In children, general anesthesia is usually chosen as the primary technique during lower extremity orthopedic procedures. Certain situations or underlying conditions may be present, however, that make the conduct of general anesthesia more difficult or even relatively contraindicated. We present 10 patients with underlying conditions or circumstances that may increase the risk of general anesthesia. Regional anesthesia (spinal or epidural) was used to provide surgical anesthesia, thereby eliminating the need for general anesthesia. The techniques used and the applications of regional anesthesia…

Surgery of the foot constitutes a substantial portion of orthopedic procedures, performed by both orthopedic surgeons and doctors of podiatric medicine. Little research exists on the medicolegal implications of foot surgery amongst these specialties. This study seeks to investigate the different medical and legal factors associated with foot surgery-based malpractice litigation. Malpractice data between 2004 and 2017 was collected using the VerdictSearch legal database. Cases involving foot surgery were identified, and case information including physician specialty, procedure, medical outcome,…

PURPOSE OF REVIEW: Joint hypermobility can increase complications in surgical management of musculoskeletal conditions. The purpose of this review is to 1) summarize clinical outcomes in patients undergoing various Orthopedic sports medicine surgical procedures by joint, and 2) compare outcomes in patients with and without hypermobility.
RECENT FINDINGS: A total of 38 studies consisting of 144,609 patients and 144,860 joints were included (140,625 shoulders, 2,043 hips, 1,499 knees, 689 ankles, and four elbows). Notable heterogeneity exists within the literature regarding outcomes in hypermob

STUDY DESIGN: Retrospective comparative cohort study using the National Surgical Quality Improvement Program.
OBJECTIVE: The aim of this study was to evaluate trends in the annual number of PSOs performed, describe the patient populations associated with each cohort, and compare outcomes between specialties.Summary of Background Data:Pedicle subtraction osteotomies (PSO) are complex and advanced spine deformity surgical procedures performed by neurosurgeons and orthopedic surgeons. Though both sets of surgeons can be equally qualified and credentialed to perform a PSO, it is possible that dif

Von Willebrand Disease is the most common inherited bleeding disorder and can present challenges to orthopedic surgeons in managing perioperative bleeding and treating orthopedic problems that manifest as a result of the disease. Appropriate history taking is essential to identify these patients prior to surgery. The most effective management of von Willebrand disease will be achieved with close consultation between the surgeon, anesthesiologist, internist or pediatrician, hematologist, and patient. With appropriate planning, these patients can undergo major orthopedic procedures safely and ef…

BACKGROUND: Postoperative pain can lead to complications, prolonged hospital stays, and increased healthcare costs. Orthopedic surgeries are particularly associated with severe pain. This study aimed to identify orthopedic procedures that cause severe pain in hospitalized patients within 72 h post-surgery.
METHODS: Pain scores were retrospectively analyzed for patients undergoing inpatient orthopedic surgery between October 15, 2015, and December 31, 2019, at three hospitals. Patient and procedure details and maximum pain scores were collected up to 72 h postoperatively. Procedures were class…

BACKGROUND AND OBJECTIVES: Despite its clinical utility and consistent procedural volume, Medicare reimbursement for anterior cervical discectomy and fusion (ACDF) has undergone policy-driven changes over the past decade. This study investigated longitudinal Medicare reimbursement trends for ACDF among orthopedic and neurological spine surgeons from 2013 to 2023.
METHODS: A retrospective analysis was performed using publicly available Centers for Medicare and Medicaid Services databases. Surgeons who billed ≥10 ACDF procedures (current procedural terminology 22551) per year were included. Pro

PURPOSE: During the coronavirus disease (COVID) pandemic elective surgeries were cancelled and operative indications curtailed to counteract shortages in resources. We aimed to review each orthopedic operative indication at an urban Level 1 Trauma Center inundated with COVID. We aimed to classify the appropriateness of each operative intervention and determine if exposure to COVID impacted morbidity or mortality.
METHODS: All orthopedic procedures between March 16, 2020 and May 16, 2020 were reviewed. The most urgent surgical indication for each procedure was classified by 2 fellowship traine…

Reconstructive microsurgery has been an essential aspect of orthopaedic surgery and extremity reconstruction since the introduction of the operating microscope in the mid-20th century. The reconstructive ladder ranges from simple healing by secondary intention to complex procedures such as free tissue transfer and vascularized composite allotransplantation. As orthopaedic surgery has evolved over the past 60 years, so too have the reconstructive microsurgical skills that are often needed to address common orthopaedic surgery problems. In this article, we will discuss a variety of complex ortho

BACKGROUND: The MAKO Robotic-Arm system is a cutting-edge technology which combines both computed tomography (CT) scanning and three-dimensional planning to determine the ideal size and orientation of implants prior to bone resection. It is typically utilized within a general orthopedic setting for joint replacement procedures, such as total joint arthroplasties. However, its use within orthopedic oncology, which contains a much more compromised patient population and more complex surgical treatment, is not well documented within the literature.

CASES: Three patients were referred to our musculoskeletal oncology service after undergoing autologous fat grafting procedures. Two masses were suspected to be "soft tissue sarcomas," and one was due to a mass of unknown origin. These findings have not been reported in the orthopedic literature and may generate potential referrals for orthopedic oncologists.
CONCLUSIONS: Awareness of potential complications of procedures from other surgical specialties and their radiographic characteristics is of utmost importance. The clinical and radiographic findings that could assist in distinguishing a

Orthopedic procedures, although commonly associated with complications such as joint infections, deep venous thrombosis, or wound infections, rarely result in vascular complications. We present a case of a posterior tibial artery (PTA) pseudoaneurysm following blunt trauma during an elective orthopedic procedure involving the removal of osteosynthesis material. The patient developed leg swelling and intermittent hemorrhage between 1 and 3 weeks postoperatively. Doppler ultrasonography and contrast-enhanced computed tomography (CT) angiography confirmed the presence of a PTA pseudoaneurysm. The…

BACKGROUND: The purpose of this study was to describe the frequency and risk factors for orthopedic surgery in patients with achondroplasia. CLARITY (The Achondroplasia Natural History Study) includes clinical data from achondroplasia patients receiving treatment at four skeletal dysplasia centers in the United States from 1957 to 2018. Data were entered and stored in a Research Electronic Data Capture (REDCap) database.
RESULTS: Information from one thousand three hundred and seventy-four patients with achondroplasia were included in this study. Four hundred and eight (29.7%) patients had at

Orthopedic surgical procedures are consistently associated with significant postoperative pain due to extensive tissue manipulation, osseous injury, and inflammatory responses. Effective perioperative analgesia is therefore fundamental to successful orthopedic outcomes, influencing early mobilization, complication rates, length of hospital stay, and patient satisfaction. Although opioid-sparing and enhanced recovery after surgery (ERAS) pathways have gained popularity, opioids remain central to analgesic management in orthopedic operating rooms. This narrative review examines the opioids most
