Management of Mandibular Ameloblastoma: A Case Report From a Bahrain Government Hospital.

Ameloblastoma is a locally aggressive yet benign odontogenic neoplasm. Its three main clinical variants - intraosseous, unicystic, and extraosseous - differ vastly in their biological behavior and recurrence rate. Here we present a case of intraosseous ameloblastoma in a 32-year-old male managed by wide en bloc excision. The mandible was reinforced with reconstruction plate to prevent pathological fracture, as the residual bone height was less than 1 cm. The patient has been on regular follow-up for one year with no clinical or radiological evidence of disease. This case demonstrates that marg

Combined microsurgical-endoscopic paramedian supracerebellar-infratentorial approach for resection of a pineal low-grade glioma.

The authors present the case of a 20-year-old male with a history of headaches and blurred vision found to have a pineal mass and chronic hydrocephalus. The patient initially underwent an endoscopic third ventriculostomy and pineal mass biopsy that revealed a low-grade neuroepithelial neoplasm. A microsurgery-endoscope-assisted paramedian supracerebellar-infratentorial approach was chosen and a gross-total resection was achieved. The patient's postoperative and follow-up course has been unremarkable, with early postoperative imaging demonstrating no residual tumoral mass. The operative video h

Primary intracranial intraventricular leiomyoma: a literature review.

Leiomyomas, benign tumors of well-differentiated smooth muscle and vascular collagenous tissue, usually occur in the uterus but can develop wherever smooth muscle is present. Primary intracranial leiomyomas are rare tumors. We present the case of a 40-year-old woman with a primary intraventricular leiomyoma. She consulted us for headache, dizziness, and black spots in her vision. Magnetic resonance images (MRI) disclosed a mass rooted in the posterior septum pellucidum. The tumor was totally excised by the senior author with no residual seen on intraoperative MRI. Pathological examination show

Anaplastic thyroid cancer with uncommon long-term survival.

In general, most thyroid cancers are indolent and have a slowly progressive course. The exception is anaplastic thyroid cancer. It is one of the most fatal neoplasms in humans, with median survival of 4-12 months. Here, we present a patient with anaplastic thyroid cancer who survived for more than 10 years after diagnosis. A 68-year-old man was incidentally found to have anaplastic thyroid cancer during operation for follicular neoplasm. Total thyroidectomy was performed and hyperfractionated radiotherapy was carried out. After operation, annual follow-up examinations were negative for residua

Cell biology of cancer dormancy.

Cancer dormancy is a dynamic state in which residual disseminated tumor cells persist without overt expansion yet retain the capacity to reawaken and drive metastatic relapses. In this Review, we discuss major conceptual and mechanistic advances in the cell biology of cancer dormancy. Firstly, we summarize the recognized principal forms and constraints of dormancy, including cellular and tumor mass dormancy, metabolic and extracellular matrix-dependent growth restriction, immune-mediated equilibrium, angiogenic limitation and drug-tolerant persistence states. Next, we examine organ-specific do

Kocher maneuver to facilitate cytoreduction within the foramen of Winslow.

A potential site for incomplete cytoreduction in patients with peritoneal metastases is the foramen of Winslow, especially the posterior aspect of the hepatoduodenal ligament. The Kocher maneuver can be used to rotate the duodenum, head of pancreas, and portal structures 180°. In so doing, the foramen of Winslow is clearly exposed for peritonectomy. Residual tumor at this site is a prominent cause of unnecessary treatment failure in the management of patients with mucinous appendiceal neoplasms.

Cervicomediastinal Hematoma: Atypical Presentation of a Parathyroid Carcinoma.

Parathyroid carcinoma (PC) is a rare endocrine neoplasm that typically presents with osteopenia/osteoporosis, nephrolithiasis, asthenia, and neuropsychiatric symptoms. We describe the case of a 48-year-old woman, presenting with a large painful hematoma in the cervicomediastinal area. The neck ultrasound (US) demonstrated a solid lesion measuring 40 × 80 × 55 mm, markedly hypoechoic, which extended from the right thyroid lobe to the mediastinum. The blood tests showed elevated serum calcium and parathyroid hormone (PTH) concentrations, consistent with hypercalcemic primary hyperparathyroidism.

Rapid testicular metastasis from bladder transitional urothelial carcinoma after radical cystoprostatectomy: A case report.

RATIONALE: Bladder cancer (BC) is commonly diagnosed in the urinary system and the most common subtype is transitional urothelial carcinoma (TCC). Even with the best treatment, tumor recurrence and metastases always occur. While clinicians commonly observe the metastases to pelvic lymph nodes, liver, lung, and bone, it may infrequently spread to some uncommon locations. PATIENT CONCERNS: The patient was a 67-year-old man with a diagnosis of high-grade TCC with squamous differentiation in the bladder and prostate. Subsequently, radical cystoprostatectomy, adjuvant radiotherapy, and chemotherap

Langerhans cell histiocytosis mimicking a residual cyst.

Langerhans cell histiocytosis (LCH) is an uncommon myeloid neoplasm characterized by clonal neoplastic proliferation of Langerhans-type dendritic cells associated with a reactive inflammatory infiltrate composed predominantly of lymphocytes and eosinophils. Only three cases of LCH mimicking periapical lesions have been reported in the English-language literature to date. Herein, we report a rare case of LCH involving the mandible of a 45-years-old woman mimicking microscopically and radiographically a residual cyst. The patient underwent enucleation and curettage of the lesion. Microscopically

[Trans-anal resection of low rectal neoplasms].

BACKGROUND: The rising incidence of colorectal neoplasms, and in particular those localised in the lower rectum is stressed and the therapeutic opportunities offered by the trans-anal resection technique are underlined. METHODS: The indispensable conditions for adopting a surgical approach are pointed out. These are identified as the size of the neoplasm, which should not exceed 4 cm, the fixity, site, the polypoid and non-ulcerated nature of the lesion, and the involvement of not more than 1/4 of the circumference of the bowel. Using these inclusion criteria, the authors operated on five pat

Glomus tumor of the hyoid bone: a case report and literature review.

Hyoid glomus tumors represent an exceptionally rare clinical entity. This study details a case presentation of a hyoid glomus tumor accompanied by a comprehensive systematic review, aiming to expand the clinical and pathological understanding of these uncommon neoplasms while evaluating therapeutic approaches. CT imaging revealed hyoid bone destruction with features suggestive of a borderline neoplasm. Histopathological examination demonstrated local spindle-shaped cells exhibiting a chicken claw-like morphology, which showed strong immunoreactivity for SMA, calponin, and collagen type IV - fi

Acute lymphoblastic leukemia.

Acute lymphoblastic leukemia and lymphoblastic lymphoma constitute a family of genetically heterogeneous lymphoid neoplasms derived from B- and T-lymphoid progenitors. Diagnosis is based on morphologic, immunophenotypic, and genetic features that allow differentiation from normal progenitors and other hematopoietic and nonhematopoietic neoplasms. Current intensive chemotherapy regimens have accomplished overall cure rates of 85% to 90% in children and 40% to 50% in adults, with outcomes depending on the genetic subtype of disease and clinical features at presentation. Therapy is optimized usin

[Multiple Pulmonary Metastases Seven Years After Transurethral Bladder Tumor Resection for Non-muscle Invasion Bladder Cancer:Report of a Case].

A 64-year-old man was admitted because of multiple pulmonary nodules in right upper lobe on chest computed tomography (CT). He had non-muscle invasion bladder cancer resected by transurethral bladder tumor resection seven years ago. Partial resections of the right upper lobe were carried out at the video assisted thracoscopic surgery (VATS) for diagnostic purposes. The postoperative pathological examination revealed featuraes of pulmonary metastasis of bladder cancer. Although chemotherapy after biopsy was performed, a residual tumor in right S3 has grown. Partial resection of residual tumor w

Myeloid/lymphoid neoplasms with eosinophilia and FLT3 rearrangement.

Myeloid/lymphoid neoplasms with eosinophilia and gene rearrangement are a unique category in the WHO classification, and include cases with rearrangement of PDGFRA, PDGFRB, FGFR1, and PCM1-JAK2. We report three patients presented with eosinophilia and FLT3 rearrangement: the first case with chronic eosinophilic leukemia, not otherwise specified and T-lymphoblastic leukemia/lymphoma; the second case with myeloid sarcoma; and the last case with high-grade myelodysplastic syndrome. The first case showed t(13;14)(q12;q32), which encoded FLT3-TRIP11. The patient was treated with intense chemotherap

Serous papillary carcinomas arising from the peritoneum and ovaries. A clinicopathologic and immunohistochemical comparison.

Primary papillary serous carcinoma arising from the peritoneal surface (serous surface papillary carcinoma; SSPC) is a distinctive neoplasm with a histomorphologic resemblance to serous ovarian papillary carcinoma (SOPC). To determine if these tumors are similar antigenically, we studied 13 examples of SSPC and 31 of SOPC immunohistochemically. Antibodies to several determinants known to occur in the Müllerian epithelium were employed. Both neoplasms were uniformly positive for cytokeratin and epithelial membrane antigen (EMA); in addition, SSPC and SOPC were similar in quantitative and qualit

[Clinical types of thoracic cancer. Mediastinal tumours].

Mediastinal germ cell tumours (teratomas, seminomas, and non-seminomatous malignant germ cell tumours) are a heterogeneous group of benign and malignant neoplasms. The standard treatment of mediastinal non-seminomatous malignant germ cell tumours is four cycles of chemotherapy followed by surgical resection of the residual mass. Small localized mediastinal seminomas may be treated with primary resection followed by radiotherapy. In patients with locally advanced disease, the preferred treatment is systemic chemotherapy followed by surgical resection of any residual disease. Thymomas can be loc

Insights on postneoadjuvant treatment among patients with triple-negative breast cancer and residual disease after neoadjuvant therapy: can expert consensus help to interpret the current evidence?

As treatment strategies for early triple-negative breast cancer continue to evolve, translating clinical evidence into practice can be challenging, especially as the standard of care also shifts and may differ across studies. The management of patients with triple-negative breast cancer who have residual disease after neoadjuvant therapy exemplifies a complex clinical scenario with emerging yet sometimes difficult-to-interpret evidence. Through a critical appraisal of current data and an expert consensus discussion from the Nordic region, the authors present a perspective on postneoadjuvant tr

Response-Adapted Therapy for Newly Diagnosed Multiple Myeloma.

PURPOSE OF REVIEW: The development of potent novel agents has improved outcomes for patients with multiple myeloma (MM). Heterogeneity of response to therapy, an expanding arsenal of treatment options, and cost are however major challenges for physicians making treatment decisions. Response-adapted therapy is hence an attractive strategy for sequencing of therapy in MM. Despite its successful application in other haematologic malignancies, response-adapted therapy is yet to become a standard of care for MM. We provide our perspective on response-adapted therapeutic strategies evaluated thus fa
