[Nutritional support for individuals with advanced oncological pathology].

Cachexia increases the risk of therapeutic failure and reduces the quality of life for cancer patients. Patients with nutritional risks should be referred to a dietitian. Nutritional interventions primarily include meal adaptation and oral nutritional supplements. When cachexia is more severe, artificial nutrition is indicated for patients with a KPS (Karnofsky Performance Status) of 40 or above and a life expectancy of more than 6 weeks. When the digestive system is functional, enteral nutrition is recommended. Parenteral nutrition shows the same benefits but requires close monitoring. The de

Nutritional support in chronic liver disease and cirrhotics.

The liver is a major organ and an essential component in maintaining an appropriate nutritional status in healthy individuals through metabolism of protein, carbohydrates, and fat. In individuals with chronic liver disease (CLD), along with a number of other essential functions that the liver serves, its role in nutrition maintenance is severely impaired. Common causes of CLD include hepatitis C, alcoholic liver disease, and non-alcoholic liver disease. Amongst this population, the most common manifestation of impaired nutritional maintenance is protein-calorie malnutrition. Aside from inheren

Providing nutritional support for patients with cancer cachexia.

Malnutrition is the single most common secondary diagnosis for patients with advanced cancer, and can affect up to 85% of patients. Anorexia and weight loss affect up to 80% of patients and are responsible for death in up to 20% of cases. Cancer cachexia is a feature of advanced cancer characterised by anorexia and a progressive loss of body weight, reduced immune response, poor treatment response, and poor quality of life. Providing effective nutritional support for patients who experience cancer cachexia at the end of life is very challenging and requires a multidisciplinary team approach to…

Early nutritional support and outcomes in ELBW infants.

Extrauterine growth restriction is a common finding among extremely low birth weight infants at discharge from neonatal intensive care units. It has been associated with significant caloric and protein deficits that accumulate during the hospitalization and with slower growth velocity observed with major neonatal morbidities. Recommendations to provide early nutritional support, both parenteral and enteral, have evolved in response to concern about the impact of poor in-hospital growth on growth and neurodevelopmental outcomes in early childhood. This paper will review the evidence supporting …

Nutritional support in children undergoing bone marrow transplantation.

Nutritional status and 'well-being' were compared prospectively in 39 children (mean age 8.1 years) who received nutritional support following bone marrow transplantion (BMT): 20 received enteral tube feeding (ETF; six received parenteral nutrition [PN] subsequently) and 19 with oral mucositis received PN (one received ETF subsequently). Poor nutritional status (height for age and/or weight for height and/or mid-arm circumference z-scores <-1) was present in 18 patients and was associated with a longer hospital stay (P = 0. 01). Both ETF and PN groups were comparable with respect to age, pretr…

Nutritional support in patients with inflammatory bowel disease.

Total parenteral nutrition with bowel rest has been used as primary therapy to reduce disease activity and achieve remission in patients with inflammatory bowel disease (IBD). However, results are short-lived and similar success can be attained through total enteral nutrition with highly specialized elemental or semielemental formulas. Enteral nutrition costs less than parenteral nutrition, maintains gut integrity, stimulates immunocompetence, and helps to control symptoms and overall disease activity. Increased use of enteral formulas can be expected in the future. The role of diet in managem

Nutritional support of pulmonary patients: a multidisciplinary approach.

Pulmonary disease affects and is affected by the nutritional status of the patient. The relationships between nutrition, medications, complications, and the course of pulmonary disease itself are multifaceted and are the focus of concern for the nutritional support team. Nutritional support of patients with pulmonary disease demands the expertise of a multidisciplinary team in monitoring the nutritional status of patients, appropriately selecting feeding solutions and routines, selecting and administering medications, and planning and implementing nursing interventions. Recognition of the impo…

Nutritional support of the bone marrow transplant patient.

BMT results in multiple toxicities and organ dysfunction, which significantly influence the nutritional status of patients. The preparative regimen, which includes high-dose chemotherapy (with or without radiotherapy), causes nausea, vomiting, diarrhea, mucositis, anorexia, dysgeusia, and xerostomia. Conditioning may also contribute to infection and organ dysfunction. TPN is used as the principal method of nutritional support during the first month after transplantation. Oral feedings are primarily employed thereafter. A team approach is required to manage the nutritional aspects of care effec…

[Nutritional support for preoperative patients with esophageal cancer].

Nutritional support for patients undergoing major operations seems to be very important for improving the operative results, especially in preoperative patients with conditions such as esophageal carcinoma. In our department, patients who have lost more than 10% of their normal weight have been given intravenous hyperalimentation for at least two weeks preoperatively to prevent postoperative pulmonary complications. However good responses to nutritional support were not always obtained in these patients. One of the reasons was thought to be possible inadequacy of the nutritional support, for e…

Postoperative nutritional support in oral and maxillofacial surgery.

The patient sustaining maxillofacial trauma, disease or deformity presents with unique nutritional problems, especially during the postoperative period. By providing a nutritionally adequate diet in the preoperative and immediate postoperative period and during convalescence, complications can be reduced and healing improved. This paper details the nutritional requirements for patients and the indications for, and complications from, total parenteral nutrition and enteral nutritional support.

Nutritional and metabolic support following acute hepatic trauma.

A patient undergoing partial hepatectomy subsequent to trauma was maintained post-operatively on a specifically tailored total parenteral nutrition regimen. This, despite obviously impaired liver function, allowed adequate nutrition without the expected adverse metabolic sequelae associated with hepatic resection and nitrogen loading. The patient also demonstrated, in the absence of normal hepatic tissue, an ameliorated metabolic response to trauma.

Value of nutritional support in adult cancer patients.

Both the presence of cancer and oncologic therapy cause metabolic alterations that may decrease the ability of the host to maintain anabolism. Nutritional support, properly administered, will replenish lean body mass, visceral proteins, and immunocompetence in human beings and experimental animals with small to modest tumor burdens. To date, stimulation of tumor growth by intravenous hyperalimentation in malnourished patients has not been documented scientifically. The exact role of intravenous hyperalimentation, in combination with chemotherapy or radiation therapy, is controversial, whereas …

Nutritional support as an adjunct to radiation therapy.

Patients with malignancies which are treated with therapeutic radiation are at risk for nutritional problems, both from their underlying malignancy as well as from their treatment. These effects may be acute or chronic and relate to the site of the tumor and regions irradiated. There is a large experience with nutritional intervention in irradiated patients, including oral feedings and enteral and parenteral nutritional support. The indications for the specific administration of nutritional support during radiotherapy depend on the nutritional status of the patient and the area irradiated, as …

Nutritional support of malnourished lactose intolerant African patients.

The effectiveness of two commonly available liquid diets was assessed in 40 severely malnourished black African patients. All patients were shown to have normal xylose absorption. The diets were given according to the manufacturer's recommendations. One diet was lactose containing (LC diet) (150 g/d) and high protein (112 g/d), the other normal protein and lactose free (LF diet) (protein 67 g/d), total energy content being similar. Patients were randomly divided into two equal groups and allocated (blind) to one of the diets. Tolerance and nitrogen balance were assessed over two three day peri
