Onchocerciasis (river blindness).

A 37-year-old African man presented for excision of a dermal nodule after a diagnosis of ocular onchocerciasis (river blindness). A nodule from the patient's left buttock contained several adult filarial worms, and results from adjacent skin biopsy specimens revealed numerous dermal microfilariae. The patient was admitted to the hospital and treated with one dose of ivermectin. Recommendations were made for ivermectin treatments every 6 months for up to 10 years. The history, clinical presentation, diagnosis, and treatment of onchocerciasis are discussed.

Onchocerciasis-Associated Epilepsy, an Additional Reason for Strengthening Onchocerciasis Elimination Programs.

A high prevalence of epilepsy has been observed in onchocerciasis-endemic regions with high onchocerciasis transmission. Recent epidemiological studies suggest that Onchocerca volvulus infection is the trigger causing the seizures, which appear in previously healthy children between the ages of 3 and 18 years. Persons with onchocerciasis-associated epilepsy present with a wide spectrum of seizures, including atonic and myoclonic neck seizures; but also absences and most frequently generalized tonic-clonic seizures. Often individuals present with intellectual disabilities and psychiatric disord…

Epidemiology and control of onchocerciasis: the threshold biting rate of savannah onchocerciasis in Africa.

Control of onchocerciasis currently focuses on community-directed treatment with the microfilaricide ivermectin which effectively kills Onchocerca volvulus microfilariae in the human host. The feasibility of elimination by this control strategy has recently been reported for some foci in Africa which has rekindled discussions on evaluating the threshold conditions of elimination of onchocerciasis. We developed a stochastic model based on a master equation which predicts, based on data from West and Central Africa, that elimination of savannah onchocerciasis can be expected around a threshold b…

Navigating the way to onchocerciasis elimination: the feasibility and affordability of Onchocerciasis Elimination Mapping.

BACKGROUND: Onchocerciasis is targeted for elimination of transmission by 2030 in at least 21 countries. To achieve this, recent and accurate data on the extent and intensity of onchocerciasis transmission are required. This will include mapping areas previously unassessed, or remapping of areas that were last visited as part surveys aiming to prevent blindness, not assess transmission in totality. There is near universal acceptance of the need to carry out these mapping reassessments, to achieve equitable and lasting elimination of onchocerciasis transmission. However, there is no consensus o…

[Ivermectin and human onchocerciasis. A study of 234 onchocerciasis patients in the Republic of Mali].

In a double blind study Ivermectin has been compared to a placebo in 234 male and female with onchocerciasis who had more than 20 microfilariae per milligram of skin and moderate ocular involvement. Patient was randomized to receive a simple oral dose of Ivermectin 100, 150 and 200 micrograms/kg or placebo. The following was 12 months. The decrease of microfilarodermia since to the 3rd day was from 72.8 to 79.3% of initial rate. At six months it was more than 91% and more than 87% in 12 months. Ocular microfilariae, initially between 12 and 23 stay lower than 2 at 12 months. Punctuated keratit…

Lymphatic filariasis and onchocerciasis.

Lymphatic filariasis and onchocerciasis are parasitic helminth diseases that constitute a serious public health issue in tropical regions. The filarial nematodes that cause these diseases are transmitted by blood-feeding insects and produce chronic and long-term infection through suppression of host immunity. Disease pathogenesis is linked to host inflammation invoked by the death of the parasite, causing hydrocoele, lymphoedema, and elephantiasis in lymphatic filariasis, and skin disease and blindness in onchocerciasis. Most filarial species that infect people co-exist in mutualistic symbiosi…

Towards the elimination of onchocerciasis.

Human onchocerciasis is a severely disabling filarial disease that is endemic in 28 African countries, six Latin American countries and Yemen. The disease causes a high burden of blindness and visual loss, along with itching and other severe dermal manifestations. It constitutes a significant obstacle to socio-economic development in highly endemic riverine areas, where the Simulium blackflies that act as vectors breed. Onchocerciasis has been subject to control efforts for more than 50 years, initially mainly through vector control but since 1988, with free access to ivermectin (Mectizan), al…
