Static orthoses for the management of microstomia.

Microstomia is a complication of facial burns, traumatic injuries, scleroderma, or surgical reconstructions involving the oral aperture. A variety of orthoses for the correction or prevention of microstomia are offered by dentists, occupational therapists, physical therapists, and other specialists. This paper provides an overview of the structural and clinical features of 12 common tissue-borne or tooth-borne microstomia appliances. The review is intended to facilitate the selection of suitable orthoses, and to indicate the need for interdisciplinary management of microstomia patients.

[Endoscopy-Assisted Suturectomy and Cranial Remodeling Orthosis for Craniosynostosis].

Craniosynostosis(CS)is now being diagnosed early as Japanese parents have become more conscious of the infants' skull shape. Endoscopy-assisted suturectomy(EAS)is a surgical treatment for early infants in combination with postoperative helmet cranial remodeling. Here we report our operative experiences with EAS for the treatment of CS. EAS is minimally invasive and has yielded good results when performed in combination with helmet therapy. However, transfusion for almost half of patients and additional surgery for syndromic patients were required. We will further develop the bone resection tec

Postoperative Shoulder Splint-Induced Palm Pressure Leading to Median and Ulnar Neuropathies: A Case Report.

INTRODUCTION: Postoperative positioning devices, such as shoulder splints, are commonly used to support and stabilize the upper limb following surgery. However, improper placement and prolonged pressure from these devices can lead to iatrogenic nerve injuries. CASE PRESENTATION: We present a case of severe axonal injury to the median and ulnar nerves at the proximal hand in a patient following shoulder surgery. This was attributed to palm pressure from a postoperative shoulder splint with a built-in palm stress ball designed to improve circulation and healing, along with impaired pain sensati

Muscle loading in exoskeletal orthotic use in an activity of daily living.

Strokes are the leading cause of major adult disability with up to 85% of U.S. survivors experiencing hemiparesis. Physical characteristics of upper-extremity exoskeletal orthotics, used in stroke rehabilitation, were evaluated in terms of performance of activities of daily living (ADL), perceived exertion, and muscle load. Simulated orthotic weight distributions, with total extremity loads of 0.81 kg, 1.25 kg and 2.27 kg, were evaluated along with a 0 kg control condition. Response measures included average shoulder/elbow muscle surface electromyography (sEMG) signal amplitude, quality of tas

Comparison of the Effect of the Combined Mechanism Ankle Support on Static and Dynamic Postural Control of Chronic Ankle Instability Patients.

BACKGROUND: Chronic ankle instability (CAI) is associated with postural control impairment. Orthotic devices are routinely used to improve postural control of CAI patients and prevent recurrence of ankle sprain. This study aimed to evaluate and compare the effect of combined mechanism ankle support (CMAS) with soft ankle support (SAS) and custom-molded foot orthosis (CFO) on static and dynamic postural control in patients with CAI. METHODS: Twenty-two patients with CAI and 22 matched healthy subjects were recruited. The participants were evaluated in four orthotic conditions (without orthosis…

Nonsurgical Offloading of the Diabetic Foot.

One of the most important fundamental cornerstones in treating diabetic foot ulcers involves offloading plantar pressure. Many dressings and devices have been developed with variable success rates secondary to design and patient adherence and tolerance of the designs. The current gold standard involves use of total contact casting or an irremovable cast walker. Following successful healing of a diabetic foot ulcer, devices such as custom orthotics and shoes are essential to reduce plantar pressures implicated in these wounds' recurrence.

Effectiveness of offloading methods in preventing primary diabetic foot ulcers in adults with diabetes: a systematic review.

BACKGROUND: The incidence of foot ulceration related to diabetes is increasing. Many foot care professionals recommend offloading measures as part of management strategies for modulating excess pressure to prevent development of diabetic foot ulcers (DFUs). These measures may include padding, insoles/orthotic devices and footwear. There is a lack of evidence-based guidance on the effectiveness of the different offloading options for preventing primary ulceration in those with diabetes. OBJECTIVES: To identify, critically appraise and synthesize the best available evidence on methods of offloa…

Robust controller for tremor suppression at musculoskeletal level in human wrist.

Tremor is a rhythmical and involuntary oscillatory movement of a body part and it is one of the most common movement disorders. Orthotic devices have been under investigation as a noninvasive tremor suppression alternative to medication or surgery. The challenge in musculoskeletal tremor suppression is estimating and attenuating the tremor motion without impeding the patient's intentional motion. In this research a robust tremor suppression algorithm was derived for patients with pathological tremor in the upper limbs. First the motion in the tremor frequency range is estimated using a high-pa…

Management of acquired adult dropfoot.

Dropfoot is a catchall term for ankle equinus, equinovarus, and equinovalgus. The deformity can be flexible or rigid and may be associated with other pathology. In the adult, dropfoot may be congenital or acquired. Acquired dropfoot results from weakness of the ankle dorsiflexors, overpull of the plantarflexors, contracture of the soft tissues, bony deformity, or any combination of these factors. Appropriate treatment includes observation, orthotic devices, bracing, tendon transfers, arthrodesis, and neurolysis. The purpose of this paper is to review the pathophysiology and treatment of acquir…
