De novo malignant optic chiasm glioma with initial clinical response to steroids.

Malignant optic nerve glioma (MONG) is a rare but uniformly fatal disease that remains poorly understood. We describe a notable case of this rare disease occurring in the optic chiasm. Normal brain imaging and normal ophthalmic examination two years prior to diagnosis provide evidence for de novo genesis of MONG in our patient. Early response to steroids highlights the degree to which MONG can initially mimic inflammatory optic neuropathies and chiasmal syndromes. Our case also demonstrates a poor outcome with MONG even with current advanced therapy for glioblastoma including radiotherapy plus…

Isolated absence of optic chiasm revealed by congenital nystagmus, MRI and VEPs.

Congenital nystagmus is a rare condition mainly characterised by rhythmic, conjugate, and horizontal oscillations of both eyes that persist in the vertical gaze. This disorder is usually noticed in the neonatal period and persists throughout life. It can be of sensory origin, associated with low visual acuity of various causes, or of motor origin, caused by a defect in the slow eye movement system. The former can be genetically determined. It can also be associated with several conditions, the most frequent being albinism. The achiasma syndrome has recently been recognized in two patients as a

Langerhans cell histiocytosis of hypothalamus and optic chiasm: CT and MR studies.

The clinical, CT, and magnetic resonance features of two cases of hypothalamic/optic chiasm involvement with Langerhans cell histiocytosis are presented. In both cases, the mass was iso- to hyperdense on noncontrast CT with uniform postcontrast enhancement, whereas with magnetic resonance (one case) the mass remained isointense to brain on both long and short repetition time sequences. The pathophysiology of hypothalamic pituitary dysfunction is briefly reviewed.

Monensin induces distortion of optic nerve crossing at the chick embryo chiasm.

The effects of intraocular injection of monensin, a specific inhibitor of membrane glycoprotein transport within the axon, on the development of the chick embryo optic nerve were examined. A proportion of axons growing from the monensin-injected eye were misrouted into the opposite optic nerve at the chiasm and grew to the retina of the opposite eye. These results suggest that formation of the decussation pattern at the chiasm primarily depends on neurite fasciculation mediated by membrane glycoproteins.

Malignant glioma of the optic chiasm eight years after radiotherapy for prolactinoma.

A 41-year-old man had rapidly progressive visual loss caused by a malignant glioma that developed in the optic chiasm eight years after radiation therapy for a recurrent prolactinoma. Radiation-induced glioma should be considered as a cause of progressive visual loss in patients who have received irradiation in the region of the sella turcica.

Olfactory Groove Meningioma Compressing the Optic Chiasm and Optic Nerve: Improvement of Visual Functions by Surgical Intervention.

OBJECTIVE: Olfactory groove meningiomas (OGMs) are rare but serious tumours that can cause visual loss. Surgical treatment of such meningiomas is critical for the recovery of visual functions. METHODS: In this case report, a 62-year-old patient presented to our clinic with sudden visual loss in the right eye and underwent ophthalmological examination. Visual field (VF), retinal nerve fibre layer (RNFL) thickness test, and contrast-enhanced cranial and orbital magnetic resonance imaging (MRI) were performed. The patient was diagnosed with OGM compressing the optic chiasm and nerve, and was ope…

Congenital aplasia of the optic chiasm and esophageal atresia: a case report.

INTRODUCTION: The complete absence of the chiasm (chiasmal aplasia) is a rare clinical condition. Hypoplasia of the optic nerve and congenit … rophthalmos and associated with esophageal atresia is described. CASE PRESENTATION: Aplasia of the optic chiasm was identified in a Caucasian Italian 8-month-old boy with esophageal atresia. An ultrasound examination carried out at 21 weeks' gestation revealed polyhydramnios. Intrauterine growth retardation, esophageal atresia and a small atrial-septal defect …

Wilbrand's knee of the primate optic chiasm is an artefact of monocular enucleation.

PURPOSE: The anterior chiasmal syndrome consists of a temporal hemianopia or complete visual field loss in one eye, plus a superior temporal hemianopia in the other eye. The superior temporal hemianopia in the other eye is thought to result from injury to Wilbrand's Knee of the optic chiasm. Wilbrand's Knee is a loop of decussating fibers which detours into the contralateral optic nerve before entering the optic tract. I studied the organization of fibers in the optic chiasm of monkeys and humans to verify the existence of Wilbrand's Knee and to elucidate further the pattern of visual field lo…

Management of glioma of the optic nerve and/or chiasm: an analysis of 20 cases.

The records of 20 patients with biopsy-proven glioma of the optic nerve and chiasm are reviewed. Symptoms and physical findings on admission are reported. Patients were classified, according to the operative findings, into disease limited to the optic nerve and a more extensive disease group. Surgical procedures, operative complications, irradiation technique, and outcome are analyzed. Patients with disease limited to the optic nerve may be adequately managed by resection alone. In the more extensive lesions, high morbidity is associated with resection; therefore, biopsy and irradiation offer

Atrophy of optic nerve fibres in compression of the chiasm. Prognostic implications.

A relationship between the degree of atrophy of retinal nerve fibres and visual field defect has been described in postoperative steady-state patients (Lundström & Frisén 1976). The same factors were studied prior to surgery in six patients with compression of the chiasm due to chromophobe adenoma. In five eyes the field defect was excessive in relation to the degree of atrophy. After surgery the visual field defects improved to a level corresponding to the degree of atrophy in these eyes. In the remaining seven eyes there was a close correspondence between the atrophy and the visual field def

Decussating axons segregate within the anterior core of the primate optic chiasm.

BACKGROUND: The axons of ganglion cells in the nasal retina decussate at the optic chiasm. It is unclear why tumours cause more injury to crossing nasal fibres, thereby giving rise to temporal visual field loss in each eye. To address this issue, the course of fibres through the optic chiasm was examined following injection of a different fluorescent tracer into each eye of a monkey. METHODS: Under general anaesthesia, cholera toxin subunit B-Alexa Fluor 488 was injected into the right eye and cholera toxin subunit B-Alexa Fluor 594 was injected into the left eye of a single normal adult male…

Chiasmal apoplexy: hemorrhage from a cryptic vascular malformation in the optic chiasm.

We describe three patients with acute chiasmal visual field loss caused by hemorrhage within an intrachiasmal arteriovenous malformation. One patient had five episodes; vision improved three times without treatment and twice after surgical decompression. Surgical intervention restored vision in two of the three patients. The role for surgery is indefinite, but may be indicated if the visual defect remains static for 1 week or progresses. Decompression is likely to be most effective if done early. The approach to the optic chiasm should be transcranial, not transsphenoidal. A radiologically nor…

Optic chiasm measurements may be useful markers of anterior optic pathway degeneration in neuromyelitis optica spectrum disorders.

OBJECTIVES: We aimed to evaluate optic chiasm (OC) measures as potential imaging marker for anterior optic pathway damage assessment in the context of neuromyelitis optica spectrum disorders (NMOSD). MATERIALS AND METHOD: This cross-sectional study included 39 patients exclusively with aquaporin 4-IgG seropositive NMOSD of which 25 patients had a history of optic neuritis (NMOSD-ON) and 37 age- and sex-matched healthy controls (HC). OC heights, width, and area were measured using standard 3D T1-weighted MRI. Sensitivity of these measures to detect neurodegeneration in the anterior optic pathw…
