Pathophysiology of trigeminal neuralgia.

Tic douloureux is a painful affliction of man without known similarities to diseases in infrahuman species or to other human afflictions. It seems to be associated with structural abnormalities encroaching upon the trigeminal nerve, gasserian ganglion, or root entry zone. The multiple unique features of tic douloureux can be explained by a theory which is based upon presynaptic inhibition and reflection sites due to focal changes in axon diameter or myelination. We believe that this theory satisfactorily explains the varied phenomena of tic douloureux and is compatible with the limited anatomi

Hereditary Motor and Sensory Neuropathies (HMSNs) With Conduction Block.

Dysmyelinating forms of hereditary motor and sensory neuropathy (HMSN) typically exhibit uniform conduction slowing on electrophysiological studies. However, this has been challenged in the past few decades with the discovery of novel gene mutations that induce nonuniform conduction slowing resembling acquired demyelinating neuropathies. Conduction block (CB) and/or excessive temporal dispersion (TD) are characteristic of segmental changes. Nevertheless, there is no consistent definition for CB and TD in inherited neuropathies. Pathological changes at paranodal regions or segmental demyelinati

Refining quality measures for electrodiagnostic testing in suspected carpal tunnel syndrome to account for acceptable variations in practice: Expert review process.

INTRODUCTION/AIMS: Using a set of process-of-care quality measures for electrodiagnostic testing in suspected carpal tunnel syndrome (CTS), the research team previously documented large variations in electrodiagnostic testing practices and adherence to quality measures. This study sought to enhance the applicability and validity of the quality measures by integrating acceptable variations in testing practices. METHODS: We recruited 13 expert electrodiagnostic medicine specialists from five specialty societies. The experts iteratively refined five quality measures, and then rated the validity

Neurophysiological findings in conus medullaris infarction-a case report and systematic review of the literature.

INTRODUCTION: Conus medullaris infarction (CMI) is a rare vascular phenomenon that has been scarcely reported in the literature. While previous studies have described the clinical and radiological features of CMI, little attention has been paid to its associated neurophysiological findings. METHODS: We present a case of idiopathic CMI and its neurophysiological findings, then present our findings from a systematic review of other reports of CMI with neurophysiological features found via PubMed search. RESULTS: Nine articles describing ten cases of CMI with associated neurophysiological data

Unclassified subtype of Guillain-Barré syndrome is associated with quick recovery.

Electrophysiological classification of Guillain-Barré syndrome (GBS) is important for predicting its clinical course; however, few reports discuss GBS patients who do not conform to the acute inflammatory demyelinating polyneuropathy (AIDP) or acute motor axonal neuropathy (AMAN) classifications. Therefore, the present study assessed the features of unclassified types of GBS and compared them to those of AIDP and AMAN. We compared clinical symptoms, nerve conduction, and laboratory data among patients with AIDP, AMAN, and unclassified subtypes of GBS, according to criteria developed by Rajabal

[Sarcoid Neuropathy].

Sarcoid neuropathy has a broader spectrum of clinical features than previously expected. It is typically characterized by multiple mononeuropathy but often shows a polyneuropathy pattern, making it difficult to be differentiated from other neuropathies. In the diagnostic process, several clinical features, including laterality and proximal-predominance of symptoms at extremities, sensory deficits in the territory of the branch of the peripheral nerve trunk, and positive neuropathic sensory symptoms typically accompanied by pain, could be the cues to evaluate the probability of sarcoid neuropat

The electrodiagnosis of Guillain-Barré syndrome subtypes: Where do we stand?

It is controversial as to whether the electrophysiological Guillain-Barré syndrome (GBS) subtypes can be diagnosed on the basis of a single study and which criteria sets and cut-offs should be used. Serial electrophysiologic studies have shown that a significant number of patients changed electrodiagnostic subtype largely because of the recognition of reversible conduction failure as a possible evidence of axonal pathology. However, other reports concluded that electrodiagnosis can be made by a single study, the subtypes depending on the characteristic of the criteria set applied. Such diverge

Safety and pain in electrodiagnostic studies.

Discomfort is an unavoidable part of electrodiagnostic (EDX) studies. The most readily modifiable mediator of electromyography (EMG)-associated pain is muscle selection. Interventions that may reduce pain include vapocoolant spray, ibuprofen, and techniques such as slapping or stretching the skin. Needlestick injuries to health care workers carry the risk of transmitting bloodborne illnesses, but other infectious complications of EDX studies are very rare. EMG probably contributes to asymptomatic hemorrhage in approximately 1% of patients, but clinically significant bleeding has only been repo

Auditory central pathways in children and adolescents with multiple sclerosis.

BACKGROUND: Multiple sclerosis (MS) is an inflammatory demyelinating disease. Auditory evoked potential studies have demonstrated conduction … components. CONCLUSION: Children and adolescents with MS had abnormal BAEP responses, with delayed neural conduction between the cochlear nucleus and the lateral lemniscus. Also, abnormal LLAEP results suggest a decrease in neural processing speed and auditory sensory discrimination response.

Convolutional Neural Network-Based ECG-Assisted Diagnosis for Coal Workers.

OBJECTIVE: To process and extract electrocardiogram (ECG, ECG, or EKG) features using a convolutional neural network (CNN) to establish an ECG-assisted diagnosis model. METHODS: Coal workers who underwent physical examinations at Gequan Mine Hospital and Dongpang Mine Hospital of Hebei Jizhong Energy from July 2020 to September 2020 were selected as the study subjects. The ECG images were preprocessed. We use Python software and convolutional neural network to establish ECG images recognition and classification model. We usecalibration curve, calibration-in-the-large, Brier score, specificity,

Differential fiber-specific block of nerve conduction in mammalian peripheral nerves using kilohertz electrical stimulation.

Kilohertz electrical stimulation (KES) has been shown to induce repeatable and reversible nerve conduction block in animal models. In this study, we characterized the ability of KES stimuli to selectively block specific components of stimulated nerve activity using in vivo preparations of the rat sciatic and vagus nerves. KES stimuli in the frequency range of 5-70 kHz and amplitudes of 0.1-3.0 mA were applied. Compound action potentials were evoked using either electrical or sensory stimulation, and block of components was assessed through direct nerve recordings and muscle force measurements.
