BACKGROUND: Anomalous head posture (AHP) measurement is crucial in the management of nystagmus and incomitant strabismus and also during follow-up assessments. The types of AHP include chin posture, head tilt, and face turn. Goniometer is a low-cost tool widely used to measure head posture. This has two arms: one reference arm and one measuring arm. The drawback of this device is that the examiner has to maintain the reference arm exactly parallel or perpendicular to the floor and the other arm has to align the AHP. Since not all the examiners are comfortable in aligning both the arms simultan

During the last decades a new vestibular syndrome has emerged that is now termed vestibular migraine (VM). The main body of evidence for VM is provided by epidemiologic data demonstrating a strong association between migraine and vestibular symptoms. Today, VM is recognized as one of the most common causes of episodic vertigo. The clinical presentation of VM is heterogeneous in terms of vestibular symptoms, duration of episodes, and association with migrainous accompaniments. Similar to migraine, there is no clinical or laboratory confirmation for VM and the diagnosis relies on the history and

Positional vertigo and nystagmus without associated neurological symptoms and signs are characteristic features of benign paroxysmal positional vertigo (BPPV). Although positional nystagmus may occur with caudal cerebellar infarction including the nodulus, positional nystagmus is usually associated with other neurological signs such as spontaneous or gaze-evoked nystagmus, perverted head-shaking nystagmus, cerebellar dysmetria, or severe gait ataxia with falling. We present a patient with nodular infarction who had positional vertigo with nystagmus as a sole manifestation. Video-oculography sh

Three distinct pathological conditions, related to different means by which dense intralabyrinthine particles interfere with the function of a semicircular canal and cause nystagmus and vertigo, are amenable to treatment with repositioning maneuvers. Known as benign paroxysmal positional vertigo and variants, these conditions are better designated collectively by the term "vestibular lithiasis." Each form requires a different treatment strategy of head maneuvers and application of other modalities to restore normal semicircular function and thereby eliminate the positional nystagmus and vertig

Main features of the benign paroxysmal positional vertigo (B.P.P.V.) are: latency before the onset of vertigo, nystagmus of the rotatory type beating toward the lower ear, nystagmus tires out, nystagmus gets inverted when the head is brought back to orthostatism, nystagmus is reproducible as many times as the position is taken. B.P.P.V. is due to a deposit of heavy material on the cupula of the posterior semi circular canal. In this unusual condition, the cupula moves under the effect of gravity acceleration. The goal of the treatment is to free the cupula. The manoeuvre consists after determi

The purpose of this paper is to demonstrate the variation of the direction of nystagmus which is elicited from the peripheral labyrinth. The direction of nystagmus depends, among other things, on the nature of the stimulus. Depending on the degree of pathophysiological conditions in the labyrinth, atypical nystagmus which does not comply with the rule is occasionally elicited even though the same stimulus is given. Hyperfunctional stimulation to the labyrinth, such as allergy, causes nystagmus which beats to the affected side and also shows hyperactivity of the whole nerve action potential and
