Orthodontics and dentistry for the hemophilic patient.

In this presentation we have contrasted the normal blood-clotting mechanism with the failure to form blood clots in hemophiliacs due to the absence of protein factors necessary for conversion of prothrombin to thrombin. The statistics, hereditary basis, and long-term disabling consequences of hemophilia to the severely ffected patient are described. The systemic means of minimizing severe joint disabilities and serious internal bleeding hazards by employing concentrates of antihemophilic factors to reverse the bleeding defects are discussed. Availability and advantages of the types of concentr

The PGO concept: Prosthetically guided orthodontics concept.

OBJECTIVE: The aim of this case report is to present a novel digital approach of guiding orthodontics and periodontal surgery by creating interim restorations that represent the final shape of the teeth before starting the treatment. CLINICAL CONSIDERATIONS: The patient presented with worn dentition, super-erupted maxillary and mandibular anterior teeth, and altered passive eruption, all this creating a gummy smile. Following a digital wax-up design, a digital orthodontic set-up, and the digital bracket bonding, temporary restorations, and bracket indirect bonding trays were fabricated. Resto…

Evidence in Orthodontics related to qualitative research.

INTRODUCTION: Research in Orthodontics has historically followed the positivist model based on the direct relationship of cause and effect between diseases and their specific etiological factors. Despite the objectivity and the great potential of statistical procedures, quantitative methods have progressively been sharing space with other models that can encompass the multiplicity of factors that affect the health-disease process, which until such time was reduced to its biological dimension. OBJECTIVES: This study aims, through an integrative review, to identify orthodontics articles publish…

[TMJ and orthodontics, "past, present and future"].

In the past, the ATM was mainly associated with the growth of the mandibular condyle. Many studies (on rats) showed the role of condylar car … ition, occlusal adjustments at the end of orthodontic treatment. The future of TMJ in relation with orthodontics is based on prevention, screening and deepening of our knowledge. The orthodontist will thus not make a treatment in patients at risk or will identify it and finish the treatment perfectly.

Periodontally accelerated osteogenic orthodontics (PAOO) - a review.

With an increasing number of adult patients coming to the orthodontic clinic, the orthodontic professional is constantly looking for ways to accelerate tooth movement. Surgical intervention to affect the alveolar housing and tooth movement has been described in various forms for over a hundred years. However, it is the spirit of interdisciplinary collaboration in orthodontics has expanded the realm of traditional orthodontic tooth movement protocols. Periodontal accelerated osteogenic orthodontics (PAOO) is a clinical procedure that combines selective alveolar corticotomy, particulate bone gra…

[Improvement of mini-implant stability in orthodontics].

Orthodontists are continuing to increase the use of mini-implants as a source of skeletal anchorage. However, the relatively high 16.4% rate of their failures remains a problem. An analysis of articles in the literature on the loss of mini-implants shows that larger size anchorage screws and plates inserted in the anterior and median regions of the hard palate are highly stable and, accordingly, contribute to a high rate of optimum orthodontic outcomes. It is the reliability of the mini-implant/skeletal structure couple that is the principal factor in this success. With mini-implants whose hea

Orthodontics at a Pivotal Point of Transformation.

The profession of orthodontics is projected to face a multitude of challenges. Do cyclic forces accelerate the rate of tooth movement and hence the speed of orthodontic treatment? Would bioengineered cementum and dentine be a solution to root resorption? What would orthodontics be like when bioengineered periodontal ligament and alveolar bone become clinical practice, or one day, entire teeth are bioengineered? Would it be possible to selectively differentiate stem cells into osteoblasts or osteoclasts by either static or cyclic forces? What is the new demand on orthodontic expertise with incr…

The role of orthodontics in temporomandibular disorders.

Temporomandibular Disorder (TMD) is the main cause of pain of non-dental origin in the oro-facial region including head, face and related structures. The aetiology and the pathophysiology of TMD is poorly understood. It is generally accepted that the aetiology is multifactorial, involving a large number of direct and indirect causal factors. Among such factors, occlusion is frequently cited as one of the major aetiological factors causing TMD. It is well known from epidemiologic studies that TMD-related signs and symptoms, particularly temporomandibular joint (TMJ) sounds, are frequently found

Witkop tooth and nail syndrome and orthodontics.

This case report presents the orthodontic treatment of a patient with Witkop syndrome, an autosomal dominant genetic disorder characterized by the absence of several teeth and abnormalities of the nails. The patient, a 6-year 4-month-old boy, was referred to our clinic for treatment of severe overjet and openbite. Radiographic and clinical evaluations showed peg-shaped maxillary lateral incisors and the congenital absence of three mandibular incisors as well as spoon-shaped fingernails. Treatment of openbite and overjet was initiated with functional appliances, and fixed orthodontic appliances

[Orthodontics, orthognatic surgery and their functional environment].

Dento-dental and dento-maxillary dysharmonies and maxillo-mandibular dysmorphologies are impairments that can be treated either by orthodontics and surgery, or with a combination of orthodontics and orthognatic surgery. Respiration, deglutition, phonation and succion play a predominant role in the occurrence of maxillofacial dysmorphologies. Contributing factors to such impairments include upper airway function disorders such as respiration, deglutition, phonation, sucking. Abnormal functions must be extensively studied in management of maxillofacial patients in order to achieve stable results…

Tongue thrust and its influence in orthodontics.

The oral myofunctional disorder of tongue thrust has been described in various ways, such as 'deviate swallow' 'infantile swallow, and 'abnormal swallow' to name a few. The term 'tongue thrust' has been adopted, mainly because of its greater usage among authors, and since it gives a more accurate description of the lingual behavior about to be discussed. 'Tongue thrust' and 'tongue thrusting' are used in preference to 'tongue thrust swallow' or 'tongue thrust swallowing' because this behavior is generally thought to be less associated with the act of swallowing and more with the resting postur

Rare earth magnets in orthodontics: an overview.

Magnets have been used in dentistry for many years. They can be made to push or pull teeth. The force they deliver can be directed, and they can exert their force through mucosa and bone, as well as within the mouth. In orthodontics they are used for intrusion of teeth, tooth movement along archwires, expansion, retention, in functional appliances, and in the treatment of impacted teeth. New 'high energy' magnets are capable of producing very high forces relative to their size. Although magnets are potentially very useful there are a number of problems that severely affect their performance; t…

[The importance of periodontal evaluation in orthodontics].

The orthodontist is in fact a periodontal-therapist since his aim is to move the teeth with and through the periodontal tissues. It is most important for the orthodontist to be able to determine at the initial clinical examination what are the various periodontal risk factors. When the pathology is obvious with inflammation, periodontal pockets, gingival hyperplasia, edema of the papillae, gingival recessions, the need for periodontal treatment is manifest. But many times, the periodontal evaluation is complicated by the presence of slight variations of the quality of the marginal tissue that

An American Board of Orthodontics case report.

The patient's opinion of himself greatly improved following correction of a disfiguring malocclusion. He experienced a marked personality change and increased self-esteem following correction of his malocclusion. Oral hygiene was much improved when the teeth became a source of pride rather than embarrassment. The chief concern of this patient was the appearance and alignment of crooked teeth. Facial balance and esthetics were improved following placement of anterior teeth over supporting bone and reduction of the bimaxillary dental protrusion. The temporomandibular joint is asymptomatic.
