INTRODUCTION: Port-site metastases are a recognised complication of laparoscopy in the presence of malignancy. With the increased use of minimally invasive technology to surgically manage gynaecological malignancy, their incidence is likely to increase. Malignant metastasis along the needle tract, called "needle tract implantation" (NTI), has been demonstrated for numerous carcinomas, including prostate, colon and liver. Unsuspected microscopic adenocarcinoma of the gallbladder was identified after operation in a 66-year-old woman undergoing elective laparoscopic cholecystectomy for symptomatic cholelithiasis. An abdominal wall metastasis developed at the periumbilical incision site through which the laparoscope was introduced and through which the gallbladder was removed. A review of the available information on tumor seeding of laparoscopic tracts and drain tracts is presented, as well as implications for the further management of gallbladder cancer and other intraabdominal malignancies.

BACKGROUND AND OBJECTIVE: Orbital metastases are an uncommon condition. They may be the clinical presentation of a previously unknown malignancy. Depending on the series, the rate of orbital metastasis as a first manifestation of a malignant tumour is 20%-42%. Since September 1983, five patients with head and neck cancers and five patients with pelvic or perineal recurrences of colorectal neoplasms received 192Ir interstitial implants through flexible afterloading catheters that were modified to allow RF hyperthermia treatments of the tumor within 1 hr pre- and post-brachytherapy. After surgical removal of a primary intranasal neoplasm, an implant device, designed to deliver 192iridium (192Ir) brachytherapy, was positioned in the nasal cavity of 8 dogs. Ribbons containing 192Ir seeds were placed in the device, using an afterloading technique. Dosimetry, to a dose of 7,000 to 10,000 centiGray (cGy), was calculated to encompass the site previously occupied by the tumor and a 1-cm margin of surrounding normal tissue.
