During the last 5 years six cases of mycetoma have been referred to the Razi Hospital, Thehran. Nocardia Asteroides was found to be the causative agent in one case only. Five of the patients were males, all being rice farmers, and one was a female. All of them belonged to Northern Iran which has a mediterranean climate. Treatment of the condition still remains unsatisfactory and ineffective and amputation may have to be performed as a last resort.

Sera from 23 patients with nocardiosis were tested for the presence of precipitating antibodies against an extract of Nocardia asteroides. Sera from nine of 20 patients infected with Nocardia asteroides and one of three sera from patients with Nocardia brasiliensis produced one to three precipitin lines with the antigen. Sera from eight of 55 patients with tuberculosis and 11 of 13 patients with leprosy also contained Nocardia precipitins. None of 33 sera from patients with cryptococcosis, blastomycosis, actinomycosis, histoplasmosis, coccidioidomycosis, mucormycosis or candidiasis contained N…

Four new phenolic siderophores were isolated from the actinomycete Nocardia altamirensis along with the known natural product amamistatin B and a putative biosynthetic shunt product. The structures of all compounds were elucidated through 1D and 2D NMR analyses as well as mass spectrometry. The iron-chelating properties of the retrieved metabolites were evaluated in a chrome azurol S assay.

Nocardia brasiliensis is a gram-positive, branched, aerobic, acid-resistant, rod-shaped bacillus that inhabits in soil, rotten organic matter and waters. Cutaneous nocardiosis in immunocompetent individuals can manifest in three different forms: actinomycetoma, superficial skin infection or lymphocutaneous infection. A case of an 85-year-old woman with an ulcerated lesion on the back of her left hand with erythematous papules in the lymphatic tract infected by N. brasiliensis is described. The microorganism was presumptively identified by conventional and inexpensive methods for a medium compl…

Nocardial brain abscess is a rare central nervous system infection with high morbidity and mortality. Most of the human infections, i.e., about 90%, are due to Nocardia asteroides group comprising N. asteroides complex, Nocardia farcinica, and Nocardia nova. Other species rarely cause human infections. Here, we report a case of left parieto-occipital abscess caused by a rare species, Nocardia araoensis, its diagnosis, treatment options, and review of literature. A 73-year-old male, known case of diabetes mellitus, on prolonged oral corticosteroid for autoimmune hemolytic anemia presented with …

Nocardia spp. are aerobic, Gram-positive, catalase positive, and non-motile actinomycetes. They are associated with human infections. However, some species produce important natural products, degrade toxic chemicals, and are involved in biotransformation of valuable products. The lack of robust genetic tools has hindered detailed studies and advanced research. This unit describes the major genetic engineering approaches using Nocardia sp. CS682 as a prototype. These methods will certainly help in understanding the basis of their pathogenicity as well as biosynthetic and biotransforming abiliti…

Nocardiosis is an uncommon bacterial gram-positive infection caused by aerobic actinomycetes in the genus Nocardia. Nocardiosis is typically regarded as an opportunistic infection with approximately two-thirds of infected patients being immunocompromised. In this case report we describe a 45-year-old female who presented with a right thigh abscess. She had been taking high-dose prednisolone and ciclosporin for Cogan's syndrome. She presented with erythema and severe pain over her right thigh. Ultrasound showed a collection and Nocardia farcinica was isolated. This case report stresses the impo…

INTRODUCTION: Nocardiosis is an infectious actinomycetic disease with a variable clinical spectrum that makes it difficult to diagnose. It mainly affects immunosuppressed individuals. Advances in molecular genomic technology have helped in identifying new pathogenic Nocardia species. This has made identification of their specific antimicrobial sensitivity possible.
AREAS COVERED: It is important to know the taxonomy, clinical features, diagnosis and precise species identification because of the multitude of pathogenic species involved and the different antibiotic susceptibility patterns. The …

PURPOSE: To report an immunocompetent patient with Nocardia exalbida endogenous endophthalmitis.
DESIGN: Case report.
METHODS: Clinical-pathologic correlation and microbiologic evaluation were performed on an enucleated eye.
RESULTS: A 56-year-old man presented with rapidly progressive vision loss associated with a posterior choroidal mass and serous retinal detachment. Pathologic evaluation of the enucleated eye demonstrated endogenous endophthalmitis. Nocardia exalbida was identified microbiologically. Systemic workup failed to demonstrate definite foci of systemic infection or evidence o…

PURPOSE: To describe a case of Nocardia keratitis resistant to 2% amikacin, with a toxic-allergic reaction to fortified topical 5% amikacin, and recurrence of the infection with topical corticosteroids.
METHODS: Nocardia was diagnosed from a smear and positive culture and identified as Nocardia asteroides by gas chromatography and quantitative fatty acid analysis using the Microbial Identification System. Treatment was started with topical 2% amikacin, which was subsequently raised to 5% because of clinical resistance.
RESULTS: A toxic-allergic reaction was observed after 5% amikacin so the …

PURPOSE: To assess the role of confocal microscopy for in vivo detection of Nocardia in patients with microbial keratitis.
DESIGN: Retrospective interventional case series.
PARTICIPANTS: Three patients with microbiologically proven Nocardia keratitis.
INTERVENTION: Confocal microscopy.
METHODS: We performed confocal microscopy in microbiologically proven cases of Nocardia asteroides keratitis. A masked observer examined the images. For better understanding of the images, we also performed confocal microscopy on a blood agar culture plate with Nocardia growth.
RESULTS: Confocal microscopy …

PURPOSE OF REVIEW: Actinomycosis and nocardiosis are uncommon pulmonary infections with distinct morphologic features. Both infections most commonly present as chronic, debilitating illnesses with radiographic manifestations simulating lung cancer or tuberculosis. Immunocompromised hosts, however, may develop fulminant disease resembling acute bacterial pneumonia. The purpose of this review is primarily to review the clinical features, diagnosis, and management of actinomycosis and nocardiosis.
RECENT FINDINGS: Treatment of actinomycosis is usually simple, requiring long-term, high-dose intra

This is the first clinical report of a case of pneumonia caused by Nocardia nova in Japan. A 52 year-old woman who had received steroids and cyclophosphamide for six years because of polymyositis was admitted to our hospital for further examination. On admission she had a mild cough, and her chest radiography and computed tomography revealed bilateral multiple nodules, some of which were cavitated. She developed a cough productive of yellow sputum and fever up to 38 degrees C. Examination of the sputum revealed a gram-positive branched organism and sputum cultures repeatedly grew Nocardia spec…

Nocardia transvalensis can be a rare cause of actinomycotic mycetoma. A 40 year old man presented with his right arm showing multiple discharging sinuses, which had been present for four years. N transvalensis was isolated from a biopsy specimen. No other underlying disease was detected. The patient was successfully managed with surgical excision of the lesion and treatment with co-trimoxazole. This is the first case of N transvalensis infection to be reported from the Indian subcontinent as far as is known.
