the first ray alone, operations are proposed which restore normal anteromedial support. The decision for choosing a particular type of operation is determined by an analysis of the cause of …

The intermetatarsophalangeal bursa--its significance in Morton's metatarsalgia.

The intermetatarsophalangeal bursa was investigated by dissection, radiography and injection. In the web spaces between the second and third and the third and fourth digits the bursa lies superior to the transverse metatarsal ligament but projects distally to it, closely applied to the neurovascular bundle. Tissue from the web spaces of patients with classical Morton's metatarsalgia often shows lymphocytic infiltration, with additional fibrinoid necrosis of the bursal wall. It is suggested that inflammatory changes in this bursa could account for the pathological and histological findings in t…

[Localisation and pathogenesis of Morton's metatarsalgia (author's transl)].

Todays concept of Morton's metatarsalgia is seen as a pseudoneuroma of the plantar digital nerve. Since clinic and pathogenesis tally, one should abandon the idea that Morton's metatarsalgia consists of interdigital pain (mainly in the 3rd space) and accept it as a pfeudoneuroma due mainly to pressure on the plantar digital nerve.

The Role of First Ray Insufficiency in the Development of Metatarsalgia.

Two theories exist in the development of central or transfer metatarsalgia. First, as the severity of hallux valgus increases, there is mechanical overload of the second metatarsal. Second, increased relative lesser metatarsal length is thought to contribute to metatarsalgia. It is imperative, in the treatment of first ray disorders (hallux valgus or hallux rigidus), to not overshorten the first ray when addressing the first ray pathologic condition. Treatment of metatarsalgia in the setting of failed hallux valgus correction can be treated with both conservative and surgical options.

Gastrocnemius Recession in the Setting of Metatarsalgia: The Baumann Procedure.

A fundamental etiologic component of metatarsalgia is the repetitive loading of a locally concentrated force in the forefoot during gait. In the setting of an isolated gastrocnemius contracture, weight-bearing pressure is shifted toward the forefoot. If metatarsalgia is considered an entity more than a symptom, evaluation of gastrocnemius contracture must be a part of the physical examination, and gastrocnemius recession via the Baumann procedure alone, or in combination with other procedures, considered as an alternative treatment in an attempt to restore normal foot biomechanics.

Metatarsalgia in Metatarsus Adductus Patients: A Rational Approach.

Metatarsus adductus (MA) is a congenital condition resulting in adduction of the forefoot at the tarsometatarsal joint, medial metatarsal deviation, supination of the hindfoot through the subtalar joint, and plantarflexed first ray. The exact underlying pathophysiology remains elusive. There is increasing evidence highlighting the importance of recognizing MA as an associated deformity that complicates management of hallux valgus (HV). Unfortunately, metatarsalgia and lesser toe pathology is also common in this population. We present a review regarding the epidemiology, pathomechanics, and a c…

[The Primary Metatarsalgia: Pathogenesis, Biomechanics and Surgical Treatment].

This paper presents a comprehensive review on the current concept of the diagnosis and treatment of central metatarsalgia on the basis of medical literature analyses. Metatarsalgia is the term for pain in the forefoot. This is a set of symptoms corresponding to a wide range of diseases. Central metatarsalgia is a kind of metatarsalgia which arises from structural-functional changes that lead to excessive pressure in the area of metatarsal heads. The data analysis demonstrated that presently various types of osteotomies of metatarsal bones are the main surgical treatment options with the chance…

First ray mobility increase in patients with metatarsalgia.

BACKGROUND: The significance and measurement of first metatarsal hypermobility has been difficult to quantify in relation to transfer metatarsalgia. We evaluated the hypothesis that dynamic elevation of the first metatarsal relative to the second metatarsal could be measured with a simple device and would be associated with transfer metatarsalgia. We also assessed intraobserver and interobserver reliability of the simple device. MATERIALS AND METHODS: A series of 352 patients were prospectively measured for dynamic metatarsal elevation: 64 patients with transfer metatarsalgia and 288 patients…

Lesser metatarsal osteotomies. A biomechanical approach to metatarsalgia.

As with most other orthopedic conditions, a firm understanding of the normal and pathologic biomechanics of the lesser metatarsals is essential when contemplating treatment of metatarsalgia. Despite its prevalence, metatarsalgia remains a technically demanding surgical challenge. Some of the difficulty with treatment of this problem arises because of some of the controversies discussed previously. Many lesser metatarsal osteotomies have been described, and their success depends on many factors. Alleviating the correct amount of pressure underneath the metatarsal head without adversely affectin…

[Distal osteotomy of the metatarsal bones in the treatment of metatarsalgia].

Authors review briefly the development of the principles of metatarsal osteotomies and the basic concepts that seem to be the most suitable in the choice of the type and execution of the operation. The effectiveness of the surgical treatment is stressed if the following viewpoints are considered: 1. Synchronous correction of the rays II, III, and IV. 2. Simultaneously achieved abbreviation and forming of the arch. 3. Instead of internal fixation transversal lifting bandage with early weight bearing. The finding, according which, beside the telescope osteotomy of Helal, Wolf's metatarsal osteot
