Sequestrated Lumbar Disc Herniation Mimicking Spinal Neoplasm.

Sequestered disc fragments do not have indistinctive features and often share the clinical and radiological presentation as spinal neoplasms making their diagnosis and treatment a clinical challenge. We report a rare case of sequestered lumbar disc fragment at the level of L2-L3 in a 70-year-old male who presented to the ER complaining of six years' history of low back pain with acute onset lower extremities weakness for six days, associated with right foot drop. He was admitted for tumor workup as the MRI showed diffuse bone high signal intensity throughout the spine with a soft tissue epidur

Targeted next-generation sequencing of malignant peripheral nerve sheath tumor of the pterygopalatine fossa with intracranial metastatic recurrence.

Malignant peripheral nerve sheath tumor (MPNST) is an uncommon neoplasm that rarely involves the head and neck region. Intracranial MPNSTs unrelated to cranial nerves are highly malignant tumors with poor overall survival, probably because of infiltrating growth into surrounding brain tissue. The pathogenesis of MPNST remains unclear. There are no conclusive explanations for the mechanisms underlying the initiation, progression, and metastasis of MPNST. In this paper, we describe a case of MPNST in the pterygopalatine fossa with intracranial metastatic recurrence and review related literatures

Malignant Cutaneous Peripheral Nerve Sheath Tumour with Rhabdomyosarcomatous Differentiation (Triton Tumour) in a Domestic Cat.

Divergent differentiation is encountered frequently within human malignant peripheral nerve sheath tumours (MPNSTs). The new component is often a rhabdomyosarcoma, but in animals this specific form of divergent differentiation within MPNSTs has only been reported once (in a dog). Incisional wedge biopsy of a locally extensive, ventral abdominal wall mass, which extended from the dermis to the subcutis, from a 12-year-old female domestic shorthaired cat, was performed. The tissue was examined with routine haematoxylin and eosin staining and immunohistochemical methods. A malignant neoplasm with

Neuromuscular complications of cancer diagnosis and treatment.

Neuromuscular disorders are a common cause of morbidity in patients with cancer. They can be a direct effect of the primary malignancy, a paraneoplastic effect, or a treatment complication. Malignant neoplasms may infiltrate or compress nerve roots, plexi, and peripheral nerves, causing various sensory and motor symptoms. Electrodiagnostic testing, cerebrospinal fluid analysis, and neuroimaging are helpful in confirming the diagnosis. Treatment for neuropathies of neoplastic origin involves irradiation and chemotherapy, which may improve pain, but usually does not improve neurologic function.

Spinal adrenal cortical adenoma with oncocytic features: report of the first intramedullary case and review of the literature.

Ectopic adrenal cortical neoplasms are extremely rare, and only a few have involved the CNS. We report the first case of an intramedullary oncocytic adrenal cortical neoplasm of the spinal cord with immunohistochemical (IMHC) confirmation. A 27-year-old man presented with progressive lower extremity weakness, spastic paraparesis, decreased reflexes, and hypoesthesia below T10. A spinal myelogram showed cauda equina blockade and obliteration of sacral nerve roots. This prompted emergent surgical intervention. A well-circumscribed, approximately 3 x 2 cm, light brown to tan, intramedullary tumor

Extramedullary plasmacytoma of the tibial nerve: illustrative case.

BACKGROUND: Extramedullary plasmacytomas (EMPs) are rare plasma cell tumors forming outside the bone marrow. EMPs spread to soft tissues with a predilection for the head and neck region. Therefore, EMPs are rarely included in the differential for peripheral nerve sheath tumors (PNSTs). The authors discuss a case of tibial nerve sheath invasion by an EMP in the context of multiple myeloma (MM). OBSERVATIONS: A 70-year-old female with MM presented with right leg pain and a suspected PNST of the tibial nerve. Imaging revealed a T1-hypointense lesion in the popliteal fossa. Resection was attempte

[Occipital nerve neuralgia as postoperative complication. Views on etiology and treatment].

Occipital neuralgia might have a various etiology but the most common cause is hypertrophic fibrosis of subcutaneous tissue following trauma to the occipital region, surrounding the occipital nerve. The other important cause of neuralgia is spondylosis of the upper part of the cervical spine (C1-C2). Rare causes are-diabetes, gout and neoplasm. In the presented material we point out the possibility of the occipital neuralgia after surgery in the treatment of the tumours of ponto-cerebellar region and trigeminal neuralgia. We observed the symptoms in two groups of patients and used pharmacologi

Disc edema, transient obscurations of vision, and a temporal fossa mass.

A 56-year-old woman presented with a four-month history of transient obscurations of vision that progressed to constant visual loss. She had a nodular, lumpy-bumpy, cauliflower-like asymmetric edema of the nerve head, which suggested direct optic nerve head invasion with foreign tissue. Imaging of her intracranial contents revealed a well circumscribed gadolinium enhancing mass in the middle fossa. Histopathology of material obtained at craniotomy revealed noncaseating granulomata consistent with sarcoidosis. Central nervous system sarcoid may present either as an infiltrative granulomatous pr

Pelvic Malignant Peripheral Nerve Sheath Tumor Revealing Previously Undiagnosed Familial Neurofibromatosis Type 1 in a Nine-Year-Old Girl: A Case Report.

Malignant peripheral nerve sheath tumors (MPNSTs) are malignant neoplasms of peripheral nerve sheath differentiation and may occur in association with neurofibromatosis type 1 (NF1). We report a case of a nine-year-old girl who presented with a rapidly enlarging left pelvic mass evolving over two months, accompanied by weight loss, pain, and functional limitation of the left lower limb with ipsilateral edema. Abdominopelvic imaging revealed a large heterogeneous mass centered on the left iliopsoas with pelvic and proximal thigh extension (108×100×151 mm), and thoracic staging identified bilate

Microarray‑based identification of nerve growth‑promoting genes in neurofibromatosis type I.

As a genetic disease, neurofibromatosis type 1 (NF1) is characterized by abnormalities in multiple tissues derived from the neural crest, including neoplasms in the ends of the limbs. The exact mechanism of nerve growth in NF1 is unclear. In the present study, the gene expression profile of nerves in healthy controls and NF1 patients with macrodactylia of the fingers or toes were analyzed in order to identify possible genes associated with nerve growth. The Whole Human Genome Microarray was selected to screen for different gene expression profiles, and the result was analyzed with Feature Extr

Immunohistochemistry of soft tissue tumours: a review.

The diagnosis and classification of soft tissue sarcomas can pose difficult problems for the histopathologist. Many sarcomas are too poorly differentiated to exhibit morphological features specific enough to define their histogenesis. Using the immunoperoxidase technique with commercially available antisera as a routine adjunct to other diagnostic aids, it is possible to arrive at more accurate diagnoses on which treatment protocols can be based. In addition a better understanding of mesenchymal neoplasms and their origins can be obtained by functional immunohistochemical studies.

A Rare Case of Vagus Nerve Schwannoma Presenting as a Neck Mass.

BACKGROUND Vagus nerve schwannoma is a benign neoplasm that usually presents as an asymptomatic slow growing mass, and its presentation as a neck mass is rare. The diagnosis can be difficult to make and complete surgical excision is challenging due to the proximity of the vagus nerve fibers from which it originates. The most common symptom associated with vagus nerve schwannoma arising in the neck is hoarseness due to vocal cord palsy. CASE REPORT We report a case of a 55-year-old woman who presented to the clinic complaining of throat irritation and feeling of something stuck in her throat fo

Endoscopic removal of giant intranasal schwannoma.

Schwannoma is a benign neoplasm originating from schwann cells of the peripheral nerve sheath. Although nearly half of all schwannomas involve the head and neck region, nasal and paranasal sinus presentations are very rare in the literature. We present a case of nasal schwannoma originating from the right nasal cavity. A 59-year-old man presented with complaints of progressive right nasal obstruction and headache. Endoscopic examinations revealed a mass that filled the right middle meatus. At first glance, it was not like polyp tissue. Endoscopic sinus surgery was performed under general anest

[Clinical aspects, diagnosis and therapy of space-occupying tumors of the orbit].

In the presence of obvious, diffuse tissue proliferation in the retrobulbar space coupled with pain, increased erythrocyte sedimentation rate and a good response to corticosteroid administration, it is safe to assume, in consultation with an ophthalmologist and without doing a biopsy, that the lesion is a pseudotumour. For all other proliferative processes in the orbit and around the medial and inferior rectus muscles histological verification is indicated. The surgical approach is dictated by the site of the tumour in relation to the optic bulb and nerve. Computer tomography does not provide

The cytopathology of reactions to ventricular shunts.

A common cause of malfunctioning ventricular shunts is the occlusion of either tip by a variety of normal or reactive tissues and foreign substances. A six-year-old girl with communicating hydrocephalus and a meningomyelocele, a 48-year-old man with an ependymoma and an 11-year-old boy with a pineal germinoma had multinucleated histiocytic giant cells and ependymal cells in cerebrospinal fluid obtained from their ventricular shunts. These cellular changes were interpreted as the cytologic counterpart of the foreign-body inflammatory reactions often seen histologically on occluded shunt tips. N

Risk of second cancer after lymphohematopoietic neoplasm.

People living with lymphohematopoietic neoplasms (LHNs) are known to have increased risks of second cancer; however, the incidence of second cancers after LHNs has not been studied extensively in Australia. The Australian Cancer Database was used to analyze site-specific risk of second primary cancer after LHNs in 127,707 patients diagnosed between 1983 and 2005. Standardized incidence ratios (SIRs) were calculated using population rates. Overall, patients with an LHN had nearly twice the risk of developing a second cancer compared to the Australian population. Among 40,321 patients with non-H

Coronal STIR sequence, a simple adjustment to routine MRI protocol for extra-spinal sciatica and sciatica-like symptoms

Abstract Background The value of adding coronal STIR images to MR imaging of sciatica aiming to detect extra-spinal abnormalities. Results Additional coronal STIR images detected extra-spinal abnormalities in 20% of the patients, thereby downgraded the normal studies from 21 to 13%. The extra-spinal abnormalities included bone abnormalities (36.4%), soft tissue abnormalities (4.5%), neurological abnormalities (2.3%), gynecological abnormalities (50%), and miscellaneous (6.8%). In 6.9% of patients, the extra-spinal abnormalities explained the patients’ pain and influenced their management. Extr

[Mental neuropathy and pathologic fracture of the mandible as initial symptoms of metastatic prostate carcinoma].

BACKGROUND: Distant metastases of solid tumors account for about 1% of all malign neoplasms. In about 30% of all patients who present with an oral metastasis, the distant primary tumour has not been diagnosed yet. CASE REPORT: We report the case of a 71-year-old man who clinically demonstrated unilateral mental neuropathy as well as pathologic fracture of the mandible. Prostate carcinoma was identified as a primary tumor. Clinically, there was significant hypesthesia of the skin area innervated by the left inferior alveolar nerve. X-ray examination revealed an osteolytic lesion of the ascendi

[Determining the degree of risk of postoperative complications in patients with a benign tumor of the parotid salivary gland].

THE AIM OF THE STUDY: Determination of the risk of postoperative complications in patients after surgical treatment of benign neoplasms of the parotid salivary glands. MATERIAL AND METHODS: The results of treatment of 228 patients with benign neoplasms of the parotid salivary gland in the period from 2019 to 2021 were analyzed. A questionnaire of 7 questions was formed the answers to which allowed patients to be assigned to a certain risk group: low, medium, high or extremely high. This gradation allows choosing the optimal method and scope of surgical treatment for each patient individually.
