Barriers and Constraints in Scientific Manuscript Preparation Among Nephrologists: Insights From India.

Introduction: Medical research shapes public health actions, emphasising the need for greater investments in health. Despite a surge in scientific publications, disparities exist in authorship from low-income countries and among female researchers. Addressing these gaps is vital for studying real-world health outcomes and promoting universal healthcare delivery. Methods: A descriptive quantitative study using an online questionnaire to gather data from Indian nephrologists and nephrology fellows was conducted by members of Women in Nephrology, India, from September 2023 to December 2023. The s…

Lysinuric Protein Intolerance: Not Only a Disorder for Pediatric Nephrologists - Case Report.

INTRODUCTION: Lysinuric protein intolerance (LPI) is a multisystemic inborn error of metabolism with a variable clinical expressivity that u … us to narrow down the diagnosis to LPI through biochemical and genetic investigations. CONCLUSION: Nephrologists should consider LPI in the differential diagnosis, whenever a patient presents with mixed proteinuria, tubular dysfunction, and/or chronic kidney disease of unknown origin. In these circumstances, we suggest looking for other signs such as growth fa …

Practice of Childhood Nephrotic Syndrome Management amongPediatric Nephrologists in the GCC Countries.

Childhood nephrotic syndrome (NS) management is greatly variable among pediatric nephrologists worldwide. We aimed to evaluate if this variability exists among pediatric nephrologists in the gulf countries and whether certain training programs influence this variability. A web-based multiple-choice survey of 35 NS management questions distributed to certified pediatric nephrologists working in the Gulf countries. Amongst 92 invitees, the response rate was 67%. The majority (73%) were older than 50 years and male (58%). Sixty percent trained in North America and 41% had >10 years of experience.…

[What should nephrologists know about cardiac illnesses in patients on peritoneal dialysis?].

Patients on peritoneal dialysis account for approximately 10% of individuals on dialysis and are very variably distributed throughout Spain, with a national mean of 47 procedures per million inhabitants. The high incidence of cardiovascular disease in patients on peritoneal dialysis is determined by the almost universal presence of traditional cardiovascular risk factors (diabetes, hypertension) and other risk factors secondary to uremia and the inflammatory process associated to atherosclerotic disease. The concomitant presence in clinical practice of highly diverse heart diseases in these pa

[Onconephrology - Interdisciplinary Cooperation of Nephrologists and Oncologists in the Treatment of Multiple Myeloma].

Plasma cell diseases, in particular multiple myeloma, represent an interdisciplinary challenge for oncologists and nephrologists. Patients often present initially with kidney problems to a nephrologist, requiring timely diagnosis and referral to an oncologist for treatment. On the other hand, a relevant part of patients will experience a - mostly temporary - decline in kidney function during the treatment course, which may require nephrological care. In any case, renal insufficiency can affect the therapeutic options. This article provides a short overview about common nephrological factors an…

Living kidney donor assessment: challenges, uncertainties and controversies among transplant nephrologists and surgeons.

The assessment of living kidney donors presents unique ethical challenges and complex psychosocial implications. This study aimed to ascertain the perspectives of transplant nephrologists and surgeons on living kidney donor assessment. Semi-structured, face-to-face interviews were conducted with 110 transplant nephrologists and surgeons from 43 transplant units in 12 countries from Europe, Australasia and North America. The challenge of defining acceptable risk to the donor was central to five themes identified: burden of responsibility (personal accountability, policing morality, democratic d…

[Questionnaire surveying nephrologists on drug dose adjustment in patients with impaired kidney function].

OBJECTIVE: Our intention was to assess knowledge and requirements related to drug dose adjustment in patients with impaired kidney function. METHOD: In 2005, we sent a questionnaire containing 22 questions to nephrologists in Germany and Austria. With 77 responses, the study was not representative. However, it was probably of importance for the target group of practising physicians and potential users of a future drug information system. RESULTS: Only 28% of the responding colleagues use the package inserts; these are obviously not considered to be an obligatory guideline for dose adjustment…

Performance of procedures by nephrologists and nephrology fellows at U.S. nephrology training programs.

BACKGROUND AND OBJECTIVES: Some procedures (e.g., placement of temporary hemodialysis catheters and kidney biopsies) are required in nephrology fellowship training. Others (e.g., placement of tunneled hemodialysis catheters, ultrasonography, and hemodialysis access interventions) are not required but are performed at some centers. To assess the procedures performed by nephrologists and nephrology fellows at U.S. adult nephrology training programs and the number of procedures required for fellow competency, a survey was conducted of all such training programs. Risk-based referral model to nephrologist specialist care in Stockholm.

BACKGROUND: For most patients, clinical management of the early stages of chronic kidney disease is performed in primary care settings. The Kidney Disease: Improving Global Outcomes (KDIGO) 2024 guidelines recommend using a 5-year kidney failure risk equation (KFRE) of 3-5% to guide nephrologist referrals. Here, we aimed to assess the impact of adopting a risk-based referral model compared with traditional referral criteria. METHODS: We conducted an observational retrospective study of adults with an estimated glomerular filtration rate (eGFR) <60 ml/min/1.73 m2 (Lund-Malmö equation) from the

Quantitative Lung Ultrasonography for the Nephrologist: Applications in Dialysis and Heart Failure.

Volume overload, and its attendant increase in acute care utilization and cardiovascular morbidity and mortality, represents a critical challenge for the practicing nephrologist. This is particularly true among patients with ESKD on HD, where predialysis volume overload and intradialytic and postdialytic hypovolemia account for almost a third of all cost for the Medicare dialysis benefit. Quantitative lung ultrasound is a tool for assessing the extent of extravascular lung water that outperforms physical exam and plain chest radiography. B-lines are vertical hyperechoic artifacts present in pa

What every nephrologist needs to know about hydroxychloroquine toxicity
.

Hydroxychloroquine (HCQ) has become the rheumatologists's "Swiss army knife" when it comes to managing the rheumatologic manifestations of SLE and other auto-immune disorders. By contrast, nephrologists are much less comfortable in managing the multifaceted effect of HCQ. As a result, nephrologists are inclined to abdicate their responsibility for HCQ management, if this therapy was initiated by a rheumatologist. This report describes such a situation, which had devastating consequences for the patient. On this basis we suggest that this report is a story worth telling, and should encourage th…
