Identifying and treating the causes of neck pain.

Future studies are needed to further understand the pathophysiology of mechanical neck pain. Robust scientific evidence is sparse on which noninvasive treatments are the most beneficial and how to better select patients for particular noninvasive or invasive treatments.

Treatment of malignant pain of the head and neck.

The treatment of pain due to malignancy in the head and neck is difficult, and the pain doctor must take a special responsibility in selecting the appropriate treatment. Specialized and often combined surgical procedures may still offer the best chance of relief, particularly when tumours involve the paranasal sinuses and the petrous bone. Peripheral and central destructive procedures can prove effective, and perhaps thalamotomy is deserving of greater usage. Chronic intrathecal analgesic administration can undoubtedly prove effective, and while the unwanted features may yet have to be establi

Balance alterations are associated with neck pain and neck muscle endurance in migraine.

BACKGROUND: Migraine patients may present with both cervical and balance dysfunctions. The neck plays an important role in balance by providing substantial proprioceptive input, which is integrated in the central nervous system and influences the balance control systems. Whether balance and neck dysfunctions are associated in patients with migraine is still to be explored. OBJECTIVES: This study aimed to assess the association between the sensory organization test of balance with neck pain features, cervical strength, endurance, and range of motion in patients with migraine. METHODS: Sixty-f…

It's A Pain in The Neck: Case Report of Bedside Diagnosis of Unilateral Neck Swelling.

INTRODUCTION: Lemierre syndrome is a rare but potentially severe thrombophlebitis of the internal jugular vein. It most often presents after … disease, migraines, and recent dental extractions presented with two days of worsening right-sided neck pain and swelling. She denied fever, chills, or recent upper respiratory symptoms. Examination revealed a tender anterior neck mass without airway compromise. Point-of-care ultrasound demonstrated a 1.22 x 1.80 centimeters hyperechoic intraluminal mass w …

Neck Pain Caused by Giant Dorsal Lipoma.

Giant lipomas are uncommon in the neck or craniofacial region. A 55-year-old male presented with a large pendulous mass arising from the dorsal aspect of his neck, which he had known about for 20 years. Discomfort during sleeping, frequent headaches, and chronic neck pain were his chief complaints. Computed topography revealed a well-circumscribed lipomatous lesion. A decrease in cervical spine lordosis was noted on sagittal x-rays. Surgical excision with local flap coverage was performed. The specimen weighed 2800 g. The patient reported immediate resolution of neck pain, and complete resolut…

Delayed Diagnosis of Neck Pain: Eagle Syndrome.

Eagle syndrome is a rare condition characterized by an abnormally elongated styloid process with or without abnormal direction and/or ossification of the styloid ligament. Clinically, it consists of throat and neck pain radiating to the ear. Here, we present the case of a 34-year-old female with the complaint of left-sided neck pain below the ear for the past year. The patient had tried different analgesics after seeing different doctors, but the pain did not resolve. After conducting radiological investigations at the hospital, a diagnosis of Eagle syndrome was made. The patient was treated w…

[Unilateral atypical neck pain in Eagle syndrome].

This article is an introduction to Eagle syndrome as a differential diagnosis in patients with lateral neck pain, a symptom, which is often difficult to diagnose and where the accepted treatment covers a variety of forms. This was the case for a 51 year-old man with lateral neck pain and a crunchy sensation/sound when he turned his head. After years of pain, the right examination and a computed tomography finally suggested Eagle syndrome. The patient had severe symptoms and was treated with an operation by which part of the ossificated stylohyoid ligament was removed. The operation relieved hi…

'Central sensitization' in chronic neck/shoulder pain.

Background and purpose 'Central sensitization' (CS) may play a major role in maintaining several chronic pain conditions. CS has been proposed to play a significant role in a range of musculoskeletal pain conditions, such as trapezius myalgia, fibromyalgia, temporomandibular disorders, and low back pain. Whether CS varies over time within an individual is not known. This study evaluated (1) whether there is an intraindividual association between clinical pain and signs of CS, and (2) whether there is an inter-individual association between clinical pain and signs of CS. Methods Twenty-seven se

Can scintigraphy explain prolonged postoperative neck pain?

AIM: Anterior cervical fusion procedures are among the most commonly performed spinal operations. Investigators have reported pseudoarthrosi … cases operated for degenerative disc disease on single level but suffering postoperative prolonged neck pain were involved in the study. All cases underwent 99mTc-hydroxymethylene diphosphonate (HMDP) bone SPECT later than postoperative twelfth month. RESULTS: Results showed a correlation between severity of neck pain and fusion status. CONCLUSION: Incre …

Retropharyngeal abscess presenting as benign neck pain.

OBJECTIVE: To present a case of an uncommon presentation of cellulitis of the neck as benign neck pain. CLINICAL FEATURES: A 44-year-old man had severe neck pain and headaches for 2 weeks with an unknown cause. Minimal response to chiropractic treatment leads to coordination of treatment with the patient's primary care physician. Laboratory assessment and magnetic resonance imaging initially were viewed as insignificant but were repeated and showed a retropharyngeal abscess. INTERVENTION AND OUTCOME: Chiropractic treatment did not reduce the patient's neck pain as expected. Fusion of C1 to C…

Paroxysmal atrial fibrillation and insidious neck pain.

Paroxysmal atrial fibrillation (AF) is an arrhythmia usually secondary to autonomic imbalance, and it may occur in the absence of any structural heart disease. The case of a patient with paroxysmal AF, in whom the arrhythmia may have been a presenting symptom of a later diagnosed cervical schwannoma, is reported.

Cognitive functional therapy for persistent neck pain.

AIMS: Persistent neck pain is a leading cause of years lived with disability. The aim of the current study is to describe and evaluate cognitive functional therapy for patients with persistent neck pain. METHODS: We recruited 74 patients (73% women) aged 22-76 years, with persistent neck pain. The outcomes were neck pain at activity and rest measured by 11-point numeric rating scales (NRS), disability by Neck Disability Index (NDI) (0-100), Fear Avoidance Beliefs Questionnaire (FABQ), Hopkin Symptom Check List (HSCL-10) at baseline and post-treatment. The treatment program included 5 sessions…
