Difficulty in diagnosis of nasopharyngeal carcinoma.

Due to difficult anatomical position nasopharyngeal carcinoma (NPC) is always challenging problems both from the diagnostic and therapeutic corner. A 24 years old lady came to our Department of Otolaryngology and Head-Neck Surgery with the complaints of epistaxis, right sided neck swelling, nasal obstruction and headache. On digital palpation Nasopharyngeal mass was found. We took biopsy from nasopharynx under indirect vision but report was not conclusive. Then we did CT scan, nasendoscopy. Nasendoscopy showed bilateral ethmoidal polyp with nasopharyngeal mass. We took biopsy from the nasophar

Nasopharyngeal carcinoma with pericardial metastasis.

Nasopharyngeal carcinoma (NPC) is prevalent in Taiwan and is characterized by a high frequency of nodal metastasis. The most common organs with distal metastases are the bones, lungs, and liver, with extremely rare cases to the pericardium. Herein, we report a rare case with NPC who presented with dyspnea and orthopnea. Serial studies, including pericardial biopsy, revealed NPC with pericardial metastasis and pericardial effusion. The tumor cells of both the original and metastatic tumors were positive for Epstein-Barr virus by in situ hybridization. This is the first histologically confirmed

Nasopharyngeal carcinoma: the next challenges.

Nasopharyngeal carcinoma (NPC) differs from other head and neck cancers in its aetiology, epidemiology and potential therapeutic options. Despite cure for the majority of the patients, challenges still exist in the prevention of disease relapse, treatment of patients with refractory or metastatic NPC and the management of long-term toxicities. This article discusses the specific challenges in pushing the boundaries of NPC treatments further, with an emphasis on prognostic/predictive markers, molecularly targeted therapies, immunotherapies and the areas of interest with regard to long-term toxi

Nasopharyngeal cylindrical cell papilloma.

INTRODUCTION: Cylindrical cell papillomas are rare tumours which usually arise in the sinonasal region. CASE REPORT: We report a case of a nasopharyngeal cylindrical cell papilloma in a 56-year-old man who presented with a four-month history of right-sided hearing loss, otalgia, vertigo and tinnitus. Investigation revealed a soft, nodular lesion obstructing the pharyngeal opening of the right eustachian tube; this was treated by wide endoscopic excision. CONCLUSION: Cylindrical cell papilloma is a possible cause of eustachian tube obstruction in adults. Effective treatment of these lesions u

Nasopharyngeal angiofibroma: rare presentations.

Nasopharyngeal angiofibromas are benign tumors of the nasopharynx that occur in adolescent boys. The extensive extensions and various clinical presentations of the tumor have been well documented in the literature. Very rarely, these tumors may extend outside the nasal cavity as a large polypoid mass. Two interesting cases of angiofibroma with atypical presentations, showing extensive spread in multiple directions, are reported here. In one case, the angiofibroma exited the right nostril as a large polypoidal mass and also presented as an intraoral tumor in the right upper gingivo-alveolar reg

Imaging of nasopharyngeal cysts and bursae.

Cysts and bursae of the nasopharynx are uncommon and seldom symptomatic when compared with malignant tumors of this region. However, it is noteworthy that in the presence of symptoms, a good knowledge of their radiological appearance is useful to establish the correct diagnosis. Cysts of Rathke's pouch, pharyngeal bursa of Luschka, Tornwaldt's cysts, retentional cysts of the seromucinous glands, oncocytic cysts, intra-adenoid cysts, branchial cysts, prevertebral or retropharyngeal abscess and pseudocysts of the nasopharynx will be discussed in this paper.

Orbital involvement by nasopharyngeal angiofibroma.

This article describes a rare cause of orbital apex syndrome in a child--juvenile nasopharyngeal angiofibroma. A high level of suspicion while treating an adolescent male with chronic nasal obstruction, epistaxis, and a nasal mass will promote early diagnosis. Multidisciplinary care is advisable, especially for advanced disease.

Epidural metastasis in nasopharyngeal carcinoma.

BACKGROUND: In nasopharyngeal carcinoma, both, a short metastasis-free interval after primary treatment and the occurrence of epidural metastasis have been associated with poor prognosis. We present the clinical course of a young patient with these two conditions and review the literature. PATIENT: A 26-year-old male with stage T2N3M0 non-keratinizing carcinoma (WHO type 2) of the nasopharynx was treated with induction chemotherapy and radical radiotherapy, 6 months after documentation of a clinical complete remission, the patient experienced metastatic disease to the C7-D1 vertebral bodies a

[A case of primary nasopharyngeal tuberculosis].

BACKGROUND: Although the prevalence of nasopharyngeal tuberculosis has decreased after the wide use of anti-tuberculous agents, recently the number of reports with the disease has slightly increased in accordance with advances in the diagnostic tools. A case of nasopharyngeal tuberculosis without any tuberculous lesions in other organs (primary nasopharyngeal tuberculosis) was reported. CASE REPORT: A 74-year-old female complained of vertigo and cervical masses, and naso-pharygo-laryngoscopy disclosed a polypoid lesion on her nasopharynx. The biopsy from the mass revealed epithelioid cell gra

Nasopharyngeal carcinoma in young patients.

Nasopharyngeal carcinoma (NPC) is rare in under 20-year-olds. Early diagnosis greatly improves survival. A retrospective review of 12 young NPC patients (seven males, five females, mean age 16 years) was performed to identify differing patient and tumour characteristics from adult NPC. Seventy-five per cent presented with neck lumps and 25% with headache. None had a family history of NPC or epistaxis. One patient had early stage NPC, and 11 had late stage NPC. Three late stage patients who received chemoradiotherapy had better clinical outcomes than six late stage patients receiving only radio

A nasopharyngeal mass: leech in the nasopharynx.

This paper presents a case with a 4-month history of nasal obstruction and intermittent epistaxis. The cause was found to be a leech in the nasopharynx by means of endoscopic examination. Leech endoparasitism, although rare, may cause serious, even lethal, complications. A high index of suspicion of leech infestation must be entertained when faced with nasal obstruction and intermittent epistaxis in children, and should be investigated surgically as with all other foreign bodies.

[Nasopharyngeal stenosis after uvulopalatopharyngoplasty].

We report a case of nasopharyngeal stenosis after uvulopalatopharyngoplasty (UPPP). A 42-year-old man underwent UPPP because of severe snoring. Two months later, a severe nasopharyngeal stenosis was observed upon examination. The stenosis was caused by the adhesion of the soft palate to the posterior pharyngeal scar. X-ray examination of the patient's pharynx revealed that the postoperative nasopharyngeal stenosis may have also been caused by the depth of the pharynx, in addition to the morphological anomaly. To avoid velopharyngeal insufficiency and nasopharyngeal stenosis, careful examinatio

The nature of juvenile nasopharyngeal angiofibroma.

OBJECTIVE: The goal of this study was to investigate the nature of juvenile nasopharyngeal angiofibroma. STUDY DESIGN: Twenty-five excised tumors were studied morphologically to confirm the histopathologic features of the tumor. METHODS: Paraffin sections of all tumor specimens were stained with hematoxylin-eosin for elastic fiber, polyclonal antibody to factor VIII-related antigen and monoclonal antibodies to smooth muscle actin, estrogen receptor, and progesterone receptor. A quantitative assessment of the vascular component changes was also performed in 14 cases. Ultrathin sections of 5 t

Malignant nasopharyngeal tumours in Iceland.

From 1965 to 1990, 46 cases of malignant nasopharyngeal tumours were diagnosed in Iceland. The incidence rate is as low as in other Western countries, 0.6/100,000 per year. Histo-pathological diagnosis were as follows: Undifferentiated carcinoma 45%; squamous cell carcinoma 30%; non-keratinizing carcinoma 7%; and plasmacytoma 9%; lymphoma 7%; rhabdomyosarcoma 2%. Four per cent were diagnosed at stage I, 13% at stage II, 29% at stage III and 54% at stage IV. The overall crude survival at 10 years from diagnosis was 28.3%. The following factors were found to have a prognostic value: Stage of dis

Trigeminal neuralgia in nasopharyngeal carcinoma.

Tumours at the skull base may show perineural infiltration of the mandibular nerve. Subsequent retrograde spread into the cavernous sinus, Gasserian ganglion, the trigeminal nerve and the pons may be seen. The patient, a known and treated case of nasopharyngeal carcinoma (NPC), complained of trigeminal neuralgia and difficulty in chewing. Magnetic resonance imaging (MRI) revealed a Gasserian ganglion and trigeminal infiltration with resultant atrophy of the muscles innervated by the mandibular nerve. Proximal cranial nerve involvement should be suspected in patients with skull base malignancy

Perinatal management of nasopharyngeal teratoma.

Nasopharyngeal teratoma is a rare benign entity, with most of the published literature represented by sporadic case reports. We present a recent case of a 23-year-old woman found to have elevated maternal serum alpha-fetoprotein and ultrasound findings of a fetal cystic mass in the mandibular region. The infant had a large nasopharyngeal teratoma protruding from the oral cavity making airway access extremely difficult. The presence of severe coexisting cardiac anomalies and respiratory disease led to death in the neonatal period. This report and the review of the literature stress the importan
