[Neurology of eye movements].

Eye movements are necessary for clear and stable vision, for which images of the world should be brought to the fovea and be held steady on the retina. The main types of eye movements consist of saccadic eye movements, vergence, vestibular eye movements, smooth pursuit eye movements and eye movements for visual stabilization. Saccades are fast eye movements and have consistent relationship between their peak velocity and the size of the movements. The burst neurons in the brainstem are the generator of saccades and receive projections from the frontal eye field, supplementary eye field, pariet

[Child neurology and rehabilitation].

The history of child neurology and the changing pattern of research methods in this field are reviewed with special reference to holoprosencephaly and recent technical advances in sleep research. This is followed by a discussion on the relationship between child neurology and rehabilitation. The majority of child neurologic disorders are developmental disabilities, but acquired child neurological diseases also show chronic progressive course in many cases. Therefore, child neurologist should understand the basis of rehabilitation approach and appreciate the three classes of disabilities; subse…

Renal failure is relatively common, but except in association with spina bifida or paraplegia it is unlikely to occur as a result of disease … tides, the paraproteinaemias, and various granulomatous conditions (considered in other chapters of Neurology and Medicine). In two of the most commonly encountered genetically determined diseases, Von Hippel-Lindau disease and polycystic kidney disease, location of pathogenic mutations will provide improved screening programmes and, possibly, allow therape …

[The neurology of dyslexia].

Developmental dyslexia is a heterogeneous disorder in which the prominent manifestation is a discrepancy between reading achievement and intelligence. There is no serious auditory, visual, psychiatric, social or educational factor that could be responsible of the discrepancy. Males are more often affected than females. It is a pervasive condition but with adequate help and spontaneous compensation, reading ability may improve. Neuroimaging, mainly MRI, allows to demonstrate in two thirds, an absence of the usual symmetry of the planum temporale favoring the left side. Twenty to 25% of the rema

[Intravenous immunoglobulins in neurology].

Immunoglobulins, used at first empirically in the treatment of thrombocytopenic purpura, occupy a prominent place not only in the treatment of antibody deficiencies, but also in that of antoimmune diseases. Their indications in neurology are ever extending; they include myasthenia, chronic inflammatory polyneuropathies with or without monoclonal gammopathy, polymyositis, dermatomyositis and, more recently, disseminated sclerosis and Guillain-Barré syndrome. Even the therapeutic priority of plasmapheresis in this syndrome is disputed by some authors. Immunoglobulins are costly, but they are wel…

[Leg pain in neurology].

A frequent presenting symptom encountered by both neurologists and phlebologists, pain in the lower limbs often results in clinical practice in the suggestion of a neurological disorder. Only pain relating to neurological conditions will be dealt with here. A review of the anatomical pathways of different types of sensation is followed by a semiological review of descriptions of pain and the underlying mechanisms. Mention is then made "from below upwards" of a number of common conditions in which pain is a frequent presenting symptom: firts peripheral: polyneuropathy (alcoholic and diabetic),

Neurology of latent nystagmus.

We report eye movement findings in 30 patients with latent nystagmus and who were found to have a variety of associated oculomotor disorders. Latent nystagmus is defined clinically as nystagmus which appears on covering one eye and beats towards the uncovered eye. Recordings showed that the latent nystagmus in 28 patients had slow phases with linear or exponentially decreasing velocity. This nystagmus is termed 'LN'. In 13 of these patients certain manoeuvres (e.g. pursuit) provoked nystagmus with exponentially increasing slow phase velocities characteristic of the congenital form of nystagmus

Peroxisomal disorders in neurology.

Although peroxisomes were initially believed to play only a minor role in mammalian metabolism, it is now clear that they catalyse essential reactions in a number of different metabolic pathways and thus play an indispensable role in intermediary metabolism. The metabolic pathways in which peroxisomes are involved include the biosynthesis of ether phospholipids and bile acids, the oxidation of very long chain fatty acids, prostaglandins and unsaturated long chain fatty acids and the catabolism of phytanate and (in man) pipecolate and glyoxylate. The importance of peroxisomes in cellular metabo

[Problems of perinatal neurology].

An analysis of more than 1000 cases of early organic brain lesions has demonstrated that in 54% of them the etiology of neurological disturbances was an antenatal pathology, in 38%-intranatal and in 8%-postnatal. The prevention of perinatal disorders of the nervous system is considered as a complex problem including measures of health prevention for the future mothers, control over the development of pregnancy, perfection of obstetric aid, effective measures for the prevention of tissue hypoxia and its sequalae, elaboration of methods of rehabilitative therapy, work with personnel.

Perceptual problems in neurology.

Physiotherapy has traditionally been concerned with muscle function. In dealing with patients with neurological disorders it may not be enough to have a thorough grasp of muscular innervation and musculoskeletal problems. We are all aware of the importance of the higher centres as manifested by motivation, courage, intelligence, the capacity to relax and so forth. This is the same for all patients. However, the perceptual problems which are unique to some neurological conditions may undermine the best efforts of a physiotherapist not familiar with them. The patient is the last person likely to

[Hiroshi Kawahara: A Founder of Japanese Neurology: The Origin of Neurology in Nagoya].

Hiroshi Kawahara (1858-1918) graduated from Tokyo Medical School, the predecessor of the present Faculty of Medicine at the University of Tokyo, where he was strongly influenced by Erwin von Baelz. Soon after, Kawahara was appointed to a professorship of internal medicine at Aichi Medical School (now Nagoya University, School of Medicine). One of Kawahara's greatest contributions to Japanese neurology was the first publication of a textbook of clinical neurology written in Japanese. Published in 1897, it contains 469 pages of text with many original figures observed by Kawahara himself, coveri…

Emerging Subspecialties in Neurology: Sports neurology training and certification: An overview in 2018.

Sports neurology is a relatively new specialty in neurology. Interested trainees may find it difficult to learn about issues in sports neurology both in medical school and residency. This review highlights what the current state is in medical school and residency training and how an interested trainee or neurologist could pursue a career in this area.
